Key Duties: * Coordinates all aspects of Arts programming across UHP sites. * Identifies artists/artists in residence; onboards and manages on site arts workshops, performances, etc. * Identifies local artists, selects art, curates and works with artists to hang their work at UHP health centers. * Support the creation of art spaces through the health center, showcase local artists, patients' art and staff art and assist with planning and implementation of the culminating arts event at the health center. * Identify arts resources in our communities that can be visited by our patients. * Updates and maintains list of local arts resources and promotes engagement among patients and staff. * Complete required forms, data collection, and attend required training and meetings. * Link patients to art resources and support services for the purpose of improving their health and well-being. * Work within an interdisciplinary setting with providers, medical assistants, social workers, case managers, etc. to operationalize the social prescribing program in a clinical setting. * Participates in program evaluation activities and interdisciplinary committees.
