Essential Functions:
Under the supervision of a clinical nurse manager, this position will provide nursing care to patients, and:
* Conducts systematic, continuous and comprehensive assessments of patient health status from admission or encounter to discharge across the care continuum to ensure patient safety, collaborating with the healthcare team, patient and family; care addresses all aspects of the individual and family, including age, gender, spiritual, cultural, physical, psychosocial and patient preferences and proactively uses safety surveillance to prevent adverse outcomes
* Independently administers medications per licensure authority, assesses and reassesses the effects of these medications, and documents administration in a timely manner per policies, procedures and regulatory requirements; consistently adheres to patient safety guidelines regarding all aspects of medication administration

Summary:
Acts as a lead and resource to the Bariatric Navigation staff as directed. Serves as a point of contact for the Bariatric Care Continuum, outside departments, patients and caregivers to provide resources and assistance with accessing clinical and supportive care services offered within Virtua and in the community. Supports the optimal team efficiency by assisting with scheduling, workflow management and quality assurance. Acts as a customer liaison, providing excellent service to all callers and projecting a professional, positive image of self and Virtua.
Position Responsibilities:
Assists with managing daily operations and productivity. Evaluates data to manage departmental workflow and service levels so the staffing is optimal to meet performance requirements. Make suggestions for process improvement. Assist in training new clinical staff and mentor less experienced co-workers.
Daily quality assurance monitoring of clinical staff and providing manager with weekly reports of staff effectiveness along with conducting team meetings. Hold coaching sessions with clinical staff to ensure ultimate productivity and customer satisfaction.
Increase Virtua's business and patient satisfaction by using excellent customer service skills to navigate patients within Virtua and coordinate all appointments, register for classes and Virtua programs. Coordinates multidisciplinary planning conferences, develops concise patient summaries for use by the care team, and documents recommendations made utilizing standardized care protocols in accordance with nationally recognized care guidelines.
Responsible for outreach efforts to establish and maintain positive working relationships with key customers (physicians, office staff, diagnostic staff, nurses, radiology staff, social services staff, and radiation oncology staff, etc.)
Make daily outbound calls to ensure timely patient access through Virtua's system along with protecting confidentiality of patient.
Perform data entry and complete all data records with concise patient information and appropriate coding to ensure proper tracking of leads. Understanding of all databases used such as IBEX, Sorian Scheduling, Sorian clinicals and TFB/NEC phone system.

5 Towers is a 21-bed advanced heart failure intermediate unit specializing in congestive heart failure, pre- and post heart transplant, left ventricular assist devices, and the treatment and management of pulmonary arterial hypertension.
About Jewish Hospital
Jewish Hospital, part of UofL Health, is an internationally renowned, high-tech tertiary referral center, developing leading-edge advancements in hand and microsurgery, heart and lung care, orthopedics and sports medicine, neuroscience, organ transplantation and outpatient care. The hospital is the site of the world's first successful hand transplant and AbioCor implantable replacement heart procedures, in addition to the first trial of adult cardiac stem cells in chronic heart failure. Education:
* Associate's Degree in Nursing from an accredited nursing program is (required)
* All new-hire Associate Degree Registered Nurses (new graduate and experienced) are required to start an approved BSN program within one (1) year of hire and complete an approved BSN program within three (3) years of hire (required)
* Bachelor's Degree in Nursing from an accredited nursing program (preferred)
Experience:
* All new RN graduates will participate in the Nurse Residency program and must successfully complete all activities (required)
* Two years of hospital patient care experience (preferred)
Licensure:
* Current KY RN licensure or compact license with privileges to work in Kentucky (required)
Certification:
* Basic Life Saving (BLS) accredited by the American Heart Association (AHA) (required)

The RN team leader is accountable to his/her area of specialty for planning, organizing, directing and controlling activities concerning his /her specific service. The RN team leader maintains standards of quality patient care on the assigned unit in an effective and efficient manner for the adolescent (ages 12-18), adult (ages 19-65), and geriatric (ages 66 and over) patient. Through the assessment of patient needs, the RN team leader assigns duties, coordinates work efforts, promotes communication, maintains harmonious relationships among staff and promotes a safe, comfortable atmosphere for patients in cooperation with the appropriate departmental supervisor.
Essential Functions:
* Coordinates resources to ensure necessary equipment and supply inventory is maintained and procured for the specific services (Physician, vendors, blood bank, sterile supply)
* Reviews weekly and daily schedules for case preparation
* Uses problem-solving and conflict resolution skills to foster effective work relationships with team members
* Demonstrates ability to set priorities in the delegation of quality patient care
* Assists the charge RN/Manager/Director with appropriate input concerning staff performance around specific issues and communicates situations and activities which may have impact on the smooth operation of the department
Other Functions:
* Participates in Quality Improvement efforts for evaluation of patient care and outcomes
* Responds to reported changes in the patients' behavioral and physiological status by evaluating and taking appropriate action, reports such changes to appropriate people and documents accurately
* Responds immediately and appropriately to all patient emergencies
* Accountability for maintaining and improving knowledge and skill level including required competencies
* Maintains compliance with all company policies, procedures and standards of conduct
* Complies with HIPAA privacy and security requirements to maintain confidentiality at all times
