You will conduct and evaluate medically prescribed physical therapy treatment programs for our patients from the comfort of their own homes. Most patients treated in this area need orthopedic care, with the remainder being various chronic disease patients.
**A caregiver in this position works variable shifts between 8:00AM - 4:00PM, depending on patients' needs. Schedules will be received the night before each workday.**
A caregiver who excels in this role will:
+ Perform initial and ongoing assessments of patient's condition.
+ Establish, revise, and evaluate a plan of care which is appropriate to problems identified and involves the patient/family according to their capabilities and desires.
+ Perform therapy interventions using standard physical therapy techniques and skills appropriate for the patient's age and condition.
+ Supervise activities of physical therapy assistants, students, and other support personnel.
+ Maintain complete and up-to-date documentation and files regarding patient condition and effectiveness and timeliness of care and interventions.
+ Participate in ongoing interdisciplinary assessment of assigned patients.
+ Participate as a member of the hospice interdisciplinary team when necessary.
Minimum qualifications for the ideal future caregiver include:
+ Bachelor's Degree or certificate
+ Graduate of a Physical Therapy Program accredited by the American Physical Therapy Association
+ Valid Ohio Driver's license upon hire; per policy 8-175 thereafter
+ Proof of automobile insurance coverage with $100,000/$3000,000 coverage, upon hire; per policy 8-175 thereafter
+ Current State licensure as a PhysicalTherapist (PT)
+ Current Basic Life Support (BLS) Certification
