Collaborative Care Mental Health Clinician

The Collaborative Care Mental Health Clinician (CCMHC) is an integral member and provider of a patient's medical and mental health care team. They are an embedded mental health profession within the primary care clinic. This role is weighted toward direct clinical care with 90% of the interactions being face-to-face patient care. The role serves as the cornerstone of the team consisting of a patient, the patient's primary care provider, and a psychiatric consultant. The CCMHC also contributes to the learning, growth, and service provision of the larger Collaborative Care Psychiatry team.
The CCMHC is responsible for providing triage, clinical assessment, direct diagnosis of mental health disorders in conjunction with DSM, ICD-10 and Collaborative Care and/or Psychotherapy codes, psychoeducation to patients and primary care providers, care coordination, case management, direct evidence-based psychotherapy, treatment plan formulation and updates, crisis coordination, and relapse prevention planning.
The CCMHC is responsible for leading regular case consultation with their partner psychiatric consultant and monitoring a caseload of assigned patients. They are also responsible for proactively identifying patients who would benefit from their services via available population health measures.
The CCMHC is responsible for oversight of a registry which serves as both a clinical tool (e.g. monitoring risk scores and patient outcomes) as well as a programmatic tool (e.g. monitoring service utilization and programmatic outcomes).
EDUCATION:
Must have the necessary education (Master's degree minimum) with Licensure in the State of Vermont LICSW, LCMHC, LMFT required.
PREFERRED:
Experience with pediatric population as well as adults
Training in acute crisis intervention
Training in suicide risk assessment and symptom reduction
Training and experience in CBT/DBT approach to mental health symptom reduction or alleviation
Additional clinical certifications and skills in specific clinical specialties are welcomed
Familiarity with psychotropic medications and their role in treatment

EXPERIENCE:

Staff Sonographer - OB/Gyn Per Diem

The Sonographer Staff performs a variety of routine and complex imaging procedures using high frequency sound waves. Since ultrasound can be degraded or blocked by various tissues and materials including lung, bowel gas and bone, obtaining a sonogram is a tailored examination requiring technical skill to adjust the machine, experience to find appropriate "windows" that can transmit an undistorted ultrasound beam, and diagnostic knowledge to obtain extra images customized to the diagnosis of concern and to abnormalities seen while obtaining the basic image set. This need to heavily customize the examination to each patient and to recognize and diagnose pathology separates the sonographer from other imaging technologists in radiology.
The Sonographer Staff must be registered by the ARDMS (American Registry of Diagnostic Medical Sonographers).
EDUCATION:
High school graduate or equivalent, graduate of a certified CAAHEP school of Diagnostic Medical Sonography, Radiologic Technology or a two year allied health equivalent. Must be ARDMS registered with an ABD or OB/GYN credential.

Our Network Department of Emergency Medicine provides clinical coverage for the University of Vermont Health across hospital sites in Vermont and northern New York. Collectively, we provide service for approximately 200,000 patient visits annually, and our hospitals serve a catchment of 40,000 square miles and 1.4M people. LCOM students and emergency medicine residents train clinically at three of our sites with elective rotations at the other sites. Our primary teaching campus is the University of Vermont Medical Center, the only Level 1 Trauma Center in the greater region. The majority of our academic faculty provide clinical coverage at multiple sites, including our rural and critical access sites in both states, underscoring our commitment to high quality rural acute care delivery.

Pulmonary Function Technologist II (Respiratory Therapist)

The Pulmonary Function Technologist II (PFT) independently provides all modes of pulmonary function testing as ordered by a physician, assists with bronchoscopies, interventional bronchoscopies and other invasive procedures, and monitors and recovers patients after treatment.
EDUCATION:
Graduate from an accredited Respiratory Program.
Registered Respiratory Therapist by the NBRC.
Licensed Respiratory Care Practitioner in Vermont.
Certified Pulmonary Function Technologist within one year of hire date.
Registered Pulmonary Function Technologist, preferred.
Current BLS and ACLS certification

EXPERIENCE:
Two years of experience in a PFT lab preferred.
Bronchoscopy, Interventional Bronchoscopy and Cardio-Pulmonary Stress testing preferred
SHIFT NOTE:
9-hour shifts - 4 days per week, rotating late days to support bronchoscopy procedures.
SIGN ON BONUS:

Unit Description: Emergency Department RNs provide professional nursing care to patients in varying state of health and illness by assessment, planning, implementation, and evaluation of the nursing plan of care.
