This position is also responsible for coordination of interdepartmental functions and activities to ensure smooth and effective operations consistent with the objectives of the department, the facility, and of the Frazier Rehab Institute.
The lead therapist should all demonstrate all competencies and skills of staff therapists. Lead therapist is responsible for the evaluation and treatment of patients referred to the facility. This includes patient evaluation, development and implementation of an appropriate treatment plan, delivery of treatment services, and comprehensive discharge planning. The therapist functions as an effective team member and contributes to the development and achievement of department and organizational goals and objectives.

Job Role Summary
The Physical Therapist I is accountable and responsible for provision of physical therapy services to patients of Eskenazi Health. This position routinely treats adult and geriatric patients; may periodically treat neonatal, pediatric and adolescent patients.
Essential Functions and Responsibilities
Evaluation
Adapts evaluation appropriately based on the age and cultural background of patient.
Reviews medical history and adjust clinical decisions based on information
Completes thorough and effective evaluation
Administers appropriate standardized and non-standardized assessments
Develops age appropriate and reasonable plan of care
Sets reasonable and appropriate client-centered goals
Treatment:
Adapts treatment appropriately based on the age and cultural background of patient
Implements appropriate and timely treatment based on evaluation of patients' physical and psychosocial needs
Provides client-centered education
Treatments are creative and effective
Documentation:
Accurately and timely documents patient evaluation and treatment per documentation standards
Obtains appropriate signatures
Justifies charges, reflects skilled therapeutic need, functional improvement and progress towards goals
Productivity:
Works at or above established productivity level
Volunteers to take walk-in patients
Volunteers to float to other teams as needed
Adjusts schedule according to caseload
Administrative:
Completion of ancillary tasks that affect patient care
E-learning and other assigned tasks are completed by the required due dates without reminders
Authorization requests and insurance requirements are submitted without delaying patient care
Professional Communication/Collaboration:
Demonstrates effective communication and teamwork with peers and other staff
Consults appropriately with physicians and caregivers regarding patient management
Delegates routine aspects of care as appropriate

Job Requirements
Graduate of APTA approved curriculum.
Licensed as a Physical Therapist in State of Indiana or eligible to do so.

The Sandra Eskenazi Mental Health Center offers both inpatient and outpatient services, including several outreach centers as well as clinic- and community-based services.

The Focus of Adult Addictions
The Adult Addiction team provides services to adults 18 & up with a primary diagnosis of substance use disorder and those with co-occurring disorders. We believe that recovery is 100% possible, and our goal is to partner with the client to help them reach their recovery goals. The Adult Addictions Clinic provides a combination of services, including peer support, individual therapy, couples/family therapy, case management, skills training, medication management, and groups. The services provided are based on the client's individual needs and treatment plan. The Adult Addictions clinic is located in our Thomas P. Stitt Building at 3171 N. Meridian.
Where You Fit In:
The Licensed Therapist - CCBHC performs a variety of individual, family and group therapeutic interventions utilizing evidence-based practices (“EBP”) specific to Certified Community Behavioral Health Clinic (“CCBHC”) certification. This position manages a caseload of clients by identifying therapeutic, rehabilitative and case management needs, engaging other team members in meeting these needs as indicated.

A key aspect of your role involves collaborating with other team members to address and fulfill these identified needs. This collaborative approach ensures a comprehensive and coordinated effort in meeting the varied requirements of the clients under your care. Am I Qualified?
• Master's degree in a behavioral health or human services that meets Indiana licensure requirements for the following associate licenses:
o Licensed Clinical Social Worker (LCSW);

o Licensed Mental Health Counselor (LMHC)
• Active and unencumbered clinical licensure in the State of Indiana (LCSW, LMFT, LMHC)

Essential Functions and Responsibilities
• Embraces, understands, and operates under the Recovery Model, including AIDET (Acknowledge, Introduce, Duration, Explanation, Thank You) and the spirit of motivational interviewing
• Adheres to the regulatory requirements for CCBHC regarding client treatment, including crisis prevention planning and crisis safety planning
• Utilizing advanced certifications and skills in EBPs, delivers treatment to fidelity of therapeutic interventions and treatment models specific to the population served.
• Prepares clinical treatment plans and completes biopsychosocial evaluation (“BPSE”), integrating information gathered by the team and developing the integrated diagnostic summary based upon the BPSE and in consultation with the client, family, treatment team.
• Manages a caseload of clients by identifying therapeutic, rehabilitative and case management needs; engages other team members in meeting these needs as indicated; provides quality case management services as indicated by client need; monitors client progress and reflects this in the clinical documentation
• Performs a variety of individual, family and group EBP therapeutic interventions utilizing EBPs specific to CCBHC certification
• Evaluates and updates the treatment plan at intervals determined by guidelines, consulting with client family and team in this process
• May provide care coordination, skills training and development to clients in group and individual settings as guided by medical necessity outlined in the person-centered treatment
• Provides crisis management and emergency interventions including but not limited to cardiopulmonary resuscitation and approved area-specific crisis intervention techniques
• Reports to treatment team in staffing including but not limited to successes and challenges, assessment of client progress, barriers to success, any level of care changes, etc.
• Advocates for clients to ensure access to needed services which include developing community relationships and resources
• Meets the agency's standard of providing the appropriate service at the right time, right place, and right frequency as determined by the client's level of need
• Meets the agency's standard of timely completion of all documentation including interpreting and applying service and billing procedures according to client needs as well as completing billing according to guidelines for timeliness and accuracy
• Serves as a clinical resource for less experienced staff
• Establishes and maintains a safe environment at all times through on-going assessment of milieu/appointment environment and intervenes as indicated in person-centered treatment plan as well as by nursing or physician order
• Identifies any high- risk client symptoms or circumstances that would result in risk for self-harm or harm to others and notifies appropriate higher level clinical staff for consultation
• Continuously evaluates clients' mental status, reporting progress to the team and treating psychiatrist

Graduate of accredited program of physical therapy, culminating in a minimum of a Bachelor's of Science Degree

Peace - Louisville Address: 2020 Newburg Rd. Louisville, KY 40205 Shift: Weekend Days (United States of America) The Mental Health Technician II provides delegated components of nursing care to patients including monitoring, specimen collection, psycho-education, assistance with activities of daily living and behavior management in accordance with physician orders and established nursing policies and procedures. Care provided is based on defined standards of patient care and practice.
:
Essential Functions:
* Observes and monitors patient's status according to patient monitoring standards, policies and procedures. This includes completion of vital signs, weight log, intake and output, dietary intake, rounds sheets, behavioral rounds sheets), narrative logs and admission checklists or other progress or milieu notes. Reports patient complaints of pain or other concerns in a timely manner.
* Provides verbal and written input to nursing staff via ongoing reporting and end of shift report so patient's needs and care can be prioritized.
* Provides skilled patient care as regarding: orientation to unit; transportation; de-escalation; teaching of coping skills, conflict resolution, problem solving skills; specimen collection; assistance with activities of daily living; admission & discharge procedures; response to codes; and, use of SCM techniques.
* Observes and evaluates patient/family response to the interventions on an on-going basis and reports changes in condition to the nurse assigned.
* Leads patient psycho-educational groups and other care activities on unit that are consistent with Personal Intervention Plans used on the specialty units.
Shift Requirements:
Shift Length (in hours): 8 hours or 12 hours
# Shifts/Week: 3-5 shifts depending on shift length

* Maintains professional development and practice requirements including certifications, CEUs and associated education.
* Maintains compliance with all company policies, procedures and standards of conduct.
* Complies with HIPAA privacy and security requirements to maintain confidentiality at all times.
* Performs other duties as assigned.
Required Qualifications:
In addition to the certification and training requirements listed below, indiviudals must possess one of the three following education and work experience combinations to be eligible for the position of Mental Health Technician II.
* High School Diploma, GED or Equivalent and 2-years social service experience, 1-year being in direct patient care; or,
* 60+ college credit hours in a social services field and 1-year direct patient care experience; or,
* Bachelor's Degree in a social services field with direct patient care experience preferred but not required.
