ESSENTIAL FUNCTIONS
Coordinates patient referrals by coordinating multiple referral sources (telephone, electronic medical record, etc.) across multiple locations within the UMMS system (UMMC downtown and Midtown locations only).
Works closely with Medical Director and other site Clinical Directors to ensure appropriate referral of patients within the WMHRP program.
Maintains referral database to track patients connected to care and lost to follow-up and using database to analyze barriers to care; monitors referral flow, time to next appointment, and management of calls.
Implements and monitors quality assurance indicators, which may include timeliness of first appointment, case management assessment procedures, timeliness of diagnostic evaluations, and completeness of assessment procedures.
Assists Medical Director with disseminating information at staff meetings, interpreting policies, reporting on progress of the programs, and providing continuity among staff members between all program locations.
Assists Medical Director with monitoring the environment of care across multiple sites and addressing any issues that occur for the WMHRP program.

General Summary
The Clinical Exercise Physiologist (CEP), under limited supervision, is responsible for the assessment of patients including the development, evaluation, and modification of the plan of care. The CEP interprets and performs complex patient care procedures specific to their area of expertise.

Minimum Qualifications:
Education/experience/licensing:
• Current physical therapy licensure from the Ohio State Board of Occupational Therapy, Physical Therapy, and Athletic Trainers.
• Graduate of an accredited physical therapy program.
• Obtains/maintains CPR Certification from the American Heart Association prior to the completion of probationary period. American Red Cross CPR Certification is acceptable for hire.
• Passes annual safety test.
• Achieves competencies for assigned work area as dictated by director (i.e., age specific, suctioning, aseptic technique).
Communication and other skills:
• Good written and verbal communication skills to allow for functioning as a team member, assure adequate documentation for payors, and to ensure continuity of care.
• Adheres to UTMC service values.
Physical Requirements:
• Visual acuity of 20/20 or corrected to 20/20 including color vision when applicable;
• Ability to hear (with or without aides);
• Ability to orally communicate;
• Good manual dexterity;
• Ability to push, pull, roll, and transfer/lift 50 pounds occasionally;
• Ability to stand, walk frequently;
• Ability to bend, reach, stoop, kneel frequently;
• Ability to perform CPR (if a required for position).

Position Summary
The Extra Help Physical Therapist is responsible for the provision of physical therapy services within her or his individual scope of clinical competence including: examination, evaluation, diagnosis, prognosis, development and implementation of a plan of care that includes direct intervention, communication, collaboration and patient education and the evaluation of outcomes.
Duties & Responsibilities
Performs the duties of a professional Physical Therapist for an assigned caseload with emphasis on providing comprehensive care. The PT is responsible for upholding professional standards and promoting quality in practice. Direct Patient Care time (actual time with patients, may vary from scheduled) - includes all direct interventions and patient education that is integrated as a part of treatment session Indirect Patient Care (IPC) - time spent in chart review, documentation (direct patient care documentation as well as any other documentation related to patient care), scheduling, attending team or family meetings and rounds (documented as clinic time), ordering equipment, assisting other staff Responsible for administrative tasks related to the patient care services they provide, related to department operations, or required for regulatory and organizational compliance. This includes time sheets, case presentations, staff meetings, committee meetings, giving inservices. Responsible for professional development and skills for self as well as sharing expertise and participation in development of the role of physical therapy throughout the health system and within the community. Responsible for clinical teaching for students and student clinicians. May provide academic teaching Other related duties as assigned. (Alternative productivity expectations may be determined within this classification based on fluctuating needs of the department) Perform other related duties and participate in special projects as assigned.
Minimum Qualifications
* Current licensure or eligibility for licensure as a Physical Therapist from the Illinois Department of Financial and Professional Regulation at time of employment (if not licensed at time of employment, passing score on examination for licensure is needed within two months)
