Works collaboratively with MOW program staff to ensure seamless, integrated service delivery and provides expert consultation on participant needs as they evolve. Responsibilities Qualified individuals must be capable of performing the following essential duties and responsibilities, with or without reasonable accommodation: Initial Assessment & Triage Conducts comprehensive initial assessments (COMPASS) for all new MOW participants home visits to evaluate health status, functional abilities, social support, environmental conditions, and service needs. Determines appropriate level of ongoing support based on assessment findings, identifying participants with complex needs who require active case management versus those appropriate for standard monitoring by diet technicians. For participants with identified complex needs requiring ongoing case management: Develops and coordinates person-centered care plans in collaboration with participants and service providers. Provides assistance with accessing services and benefits, including coordinating in-home support services, arranging home safety assessments and modifications, facilitating healthcare and transportation connections, and assisting with benefit applications (e.g., SNAP, HEAP). Serves as a professional resource and consultant to participants, helping them navigate the service network. Conducts documented follow-up contacts at least every two months to monitor progress and adjust service plans as needed. Facilitates warm handoffs to community partners and specialized services when participant needs exceed program scope, ensuring seamless transitions and successful service connections. Documentation & Compliance Maintains timely, accurate, and confidential documentation of all case management activities in the NYS Health Commerce System and NYSOFA Statewide Client Database. Completes required assessments, reassessments, care plans, and follow-up documentation in accordance with MCOFA and NYSOFA regulations. Ensures all participant information is treated confidentially and referrals are made with appropriate consent. Retains documentation for required retention periods and completes case file reviews and audits as needed. Professional Development & Collaboration Maintains active New York State Office for the Aging (NYSOFA) Case Management Certificate and completes required continuing education (16 hours annually). Stays current on aging services regulations, community resources, and professional best practices through ongoing education and training. Participates actively in internal program meetings, external community partner meetings, and case reviews. Collaborates with MOW program staff, volunteers, and community partners to ensure coordinated service delivery. Administers comprehensive physical therapy modalities in the home setting. The qualified therapist assists the physician in evaluating level of function, helps develop the plan of care (revising it as necessary), prepares clinical and progress notes, advises and consults with the family and other agency personnel, and participates in in-service programs. RESPONSIBILITIES: Direct Patient Care Duties: Develops and revises plan of treatments and discharge plan in conjunction with patient, family, physician, care delivery team and third-party payors including obtaining medical orders and approval of physical therapy services. Provides supervision of PTA to ensure patient health, safety, and compliance with plan of care. Consults with and educates the patient, family and other team members regarding disease process, self care techniques and preventive strategies. Implements home therapy programs including but not limited to gait training, transfers, ADL's, home modification, care, and use of equipment. Periodically evaluates patient's progress and makes necessary adjustments to physical therapy program. Instruct para-professional staff in body mechanics, use of equipment, bed mobility, transfers, gait patterns, home exercise program, etc. Directs, coordinates, evaluates, and supervises the quality of patient care services provided by the Home Health Agency, including the clinical supervision of Home Health Aides. Interacts with patients, physicians, referral sources, and others in a manner conducive to continued positive relationships. Displays an exemplary level of patience, courtesy, and tact. Participates in regularly scheduled patient conferences with HHA's, nurses, therapists and MSW's for the purpose of discussing patient care, patient/family problems and to ensure comprehensive coordinated plan of treatment. Cooperatively assists in the orientation of new rehabilitation personnel. Performs other duties and responsibilities as assigned to support the efficient operation of their team, group, and discipline. Assures the completion of all appropriate clinical records needed for compliance with state and federal legislation and the delivery of seamless service within established timeframes. Ensures problem resolution at the point of contact. Ensures authorization and release of information form has been signed by patient or responsible party. Monitors results, progress and takes corrective action on quality indicators, including the level of customer satisfaction with the Physical Therapist function. Develops quality plans of care consistent with care paths and complies with TJC, OSHA/Regulatory practices and guidelines. Responds to findings of various Quality Management and QA processes and ensures resolution of potential problem areas. Maintains productivity requirements. Participates in education including orientation, in-service training programs, and self-development, as needed. Responsible for the Key Performance Indicators (KPI's) established by their manager. QUALIFICATIONS: A B.S., M.S. or D.P.T. degree in Physical Therapy Possession of a current New York State License in Physical Therapy. Ability to physically lift and assist patients in hands on care. Prefer minimum of one year experience in Physical Therapy in a hospital setting (not including internship). Good verbal and written communication skills. Good interpersonal skills.
