    The Quality Pharmacist supports quality operations in a 503B outsourcing facility, ensuring all compounded sterile preparations meet regulatory and company quality standards. This entry-level role provides exposure to quality assurance, regulatory compliance, and compounding oversight under cGMP and FDA guidelines. The position assists with documentation review, investigations, and audits while promoting a culture of safety and continuous improvement within pharmaceutical operations Responsibilities: Review batch records and documentation for accuracy, completeness, and compliance with SOPs and cGMP. Support environmental monitoring, media fills, and sterility assurance activities. Participate in deviation investigations, corrective and preventive actions (CAPA), and change control processes. Assist in internal and external audit preparation and responses. Verify labeling, packaging, and final product release documentation. Complete annual product reviews. Submit biannual reporting to FDA for compounded products. Obtain and maintain pharmacist licensure in multiple states. Support maintenance of compliance with FDA, DEA, USP /, and state board of pharmacy regulations. Assist in maintaining facility and product licenses and registrations. Participate in periodic reviews of standard operating procedures and quality documentation. Ensure documentation practices meet FDA 503B outsourcing facility standards. Identify opportunities for process improvement and operational efficiency. Support quality metrics reporting and trending of deviations or customer complaints. Assist in training staff on quality systems, GMP documentation, and aseptic techniques. Collaborate cross-functionally with operations, production, and R&D to maintain a compliant environment. Provide quality-related guidance during daily production activities and support troubleshooting efforts. Promote a culture of compliance, accountability, and continuous learning throughout the organization. Required Skills/Abilities: Strong understanding of pharmaceutical compounding and cGMP fundamentals. Excellent organizational, analytical, and problem-solving skills. Strong written and verbal communication abilities. Proficiency with Microsoft Office and familiarity with electronic quality management systems (QMS). High attention to detail and ability to manage multiple priorities. Willingness to learn regulatory compliance principles for 503B operations. Excellent time management skills with a proven ability to meet deadlines. Strong analytical and problem-solving skills. Strong supervisory and leadership skills. Ability to prioritize tasks and to delegate them when appropriate. Ability to function well in a high-paced and at times stressful environment. Proficient with Microsoft Office Suite or related software. Education and Experience: Doctor of Pharmacy (PharmD) degree required. 0-2 years of experience in a compounding pharmacy or cGMP-regulated pharmaceutical setting (internship or rotation experience acceptable). Knowledge of USP , , and FDA guidance for outsourcing facilities preferred. State pharmacist license in good standing required (or eligibility for licensure). Requirements: Pharmacist will apply for licensure in any state(s) that STAQ requests them to do so, at STAQ's cost and expense. Pharmacist will allow STAQ to use their license for any site in which STAQ is licensed or applying to be licensed. Physical Requirements: Prolonged periods of sitting at a desk and working on a computer. Must be able to lift up to 15 pounds at times.

    INTRODUCTION The purpose of this standard procedure is to establish and define the duties, responsibilities, authority, accountability, reporting and measures of performance for the Pharmacist-In-Charge, Pharmacist and Compounding Pharmacist with Curexa. SUMMARY OF DUTIES The Pharmacist is responsible to ensure that prescriptions that are ordered by practitioners are filled accurately, to counsel patients regarding medications, to maintain a safe and clean working environment by complying with Curexa policies and procedures, SOPs, NJ state Board of Pharmacy regulations and federal laws, to supervise the activities of the assigned pharmacy personnel, to ensure that patients are serviced professionally, promptly and courteously and to facilitate Curexa's growth and success of the pharmacy. REPORTING RELATIONSHIPS The Pharmacist-in-Charge is the President and CEO or the Director of Operations. The Pharmacist and Compounding Pharmacist report directly to and are accountable to the Pharmacist-in-Charge and Director of Operations. The following positions report to the Pharmacist, as applicable (oversight capacity): Compounding Lab Manager Compounding Lab Supervisor/HDDP Operations Manager Patient Care Coordinator Supervisor Compounding Care Coordinator Supervisor Patient Care Coordinators/Compounding Care Coordinators/Compounding Lab/Fulfillment Technicians Patient Care Manager Patient Care Representative Shipping Specialist Shipping Packaging Specialist Delivery Driver REQUIREMENTS EDUCATION Required: BS or PharmD. graduate degree obtained from an accredited university/college of Pharmacy by the Accreditation Council for Pharmacy Education and a Pharmacist license issued by the domicile state Board of Pharmacy and any other state where required. EXPERIENCE Required: Completed an internship or other program that has been approved by domicile State Board of Pharmacy or demonstrated to the board's satisfaction experience in the practice of pharmacy which meets or exceeds the minimum internship requirements of the board. Passing the NAPLEX and MJPE testing, proof of character, and criminal history background check for initial licensure as a Pharmacist. Preferred: 3-5 years Retail, Independent, or Clinical working environment experience. LICENSES Required: Actively licensed and registered by domicile State Board of Pharmacy. License should be in good standing and continuing education credits and fees maintained. The current license shall be conspicuously displayed in the pharmacist's principal place of employment and a license card or primary source verification carried on the pharmacist's person if working in a secondary location. Passing of the criminal history background check and fingerprinting as per state registration requirements. Manage dispensary procedures, support personnel, and inventory control operations in a highly efficient, productive and accurate manner. Monitor patient customer satisfaction levels and resolves customer complaints in a professional manner to the satisfaction of the patient. Supports the goals, policies, and standard operating procedures of the pharmacy and maintains supervision of pharmacy support personnel for compliance. Maintain confidentiality of all company information and ensure support personnel exercise equal discretion. Operate ethically and in compliance with all federal, state board of pharmacy and accreditation regulations and all USP 795 and USP 800 guidelines for an FDA 503a pharmacy. DUTIES AND TASKS RETAIL PHARMACIST Review incoming prescriptions for completeness and validity. Supervise and support all pharmacy support personnel in completing their tasks in a timely, efficient, and effective manner and according to federal and state laws. Ensure all pharmacy support personnel are practicing within their scope of responsibilities as defined by the NJ State Board of Pharmacy for Technicians. Provide training and be accessible to answer questions whether patient or work related in nature to support personnel's job performance. Provide feedback, positive or negative, to ensure customer satisfaction. Re-train, if necessary, to clarify job expectations and responsibilities. Ensure patient confidentiality is maintained by all pharmacy personnel according to HIPAA guidelines. Support and enforce all federal and state board of pharmacy laws pertaining to drug dispensing, compounding, patient information and company policy and procedures. Ensure that all customer complaints are acted upon. Refer all complaints that have not or cannot be settled to the Operations Manager for advice or final resolution. Complete Complaint forms and APMS(PQC) program for compliance and workflow efficiency. Ordering and maintaining proper inventory of Rx medications and OTC products or delegating this task as necessary. This includes returns for overstocked or special-order items and ordering pharmacy supplies. Check special orders from previous day orders and make sure patients are called, when necessary, to inform them of availability. Ensure the will-call Rx bins are purged of filled prescriptions to be put back in stock if not picked up within 2 weeks and medications are returned to the shelf according to state laws and HIPAA guidelines. Ensure outdated medications are pulled from the shelves in a timely manner and returned for credit to the wholesaler or reverse distributor. Ensure customers are called if a prescription cannot be filled due to short supply, not in stock, refills need to be ordered by physician, insurance problems or any other problems that will delay the prescription from being filled. DEA Schedule Class 2 orders should be reviewed and received appropriately, including invoice filing, perpetual inventory, and proper storage. Proper use of the PMP system for dispensing of controlled substances before fulfillment. Facilitate prescription transfers to and from other pharmacies in compliance with federal and state laws. Delegate job or role-specific responsibilities accordingly to ensure all areas of operation are staffed appropriately for complete customer service and productivity. Communicate with the management team to maintain optimal workflow and foster an attitude of teamwork. Ensure that the Pseudoephedrine log is maintained properly by technicians and customer service specialists when applicable to scope of business. Ensure that a clean, neat work environment is maintained daily to ensure efficiency, so store appearance is reflective of an orderly, customer-friendly atmosphere. Completion and passing of annual competency, training, and PRS compliance modules proficiently. Follow proper safety standards for opening and closing the pharmacy including alarm systems, key locks, fire alarm protocols for the safety of employees and customers. Maintain teamwork with Sales Team to ensure proper communication and representation of Curexa services and products. Support the Operations team in problem solving, analyzing opportunities to improve the overall profit and growth of the company. The Pharmacist is a direct reflection of Curexa and is responsible for the continued success of the business. Comply with all applicable company policies, procedures, and patient protocols along with NJ State Board of Pharmacy. Perform any other task assigned by the Leadership team. PHARMACIST-IN-CHARGE (PIC) (IF APPLICABLE) NEW JERSEY The PHARMACIST-IN-CHARGE is responsible for all the duties listed for Retail Pharmacist along with the following listed guidelines set forth by the New Jersey State Board of Pharmacy: Every pharmacy shall name a pharmacist whose license is in good standing in New Jersey as the pharmacist-in-charge (PIC) of the pharmacy. No pharmacy shall operate without a PIC for longer than 30 days. Whenever the pharmacist-in-charge (PIC) is absent from the pharmacy for more than 30 days, the PIC and the permit holder shall notify the Board of the name of the pharmacist who shall act as the interim PIC. A pharmacist shall not assume the responsibilities of a PIC of more than one pharmacy or pharmacy department simultaneously. Whenever there is a change of a PIC of a pharmacy, an inventory of all controlled dangerous substances shall be performed consistent with NJ requirements. Whenever a pharmacist assumes or terminates the duties as a PIC of a pharmacy, the PIC and the permit holder shall so advise the Board in writing within 30 days by completing a form provided by the Board. A PIC shall be a full-time employee, employed for a minimum of 35 hours per week and shall be physically present in the pharmacy or pharmacy department for that amount of time necessary to supervise and ensure that: The pharmacy is staffed by sufficient, competent personnel in keeping with the size, scope and complexity of the pharmaceutical services provided by the pharmacy. Accurate records of all prescription medications received and dispensed are maintained. Policies are in place regarding accurate dispensing and labeling of prescriptions and compliance of policies. Security of the prescription area and its contents are maintained at all times consistent with the regulations. Only pharmacists and interns or externs under immediate personal supervision provide professional consultation with patients and practitioners. Only pharmacists, interns or externs accept telephone prescriptions and only pharmacists, interns or externs or pharmacy technicians consistent with Board of Pharmacy requirements, accept renewal prescriptions. No misbranded, deteriorated, adulterated, improperly stored or outdated drugs or any drugs marked 'sample' or with any like designation or meaning are dispensed or present in the active stock of the Pharmacy. The prescription area is maintained in an orderly and sanitary manner. The pharmacy and all pharmacy personnel provide pharmaceutical services in accordance with acceptable professional standards and comply with all Federal and State statues, rules, and regulations regarding the practice of pharmacy. There are written policies and procedures to ensure the proper storage and delivery of all prescription drugs and chemicals consistent with the requirements set forth in N.J.A.C. 13:39-5.11 and that such policies and procedures are followed. The pharmacist-in-charge is responsible for the accuracy and completeness of the biennial inventory of all controlled dangerous substances required under N.J.A.C. 13:45H-5.5 and shall sign and date the biennial inventory upon its completion. This requirement applies whether the inventory is conducted by the pharmacist-in-charge or another licensed pharmacist. If the pharmacist to pharmacy technician ratio exceeds 1:2, the pharmacy shall maintain a policy and procedure manual with regard to pharmacy technicians. The pharmacist-in-charge shall review at least every two years and, if necessary, amend the policy and procedure manual. Documentation of the review shall be made available to the Board upon request. Manages all aspects of the pharmacy operation and compliant with all Curexa SOPs, policies and procedures, NJ State regulations and federal law in accordance with Curexa's accreditation bodies. PIC and Compounding Pharmacist The PIC and Compounding Pharmacist shall be responsible for the Retail Pharmacist duties along with the following listed guidelines set forth by the NJ State Board of Pharmacy 13.39 Subchapter 11A for Compounding Non-Sterile Preparations in a retail pharmacy which ensures that: 1) Compounded non-sterile preparations have been properly prepared, labeled, controlled, stored, dispensed, and distributed compliantly. 2) All aspects of the compounding process set out in N.J.A.C. 13:39-11A.15 are documented and that accurate compounding records for all compounded non-sterile preparations prepared by the pharmacy are maintained. 3) Pharmacists and Compounding Lab Technicians are capable of performing and qualified to perform their assigned duties including compounding medications. The compounding pharmacist shall provide immediate personal supervision to pharmacy technicians, pharmacy interns, or pharmacy externs who are performing non-sterile preparation compounding. Supervision shall include, but is not limited to, the checking of each ingredient used, the quantity of each ingredient whether weighed, measured, or counted, and the finished label. The compounding pharmacist may delegate to pharmacy technicians, pharmacy interns, or pharmacy externs only the following tasks: recording of the prescription, selection of the drugs and container, typing of labels, and compounding of preparations. The compounding pharmacist shall ensure that each task has been performed correctly. 4) Ingredients used in compounding have their expected identity, quality, and purity consistent with the requirements of N.J.A.C. 13:39-11A.12. 5) Compounded preparations are of acceptable strength, quality, and purity, with appropriate packaging and labeling, and are prepared in accordance with good compounding practices, official standards, and relevant scientific data and information as set forth in USP 795. 6) Critical processes are recorded and validated to ensure that procedures will consistently result in the expected qualities in the finished preparation. 7) The compounding environment is suitable for its intended purpose. 8) Appropriate stability evaluation is performed or is determined from the literature for establishing reliable beyond-use dating to ensure that the finished preparations have their expected potency, purity, quality, and characteristics, at least until the labeled beyond-use date. 9) Compounding conditions and procedures are in place to minimize the potential for errors. 10) Adequate procedures and records exist for investigating and correcting failures or problems in compounding, testing, or in the preparation itself. 11) The patient is advised that the product dispensed is a compounded preparation. 12) A pharmacist shall not compound preparations that contain drug products that appear on the Federal Food and Drug Administration's list of Drug Products Withdrawn or Removed from the Market for Reasons of Safety or Effectiveness, codified at 21 CFR 216.24. b) A pharmacist shall not compound any commercially available drug products except as provided in N.J.A.C. 13:39-11A.4. Any confirmed incidents of product contamination shall be reported by the pharmacist-in charge to the New Jersey Board of Pharmacy within 48 hours of becoming aware of any such incidents. Per NJ 13:39-11A.14 a) The compounding pharmacist shall provide immediate personal supervision to pharmacy technicians, pharmacy interns, or pharmacy externs who are performing non-sterile preparation compounding. 1) Supervision shall include, but is not limited to, the checking of each ingredient used, the quantity of each ingredient whether weighed, measured, or counted, and the finished label. b) The compounding pharmacist may delegate to pharmacy technicians, pharmacy interns, or pharmacy externs only the following tasks: recording of the prescription, selection of the drugs and container, typing of labels, and compounding of preparations. All Pharmacists and Pharmacy Technicians that are compounding preparations comply with all USP 795, USP 800 guidelines, federal and state board of pharmacy laws along with all Curexa accreditation body requirements in compliance with FDA 503a pharmacy guidelines. MEASURES OF PERFORMANCE The Pharmacist should conduct themselves in an ethical and prudent manner in compliance with all company policies and procedures. The Curexa code of ethics should be followed in all duties performed as described by the employee handbook. All prescriptions are filled, checked and dispensed accurately. Compliance with all federal and state board of pharmacy laws pertaining to checking of prescriptions, dispensing, and patient confidentiality. Patient consultation is completed proficiently and according to state and HIPAA guidelines including all drug and pharmacy information required regarding adverse reactions, drug interactions, drug allergies, disease states, storage and disposal of medications, complaint and return policy and pharmacy contact information. The pharmacy should have no unaccountable inventory shortage or financial shortage which includes investigation of any negative profit claims. Customer complaints from individuals or practitioner offices should be few and professionally, efficiently, and expeditiously resolved. Complaint forms should be completed along with APMS(PQC) platform utilized for accountability and analysis of workflow efficiency. All Curexa SOPs and PPs are followed regarding Retail and Compounding Pharmacy practices. Compounding Pharmacy practices are compliant with all USP 795, USP 800 guidelines and state board of pharmacy and federal laws along with Curexa accreditation requirements for an FDA 503a pharmacy. If conflicting regulations, the most stringent laws are followed. Curexa recall procedures and adverse reaction reports are completed properly and followed-up, as applicable. Participation in marketing and sales programs in-store and external activities performed with positive energy and professionalism for company growth. Continuing education courses, reading didactic literature sources, attending live seminars, and overall furthering of knowledge pertinent to retail pharmacy and compounding practices should be ongoing to keep compliant with accreditation guidelines and state licensure renewals. Completion and passing of annual competency, training and PRS compliance modules proficiently. Proper inventory control and management including controlled substance monitoring, use of PMP and dispensing is maintained. Supervision of pharmacy support personnel is done proficiently to support an efficient workflow and maintain productivity of the pharmacy. Proper communication with management is maintained. Prescription volume, total dollar sales, inventory control, and profitability of the company have a direct impact to the success of the business which is a responsibility of the Pharmacist to assist in the overall growth of the company. This includes returns for overstocked or special-order items and ordering pharmacy supplies. Check special orders from previous day orders and make sure patients are called, when necessary, to inform them of availability. Ensure the will-call Rx bins are purged of filled prescriptions to be put back in stock if not picked up within 2 weeks and medications are returned to the shelf according to state laws and HIPAA guidelines. Ensure outdated medications are pulled from the shelves in a timely manner and returned for credit to the wholesaler or reverse distributor. Ensure customers are called if a prescription cannot be filled due to short supply, not in stock, refills need to be ordered by physician, insurance problems or any other problems that will delay the prescription from being filled. PHARMACIST-IN-CHARGE (PIC) (IF APPLICABLE) NEW JERSEY The PHARMACIST-IN-CHARGE is responsible for all the duties listed for Retail Pharmacist along with the following listed guidelines set forth by the New Jersey State Board of Pharmacy: Every pharmacy shall name a pharmacist whose license is in good standing in New Jersey as the pharmacist-in-charge (PIC) of the pharmacy. No pharmacy shall operate without a PIC for longer than 30 days. Whenever the pharmacist-in-charge (PIC) is absent from the pharmacy for more than 30 days, the PIC and the permit holder shall notify the Board of the name of the pharmacist who shall act as the interim PIC. A pharmacist shall not assume the responsibilities of a PIC of more than one pharmacy or pharmacy department simultaneously. A PIC shall be a full-time employee, employed for a minimum of 35 hours per week and shall be physically present in the pharmacy or pharmacy department for that amount of time necessary to supervise and ensure that: The pharmacy is staffed by sufficient, competent personnel in keeping with the size, scope and complexity of the pharmaceutical services provided by the pharmacy. Accurate records of all prescription medications received and dispensed are maintained. Policies are in place regarding accurate dispensing and labeling of prescriptions and compliance of policies. Security of the prescription area and its contents are maintained at all times consistent with the regulations. Only pharmacists and interns or externs under immediate personal supervision provide professional consultation with patients and practitioners. Only pharmacists, interns or externs accept telephone prescriptions and only pharmacists, interns or externs or pharmacy technicians consistent with Board of Pharmacy requirements, accept renewal prescriptions. No misbranded, deteriorated, adulterated, improperly stored or outdated drugs or any drugs marked 'sample' or with any like designation or meaning are dispensed or present in the active stock of the Pharmacy. The prescription area is maintained in an orderly and sanitary manner. The pharmacy and all pharmacy personnel provide pharmaceutical services in accordance with acceptable professional standards and comply with all Federal and State statues, rules, and regulations regarding the practice of pharmacy. There are written policies and procedures to ensure the proper storage and delivery of all prescription drugs and chemicals consistent with the requirements set forth in N.J.A.C. 13:39-5.11 and that such policies and procedures are followed. The pharmacist-in-charge is responsible for the accuracy and completeness of the biennial inventory of all controlled dangerous substances required under N.J.A.C. 13:45H-5.5 and shall sign and date the biennial inventory upon its completion. The compounding pharmacist may delegate to pharmacy technicians, pharmacy interns, or pharmacy externs only the following tasks: recording of the prescription, selection of the drugs and container, typing of labels, and compounding of preparations. The compounding pharmacist shall ensure that each task has been performed correctly. 4) Ingredients used in compounding have their expected identity, quality, and purity consistent with the requirements of N.J.A.C. 13:39-11A.12. 5) Compounded preparations are of acceptable strength, quality, and purity, with appropriate packaging and labeling, and are prepared in accordance with good compounding practices, official standards, and relevant scientific data and information as set forth in USP 795. 6) Critical processes are recorded and validated to ensure that procedures will consistently result in the expected qualities in the finished preparation. 7) The compounding environment is suitable for its intended purpose. 8) Appropriate stability evaluation is performed or is determined from the literature for establishing reliable beyond-use dating to ensure that the finished preparations have their expected potency, purity, quality, and characteristics, at least until the labeled beyond-use date. 9) Compounding conditions and procedures are in place to minimize the potential for errors. 10) Adequate procedures and records exist for investigating and correcting failures or problems in compounding, testing, or in the preparation itself. 11) The patient is advised that the product dispensed is a compounded preparation. 12) A pharmacist shall not compound preparations that contain drug products that appear on the Federal Food and Drug Administration's list of Drug Products Withdrawn or Removed from the Market for Reasons of Safety or Effectiveness, codified at 21 CFR 216.24. b) A pharmacist shall not compound any commercially available drug products except as provided in N.J.A.C. 13:39-11A.4. Any confirmed incidents of product contamination shall be reported by the pharmacist-in charge to the New Jersey Board of Pharmacy within 48 hours of becoming aware of any such incidents. Per NJ 13:39-11A.14 a) The compounding pharmacist shall provide immediate personal supervision to pharmacy technicians, pharmacy interns, or pharmacy externs who are performing non-sterile preparation compounding. 1) Supervision shall include, but is not limited to, the checking of each ingredient used, the quantity of each ingredient whether weighed, measured, or counted, and the finished label. b) The compounding pharmacist may delegate to pharmacy technicians, pharmacy interns, or pharmacy externs only the following tasks: recording of the prescription, selection of the drugs and container, typing of labels, and compounding of preparations. The compounding pharmacist shall ensure that each task has been performed correctly. All Pharmacists and Pharmacy Technicians that are compounding preparations comply with all USP 795, USP 800 guidelines, federal and state board of pharmacy laws along with all Curexa accreditation body requirements in compliance with FDA 503a pharmacy guidelines. MEASURES OF PERFORMANCE The Pharmacist should conduct themselves in an ethical and prudent manner in compliance with all company policies and procedures. The Curexa code of ethics should be followed in all duties performed as described by the employee handbook. All prescriptions are filled, checked and dispensed accurately. Compliance with all federal and state board of pharmacy laws pertaining to checking of prescriptions, dispensing, and patient confidentiality. Patient consultation is completed proficiently and according to state and HIPAA guidelines including all drug and pharmacy information required regarding adverse reactions, drug interactions, drug allergies, disease states, storage and disposal of medications, complaint and return policy and pharmacy contact information. The pharmacy should have no unaccountable inventory shortage or financial shortage which includes investigation of any negative profit claims. Customer complaints from individuals or practitioner offices should be few and professionally, efficiently, and expeditiously resolved. Complaint forms should be completed along with APMS(PQC) platform utilized for accountability and analysis of workflow efficiency. All Curexa SOPs and PPs are followed regarding Retail and Compounding Pharmacy practices. Compounding Pharmacy practices are compliant with all USP 795, USP 800 guidelines and state board of pharmacy and federal laws along with Curexa accreditation requirements for an FDA 503a pharmacy. If conflicting regulations, the most stringent laws are followed. Curexa recall procedures and adverse reaction reports are completed properly and followed-up, as applicable. Participation in marketing and sales programs in-store and external activities performed with positive energy and professionalism for company growth. Continuing education courses, reading didactic literature sources, attending live seminars, and overall furthering of knowledge pertinent to retail pharmacy and compounding practices should be ongoing to keep compliant with accreditation guidelines and state licensure renewals. Completion and passing of annual competency, training and PRS compliance modules proficiently. Proper inventory control and management including controlled substance monitoring, use of PMP and dispensing is maintained. Supervision of pharmacy support personnel is done proficiently to support an efficient workflow and maintain productivity of the pharmacy. Proper communication with management is maintained. Prescription volume, total dollar sales, inventory control, and profitability of the company have a direct impact to the success of the business which is a responsibility of the Pharmacist to assist in the overall growth of the company.
