JOB SUMMARY The PhysicalTherapist (PT) is responsible for planning, delegating, and delivering safe, effective, and individualized physical therapy interventions as prescribed by a physician. The PT promotes functional independence and optimal quality of life for patients through comprehensive assessments and therapeutic care. ESSENTIAL FUNCTIONS
Conduct thorough physical therapy evaluations and develop individualized treatment plans to enhance patient mobility and independence.
Implement therapy programs and adjust as needed based on patient progress and goals.
Supervise PhysicalTherapist Assistants, students, and support personnel in accordance with state licensure laws and professional practice standards.
Recognize and appropriately refer patients to other healthcare disciplines for comprehensive care coordination.
Demonstrate proficient use of therapy techniques and equipment in accordance with current clinical standards.
Monitor and report changes in patient condition to the care team.
Collaborate in discharge planning and assess community mobility needs.
Recommend and coordinate the use of durable medical equipment as needed to support patient function and safety.
Ensure timely and accurate documentation, including billing and clinical records.
Address family and caregiver questions or concerns in a timely and compassionate manner.
Promote a safe, respectful care environment free from abuse or neglect.
Maintain patient confidentiality and adhere to HIPAA and organizational policies.
Participate in interdisciplinary care planning and case conferences.
Provide training and support to Restorative Nursing staff and other team members.
Deliver patient and caregiver education to support continued progress and home care.
Assist with training initiatives for facility staff and healthcare partners.
Ensure compliance with mandatory education, training and competency programs

JOB SUMMARY
The PhysicalTherapist works under the supervision of their Clinical Supervisor and the Rehab Director. He/she is responsible to organize and conduct medically prescribed physical therapy programs to restore function and prevent disability following disease, injury, or loss of a body part; helps patients reach their maximum performance and assume a place in society while learning to live within any limits for his/her capabilities.
PRIMARY JOB DUTIES
Performs patient assessment and documents in medical record.
Develops an age appropriate plan of care, which addresses physical, psychosocial and spiritual needs, in collaboration with other members of the health care team.
Implements the plan of care, delegates appropriately and ensures documentation of interventions.
Sees all patients as scheduled and assists with other patient needs throughout the continuum if needed.
Completes daily treatment notes on time that provide support for continuation of skilled therapy.
Completes and turns in daily all notes, charges, and other paper work.
Evaluates progress towards desired outcomes, revises plan of care, and intervenes as necessary.
Educates patient and families regarding current health status, plan of care, and anticipated outcomes.
Provides service excellence to all customers.
Demonstrates efficient time management and prioritization skills.
Maintains professional standards related to clinical practice, staffing and continuing education.
Practices fiscal responsibility and accountability.
Works required work hours and participates in weekend coverage to meet patient needs. Is flexible during times of high and low census and will provide coverage to other CCH rehabilitation service lines as needed.
Demonstrates positive interpersonal relations.
Maintains a clean and safe working environment that is conducive to high quality care.
Adheres to CCMH policies and procedures as well as the WYOT statutes governing the supervision of PTAs.
Complies with the hospital's Corporate Compliance Program including, but not limited to, the Code of Conduct, laws and regulations, and hospital policies and procedures.
Must be free from governmental sanctions involving health care and/or financial practices.
Performs all other duties as assigned.

The incumbent is a staff member of the Physical Medicine and Rehabilitation Service of the West Texas VA Health Care System. The position is located in the Physical Medicine and Rehabilitation Service at the Community Based Outpatient Clinic (CBOC) in Abilene, Texas. The incumbent carries a full patient load; is responsible for maintaining appropriate patient record in accordance with Physical Medicine and Rehabilitation, VA and JCO policies and standards. In addition, the incumbent attends team rounds or conferences; participates in in-service education activities; student education programs of various levels; and is responsible in maintaining Physical Therapy Clinic section and equipment, reporting hazardous conditions, supply needs etc. The incumbent independently evaluates each patient's functional level, develops and implements treatment plan specific to the patient needs, arranges appropriate, meaningful clinical and educational experiences for students (as needed). Incumbent offers daily support for physicaltherapists (PT) and supervision/support for physicaltherapist assistants (PTA) (as needed). PTs at this level are responsible for the examination and treatment of Veterans referred to the Physical Therapy Department. Designs, conducts, and evaluates education, experiences within specialty content area for staff members and seeks educational opportunities to enhance the Physical Therapy Department, PM&RS, and the Medical Center. Establishes and maintains contact with other key staff involved in Veteran care via written, telephone, and personal communication on a regular basis at the Medical Center level. Represents the Physical Therapy Department and PM&RS at various clinical and administrative meetings at the discretion of the Supervisor. Offers daily support to PTs and supervision/support to PTAs. Conducts comprehensive Veteran examinations to identify rehabilitation problems and establish treatment goals. Develops and implements treatment plans designed to meet established goals. Evaluates Veteran response to treatment and modifies the treatment plan to provide optimum outcome. Orders appropriate Durable Medical Equipment and instructs Veteran in the safe use of the equipment. Makes recommendations to PCP and Specialty Care. Supervises clinical training experience for PT students but must have minimum of 1-year clinical experience.

Dartmouth-Hitchcock Medical Center seeks a full-time PhysicalTherapist to join our acute care rehabilitation team comprised of over sixty staff members including PT's, OT's, SLP's and mobility technicians. In this role the PhysicalTherapist will work with hospital patients after surgery, illness or injury. The selected candidate will provide physical therapy services to patients on all units and of all ages from pediatrics to geriatrics. At this academic, Level-I trauma center, the physicaltherapist will work closely with a large team of diverse providers in a collaborative, collegial work environment that fosters the best care possible for our patients.
$10,000 signing bonus and relocation available
Position Highlights:
Position is: full-time, 40 hours/week, Mon-Fri, 8-4:30
Includes occasional, scheduled weekend shifts with associated days off during the week
Responsible for autonomous provision of physical therapy services in the acute care setting
Available ICU and Pediatric competencies for those demonstrating committed interest

Responsibilities
Examines, evaluates, diagnoses, determines prognosis and provides intervention. Reports findings to other health care team members. Provides discharge recommendations, exercise programs, and family teaching.
Plans therapy involving physical exercise, massage, heat, water, etc.
Participates in and supports in-service education, student programs, volunteer/observation programs, Professional Practice Committee, committees, etc.
Provides documentation, billing, and record keeping. Participates in compliance audits.
Coordinates the scheduling of patients and meetings.
Determines and implements annual plan for professional development.
Delegates tasks to support personnel.
Performs other duties as required or assigned.
Qualifications
Bachelor's degree if graduate prior to 2002; minimum of Master's degree if graduating in 2002 or later.

Provides physical therapy services under the supervision of a designated licensed physicaltherapist. Documents treatment sessions in accordance with policies and procedures of the department and regulatory bodies. Supervision of a PhysicalTherapist Assistant by a licensed physicaltherapist shall be continuous but not necessarily on site, in accordance with NYS Department of Education and other regulatory bodies.
ESSENTIAL FUNCTIONS/RESPONSIBILITIES:
• Provides direct physical therapy services, documents treatments, and submits proper billing codes.
• Implements treatment plans established by the supervising PhysicalTherapist. Communicates with supervising PhysicalTherapist regarding any noted change in patient status, as well as patient's response to the treatment plan.
• Maintains productivity expectations set by rehab leadership team.
• Makes recommendations for the purchase of specialized equipment, materials, and wheelchair modifications.
• As a member of the interdisciplinary team, participates in professional meetings as assigned to integrate services with other providers.
• Lends counsel and education to patients and their family members/care givers (if applicable) regarding the patient's physical therapy services.
• Functions as a clinical instructor for PTA students. (after a minimum of 1 year's work experience).
• Maintains clinical competency by maintaining CEU requirements for NYS licensure.
• Maintains 90% compliance on Chart Audits. Follows required documentation guidelines and expectations set forth by the rehab department and regulatory bodies (Medicare, Medicaid, NYS Ed, DOH, Commerical Insurances, etc.).
• Complies with all agency policies and procedures.
• Other duties as assigned
