Schedule: Full-Time, (Some weekend work required)
Position Key Accountabilities:
1. Performs relevant patient assessments and physical examinations.
2. Coordinates, directs, and implements patient's plan of care.
3. May make house calls, follow patient load through hospital and/or clinic episodes, or monitor in-patients in specialized settings other than surgical or acute care.
4. Orders, conducts, and interprets pertinent laboratory and diagnostic studies and procedures.
5. Makes medical recommendations and/or referrals.
6. Formulates, implements, and evaluates an outcome-based treatment plan.
7. Maintains appropriate records detailing the patient's treatment plans and outcomes.
8. Facilitates patient education relating to medical processes, medication, nutrition, and health promotion.
9. Collaborates with patients, families, other clinicians, and referral services to assist in the patient's recovery.
10. Performs other duties as assigned.
Certification/Skills:
* Registered Nurse (RN) State Licensure or Nurse Licensure Compact (NLC) by Texas Board of Nursing (BON) required
* Advanced Practice Registered Nurse - NursePractitioner (APRN-NP) by Texas Board of Nursing (BON) required
* Basic Life Support (BLS) by American Heart Association (AHA) required
* Specialty certification determined by the department required
Minimum Education:
Masters in Nursing as NP or Masters in Nursing with completion of a post masters NursePractitioner Certification Program.

Essential Functions:

Education and Experience Requirements:
Master of Science in Nursing (MSN) required. Required certification and licensure as follows: current APRN in the state of Texas, Basic Life Support, Advanced Cardiovascular Life Support, Pediatric Advanced Life Support. Two to five years' experience is recommended, preferably in the correctional or emergency medicine environment. General Position Description:
The NursePractitioner assists physicians in overseeing the clinical care and treatment of patients and families; to consult with the patients' attending physician, in consultation with the hospice physician; provide clinical direction to the Interdisciplinary Group/Team to ensure quality care. Completing F2F - Face-to-face documentation and home visits for palliation and management of the patient's terminal illness and conditions related to the terminal illness. Serves as a member of Interdisciplinary Group/ Interdisciplinary Team. The NursePractitioner reports to the Hospice Administrator.
Role Expectations:
Collaborates with the IDG/T to ensure appropriate evaluation and palliative treatment of terminal disease.
Consult with and assist in the IDG/T's development of patient/family plan of care to ensure quality care is provided.
Consults with and educates the patient/family regarding disease process, self-care techniques, end-of-life care. Provides training to other staff as needed.
Performs and documents visits to patient/family based on plan of care and patient/family need.
Interacts with attending and consulting physicians as necessary regarding palliative care, in collaboration with hospice physician.
Consults with and provides education to other IDG/T members on a prn basis for clinical issues relating to palliation.
Actively participates in the IDG/T. Communicates effectively, timely, and appropriately at all times.
Informs physicians and other personnel of changes in the patient's needs and outcomes of intervention, while evaluating patient/family response to care.
Completes face-to-face encounters with patients, as assigned, prior to 3rd benefit period and each period afterwards.
Provides clinical assessment results to hospice physician after making face-to-face encounters with patients, as assigned.
Provides medical direction and education to the clinical staff, acts as a role model and provides consultation for attending physicians
Communicates with the medical community/attending physician as needed to assure optimum patient care and acts as a liaison in the medical community.

ESSENTIAL FUNCTIONS OF THE ROLE
- Assesses the physical and mental condition of patients by performing and obtaining comprehensive or problem-focused physical examinations and medical histories.

The Medical College of Georgia is one of the nation s largest medical schools by class size, with 260 students per class. The educational experience is anchored by the main campus in Augusta, regional clinical campuses for third- and fourth-year students across the state and a second four-year campus in Athens in partnership with the University of Georgia. MCG s expanding partnerships with physicians and hospitals across Georgia currently provides about 350 sites where students can experience the full spectrum of medicine, from complex care hospitals to small-town solo practices. MCG and its teaching hospitals also provide postgraduate education to more than 500 residents and fellows in 50 different Accreditation Council for Graduate Medical Education-approved programs. Job Summary
This position works within the specified department or services and will function as an integral part of the healthcare team. Serves various roles including working directly alongside faculty and staff, providing care to the entire spectrum of patient acuity levels, working semi-autonomously and serving as a provider to initiate appropriate care for individuals with serious mental illness in an in-patient state hospital. Care involves primary care duties, including ordering routine healthcare screenings, addressing acute issues, and psychiatric mental health as needed.
Responsibilities
The responsibilities include, but are not limited to:
* The NursePractitioner in collaboration with the physician, treats physical conditions and is accountable for the delivery of primary health care to patients; collaborates with the Health Care Team to enhance the delivery of care to eligible patients in accordance with individual patient needs, using social and behavioral health protocols as general guidance. Educates nurses and other members of the health care team as needs are identified.
* Prescribing drugs for acute and chronic illness as well as physical therapy, occupational therapy and other rehabilitation treatments in accordance with Scope of Practice as allowed by Georgia Nurse Practice Act.
* Counsels and educates patients and families on health behaviors, self-care skills and treatment options, communicating as needed with the inter-professional team about the comprehensive care plan. Communicating with community referral sources to obtain information relating to patients. Provides information to other care providers about the health care needs of person being discharged. Helps to develop and maintain community and referral relationships.
* Ensures quality assurance measures are met and maintained in programmatic areas. Participates as member for performance improvement initiatives as needs arise; may assist in staff training, education and mentoring to improve quality care; and assists in the education and preceptoring of students.
* Collaborates with nursing leadership to identify and deliver ongoing educational programs. May perform direct teaching or provide guidance to others in providing these services; responsible for assuring that education and training content meets professional standards.
* Licensure: CME and Grand Rounds.
* Charting.

Required Qualifications
Educational Requirements
Master's degree in Nursing from an accredited NursePractitioner program. Active Advanced Practice Registered Nursing Licensure in Georgia. Current certification to practice as a Family or Adult NursePractitioner through approval by the Georgia Nursing Board. Qualified to obtain prescriptive authority in the State of Georgia. Ability to meet requirements for hospital medical staff credentialing. Two years' experience in professional nursing. Ability to formulate diagnoses based on examination and interpretation of test results. Ability to identify deviations from the norms and intervene or make appropriate referrals. Ability to complete nursing assessments, develops and implements nursing care plans, and revise plans as appropriate. American Heart Association BLS Certification.

We are seeking a dedicated Full-Time Acute Care NursePractitioner to join the UTHealth Department of Internal Medicine - Hepatology in Houston, TX. In this vital role, you will provide advanced, patient-centered nursing care to individuals with complex acute, critical, and chronic health conditions in both clinic and hospital settings. Working collaboratively under physician direction, you will support specialized hepatology care, contribute to high-quality treatment plans, and help optimize outcomes for a diverse patient population.

Position Key Accountabilities:
* Conducts complex, standardized patient care procedures which require experience and specialized training which would otherwise be performed by a physician, but which do not constitute the practice of medicine.
* Conducts independent and physician supervised rounding on unit.
* Performs relevant patient evaluations and assessments, and provides appropriate treatment; including monitoring and ordering medications.
* Coordinates, directs, and implements the patient's plan of care.
* Assists in collecting patient histories, screens for co-morbidities, and provides guidance and preventative health care as needed.
* Performs physical examinations and provides episodic care, or problem-oriented care, under the supervision of a physician.
* Orders appropriate laboratory, diagnostic and radiographic tests, and interprets results.
* Monitors the treatment program of the patient to inform the attending physician of pertinent medical information.
* Facilitates patient education relating to medical processes, medication, nutrition, and health promotion or preventative health care.
* Assists physicians in formulating therapeutic decisions.
* Maintains appropriate records, and prepares charts for review and counter signature by physician.
* Prepares and cares for supplies and equipment.
* Responds to emergency situations.
* Performs within the scope of practice prescribed by licensure and certification.
* Performs other duties as assigned.
Certification/Skills:
* Current Texas Licensure as a Registered Nurse
* Current Texas Licensure as an APRN-NP
* Current specialty certification as required by department, excluding FNP certification unless paired with a post master's certification in specialty.
* Basic Life Support (BLS) certified or specialty certification as required by department.
Minimum Education:
* Masters in Nursing as NP or Masters in Nursing with completion of a post-masters NursePractitioner Certification Program.
Minimum Experience:
* None; previous experience in area of clinical focus preferred.
Physical Requirements:
* Exerts up to 50 pounds of force occasionally and/or up to 20 pounds frequently and/or up to 10 pounds constantly to move objects.
