+ Coordinates post-acute discharges for complex patients in collaboration with Care Management Discharge Manager.
+ Identifies the need for and conducts in a timely fashion patient family meetings that result in decisions regarding advance directives, comfort measures, power of attorney, guardianship, conservatorship, and goals of care.
+ Completes initial psychosocial screen of patients and families as indicated. Serves as the lead in addressing psychosocial needs of patients relating to social determinants of health, barriers to equal access to healthcare, and patient progression; this includes obtaining charity and financial resources, legal guardianship, adoptions, psychiatric referrals, and competency determination.
+ Provides referrals for post-acute transitions to/for LTACH, SNF, IRF, LTC , HH and DME.
+ Conducts practice consistent with social work ethical principles, adhering to standards set forth from NASW and ACMA Case Management practice standards.
+ Identifies the need for and conducts family meetings that result in comfort, treatment and discharge planning decisions, and other important outcomes.
+ Leads Care Coordination/Interdisciplinary Rounds and documents.
+ Advocates for patient care and timely discharge plan.
+ Works with people and agencies in the community to improve responsiveness, capabilities, alignment, and evaluation of services to patients and families.
+ Utilizes age-appropriate assessments and interventions during all client contacts.
+ Collaborate with Risk Management, Patient Relations, Utilization Management, the Ethics Committee and other departments for ethical issues and utilizes the NASW Code of Ethics in appropriate decision-making.
+ Demonstrates flexibility and partnership with the care management team members to ensure the needs of patients are met.
+ Assists patients and families in understanding their illness and treatments options, consequences to various treatments or refusal of treatment, and necessary levels of care, including acute, subacute, and community services.
+ Assists patients and families in communicating with treatment team
+ Educates hospital staff on patient psychosocial needs.

MINIMUM REQUIREMENTS
Education: Master's of Social Work required.

General Summary: Serve as a financial aid mentor and provide support for the Pittsburgh Job Corps cohort of students at each of the CCAC campuses with matters pertaining to the Financial Aid program. Monitor the academic progress of students as it pertains to financial aid eligibility.

General Information:
The Dean of Students Case Manager is primarily responsible to support the students who are referred to Madison Cares and are at a low level of concern (students do not appear to pose threat of harm to self/others, nor significant disruption, but may need resources). The Case Manager will also provide significant support to students who have withdrawn from JMU by partnering with the Office of the Registrar to send emails and call students who have questions regarding the withdrawal process and may need support regarding re-entry.

Madison Cares Support:
* Responsible for helping with case management for the university's care referral program, Madison Cares.

This position serves as a liaison between bicultural families and Arlington Public Schools and programs. In partnership with others, the Bilingual Family Specialist supports families through building relationships and cultural understanding, facilitating communication on behalf of the school and/or district, empowering parent, and community involvement, and providing school/community resources.
REQUIRED EDUCATION, TRAINING AND EXPERIENCE:
High School Diploma or General Education Degree (G.E.D.) and two (2) years of progressively responsible experience in social services, community relations, or related fields is required.
Bachelor's Degree in Sociology, Psychology, Social Work or Education and three (3) to five (5) years of progressively responsible experience in social services, community relations, or related fields is preferred.

ESSENTIAL FUNCTIONS OF THE CLASS (May not include all duties)
Welcomes new immigrant families, assisting with enrollment, providing school tours and key information related to the school, district, and educational programs, while building connections between school staff members, family and community.
Actively, dependably, and confidentially manages messages, questions, and responses to and from school staff and parents in a timely manner.
Accurately communicates student need(s) to appropriate school support staff based on parent or student disclosed information while maintaining confidentiality.
Provides, facilitates, and/or leads parent workshops in native language and/or English to increase parents': familiarity with school expectations; ability to navigate the school system; confidence participating in leadership roles; and ability to monitor child's academic progress. Assists parents in engaging in academic planning for their child's future success.
Plans, schedules, organizes, informs and participates in delivering a variety of school or community outreach programs, including one-on-one consultations, small meetings, presentations, conferences, workshops for families and staff members and/or mass communication efforts.
Establishes/Maintains regular parent group meetings to allow exploration of parent interests or needs through providing presentations by invited speakers from the school and community.
Advocates for and supports parental engagement, fostering parent leadership and participation in child's academic progress and growth.
Identifies, and connects families to potential resources for a variety of needs including, but not limited to: academic, behavioral, mental health and/or family support. These resources may be located within the school, district, or community.
Performs other duties related to assisting families through coordination with other staff or agency representatives and/or community-based organizations/community resources (e.g. Help families with unexpected emergencies of medical, stability, transportation needs, etc.)
Assists in providing basic technological support and guidance regarding connectivity to ensure parents can access and use platforms to view report cards, report absences and navigate student learning platforms (e.g.: ParentVue)
May be called upon to interpret for various school meetings and programs.
Facilitates parent conferences through interpretation, assisting in scheduling additional interpreters and building capacity among school staff in utilizing communication tools such as Language Line or Talking Points.
Assists parents in understanding and completing various APS Surveys, questionnaires, and district community engagement activities. Works to ensure diverse voices are heard.
Attends monthly professional development meetings with central office staff, assigned schools and appropriate training opportunities as assigned.
Collaborates to support Parent Teacher Association (PTA) meetings.

Position is responsible for K-12 English as a Second Language initial testing for possible student placement in an ESL program. Position provides training to ESL staff on assessment instrument used and maintains testing database. Position processes all testing paperwork and works with all schools and families with communicating proper ESL placement.
Essential Duties
Serves as liaison between student, families, school, home, and the ESL Welcome Center.
Cooperates with administrators and staff in implementing the ESL program.
Adheres to all federal and state ESL guidelines pertaining to ESL assessment.
Administers all initial ESL language assessment components to incoming students K-12 that may qualify for ESL as well as support for division-wide WIDA Assessments in the spring.

Chief Objective of Position
The Department of Counseling Services is seeking a Licensed Professional Counselor(s) (LPC) to assist students in defining and accomplishing personal and academic goals. To provide counseling, including on-call crisis intervention, to assist students in overcoming specific personal, educational or behavioral problems that interfere with healthy living and success at the university. To support the development of students by helping all students enhance their growth potential and thereby benefit from their academic and social environment.
Work Tasks
1. To provide individual and group counseling, consultation, and outreach focused on developmental needs and issues of students.
2. Provide on-call crisis counseling and intervention support to students in need.
3. Provide psycho-education about anxiety, depression, stress, emotional regulation, relationships, eating concerns, substance abuse, sexual assault and other topics of concern to the welfare of students.
4. Support student adjustment and student learning in the residence hall through appropriate outreach and educational programs and services.
5. Support the training of the campus community related to suicide awareness and appropriate referral, listening skills, conflict management, crisis intervention and other topics as needed.
6. Provide consulting services as needed concerning student issues related to counseling, development and behavior management in the residential community.
7. Develops and maintains very positive and professional customer service and relations with all constituencies to include students, faculty, staff, administrators, other employees, other agency personnel and community members. Demonstrates a positive and professional attitude and treats everyone with dignity and respect. Fully supports the "Student's First" value at CNU and routinely goes the extra mile in providing service. Completes all work in a timely, efficient and accurate manner.
8. Safety issues are reviewed and communicated to assure a safe and healthy workplace and a reduction in work related absence.
