Medical Records Tech WEEKLY HOURS: 40 OHC - Specialists in Cancer and Blood disorders, is seeking a medical records technician to be based out of our Wooster office. As a medical records tech, you will prepare charts for patient visits, locate, retrieve and deliver medical records and / or electronic medical records as assigned, as well as assist with organizing, sorting and filing all incoming and outgoing patient information. As the only independent adult cancer program in the region, we offer Medical, Radiation, Blood and Marrow Transplant, Gynecologic, and Breast Surgical Oncology services. Responsibilities
Responsibilities Include by not limited to:
* Scan and attach all internal and external correspondence and electronic medical reports into patient' medical record chart according to filing system.
* Pulls charts for scheduled appointments in advance according to guidelines. Ensure that all appropriate documentation for the scheduled patient visit is attached to the patient's chart.
* Prints, mails, and/or faxes patient chart information as requested and authorized. Documents all processes.
* Releases medical records information to persons or agencies according to State and Federal regulations.
* Compile and maintain patients' medical records to document condition and treatment and to provide data for research or cost control and care improvement efforts.
* Picks up out guides at designated locations and returns to Medical Records.
* Catalogue charts for storage and keeps electronic reports of those records.
* Submits request for chart retrieval from storage if needed to comply with a medical records request.
* Makes copies of dictated interval notes accordingly. Monitors physician dictation and makes sure it is completed in the EMR (electronic medical record) and alerts physicians who are behind.
* Keeps a record of new patients for weekly physicians OCM meetings
* Follows policies and procedures to contribute to the efficiency of the front office Provides back-up assistance as needed by front office staff.
* Sends out dictations to referring providers via manual faxing, right fax, or electronically "Direct Message" (direct message via EMR is required for MIPS.
* In Radiation department may convert dosimetry plans into a PDR of zip file when a patient transfers to another facility for treatment.
* Logs FMLA/Disability forms for provider completion. Will mail or fax forms and contact patient once completed.
* Sends outgoing faxes and distributes incoming faxes.
* Prepares correspondence, memos, forms and other typing as requested by supervisor.
Qualifications
MINIMUM QUALIFICATIONS:
High school diploma or equivalent required. Position is entry level and requires 0-3 years' experience, preferably in a medical office setting. Previous experience in a medical records' experience preferred. Knowledge of electronic health record systems. Time Management, Organization, Attention to Detail and Quality Focus skills needed.

The Tumor Registrar (Oncology Data Specialist) assures thorough, accurate and quality data collection as required by the Commission on Cancer (CoC), the Surveillance, Epidemiology and End Results Program (SEER) and State of Connecticut. This includes case-finding, abstraction, follow-up and reporting requirements. This role will maintain accurate cancer registry data, ensuring all cancer cases are documented and reported in compliance with national standards.
Essential Duties & Responsibilities
Under general supervision the Tumor Registrar will;
Review medical records to identify and abstract cancer cases and related information.
Collect detailed data on patient demographics, tumor characteristics, staging, treatment, and outcomes.
Maintain an accurate, up-to-date cancer registry, ensuring comprehensive and timely data entry
Perform data validation and quality checks to ensure completeness and consistency of the cancer registry.
Monitor data trends and resolve discrepancies in the dataset through communication with medical staff or external organizations.
Ensure that all data complies with industry standards and regulations, such as the American College of Surgeons (ACoS) and the Commission on Cancer (CoC) guidelines
Prepare and submit cancer registry reports to state, national, and regulatory agencies as required (e.g., SEER, National Cancer Database).
Ensure compliance with HIPAA and other confidentiality regulations when handling patient information.
Assist in preparing data for quality assurance, audits, and accreditation reviews.
Work closely with physicians, clinical staff, and healthcare providers to gather accurate and complete cancer data.
Serve as a resource for oncology teams by providing data for case reviews, performance improvement initiatives, and clinical research.
Collaborate with multidisciplinary teams to improve data collection processes and accuracy.
Participate in the analysis of cancer data to identify trends, survival rates, and outcomes for internal reporting or external research studies.
Coordinates weekly tumor boards for the interdisciplinary team, this includes preparing detailed case summaries for each case presented.
Support research initiatives by providing tumor registry data for studies and clinical trials.
Assist in tracking patient outcomes and treatment patterns to guide cancer care planning.
Stay current with changes in tumor registry standards, coding systems (e.g., ICD-10, AJCC staging), and cancer care practices.
Participate in professional development and certification programs to maintain and enhance expertise in tumor registry operations.

Under the direction of the facility Coding and Reimbursement Manager, conducts coding quality reviews and audits of chart documentation to assess accuracy, ensure compliance with federal and payer policies, and identifies areas for improvement for hospital outpatient coding. Develops and delivers training on coding accuracy and compliance, staying updated on regulations and providing expert guidance to coders. Provides ongoing coding education and training to coding team and serves as mentor to all new coding team members. High School diploma or equivalent and medical coding education. In lieu of medical coding education, an active coding certification is required. Associate degree in healthcare related field preferred.
