    The Assistive Technology Assistant is responsible for the day-to-day operations of the Upstate Caring Partners Technology Program (TRAID) office including customer service, data management and equipment maintenance. Also responsible for the day to day operations and management of the Assistive Technology Grant (ATG). This candidate will be able to work independently with limited direction after training. CORE RESPONSIBILITIES * Assist with the day-to-day in-office customer service; including arranging for equipment delivery/pick up. * Assist with maintaining a contact list and conduct quarterly mailings for information dissemination as specified in the DBTAC (NEADA/IT) grant. * Maintain records of customer contacts (phone calls, demonstrations, loans, outreach activities) and mailings. * Implement follow-up procedures and customer satisfaction surveys. * Assist with maintenance, cleaning and tracking of equipment; including obtaining supplies necessary for equipment maintenance. * Maintain a catalogue/reference library. QUALIFICATIONS * Associate's Degree in Human services or related field. * Strong knowledge of a variety of assistive technology devices in the required domains of education, community living, employment and information technology/telecommunications, as well as disabilities, and funding sources. * Intermediate Computer Skills. * Must have a valid NYS Driver's License. You'll provide comprehensive mental health and substance use assessments, treatment planning, psychotherapy, crisis intervention, and care coordination for individuals with co-occurring disorders-all while collaborating with a multidisciplinary Integrated Care Team. Full-Time and Part-Time Opportunities available - This is a primarily on-site position at one or more of our Utica, Rome, and/or Herkimer locations. Hybrid work (1-2 days per week) may be approved based on role expectations, performance, and program needs. The final work location arrangement will be determined collaboratively during the hiring process. Position Summary The Behavioral Health Professional delivers integrated, person-centered, and recovery-oriented behavioral health services within the CCBHC model. This position provides comprehensive mental health and substance use assessment, treatment planning, psychotherapy, crisis intervention, and care coordination for individuals with co-occurring disorders. Services are provided in collaboration with a multidisciplinary Integrated Care Team and in accordance with CCBHC requirements, evidence-based practices, and all applicable regulatory and licensure standards. Core Responsibilities Clinical Assessment & Treatment * Conduct timely and comprehensive intake assessments, biopsychosocial evaluations, and suicide and substance use risk assessments using CCBHC-required and evidence-based screening tools. * Complete standardized screening tools (e.g., PHQ-9, PCL-5, C-SSRS, and SCARED) to support diagnostic clarification and ongoing clinical care. * Develop, implement, and regularly update individualized, person-centered treatment plans that address mental health, substance use and/or social determinants of health-related needs, in collaboration with clients and their Integrated Care Team. * Monitor client progress, outcomes, and risk indicators; adjust treatment interventions as clinically indicated. * Provide individual, group, and/or family psychotherapy using evidence-based modalities including Cognitive Behavioral Therapy (CBT), Motivational Interviewing (MI), and Integrated Dual Disorder Treatment (IDDT), or Supportive Counseling approaches as needed. * Deliver substance use counseling, relapse prevention interventions, psychoeducation, and recovery-oriented supports tailored to client needs and stages of change as needed. * Participate in crisis response activities, including on-site or telephonic/video support, in alignment with CCBHC crisis response expectations. Integrated Care & Care Coordination * Collaborate closely with the multidisciplinary Integrated Care Team, including medical providers, targeted case managers, peer support specialists, and other behavioral health staff, to ensure coordinated and whole-person care. * Communicate regularly with clients and, when appropriate, families or identified community supports to promote engagement, retention, and adherence to treatment plans. * Coordinate referrals and warm handoffs to internal CCBHC services and external providers, including inpatient psychiatric care, detoxification, residential treatment, and community-based supports. * Participate in interdisciplinary collaboration with prescribing providers to support integrated, person-centered treatment planning in accordance with CCBHC standards. * Engage in community outreach and partnership activities consistent with CCBHC access and continuity-of-care requirements. * Assist with social determinants of health-related needs as needed. Documentation, Quality & Compliance * Complete accurate, timely, and compliant clinical documentation (assessments, treatment plans, progress notes, discharge summaries) in accordance with CCBHC standards, clinic policies, and state and federal regulations. * Ensure all services are delivered within the clinician's scope of practice and consistent with licensure, ethical standards, and agency policies. * Participate in utilization review, quality improvement activities, and data-driven care initiatives as required by CCBHC oversight. Professional & Team Responsibilities * Participate in regular clinical supervision, case reviews, interdisciplinary team meetings, and integrated care planning forums. * Contribute actively to multidisciplinary treatment discussions and shared decision-making. * Maintain professional competency through ongoing training, continuing education, and participation in workshops, conferences, and professional networks * Uphold positive, respectful and supportive attitude with colleagues and client. * Represent agency in a professional manner during and outside of work hours. * Support organizational and CCBHC program goals by adapting to evolving service needs and contributing to program development. * Perform other duties as assigned. In the course of their work, YPAs are also promoting constructive advocacy at all levels, modeling and mentoring young people in those same skills. YPAs are individuals between 18 and 30 years of age who have self-identified as a person who has first-hand experience with social, emotional, medical, developmental, substance use, and/or behavioral challenges. They are able to use their lived experience with a disability, mental health challenge, substance use disorder and/or experience in juvenile justice, special education, and/or foster care to support young people in their resiliency/recovery and wellness journeys. YPAs receive training to develop skills and strategies to empower and support other young people. The core required training for this is the Youth Peer Advocate Training, but advocates receive a wide range of other training as well. A Youth Peer Advocate is responsible for implementing and monitoring service plan goals, in compliance with state, federal and Agency regulations and operating standards. The youth peer advocate works directly with children who have an emotional disability by skill building and behavioral support. The youth peer advocate will work to increase skill level and decrease challenging behaviors of children by addressing goals outlined in their individual goal plan. The youth peer advocate will provide services within the child's community in order to promote independence, inclusion and quality of life. Core Responsibilities * YPAs work with young people (primarily between ages 14-21) who are experiencing social, medical, emotional, developmental, substance use, and/or behavioral challenges in their home, school, placement, and/or community. These services provide the training and support necessary to ensure the active participation and engagement of young people in the treatment planning process and in the development and reinforcement of skills.
