ESSENTIAL JOB FUNCTIONS AND TASKS:
Manage the Assisted Living services that are provided including completing essential
paperwork and complying with state regulations.
Works to assure the facility is in compliance with federal state and local standards and
regulations.
Works with Assisted Living Director to keep forms updated including policies,
procedures, Bill of Rights, and related assisted living forms and manuals.
Meets with Case Workers regularly, as needed.
Maintains an up-to-date knowledge of current trends regarding Assisted Living facilities
Is responsible for investigating and reporting incidents of suspected resident neglect

Communicates in staff communication books any information that staff needs to be
aware of.

Send tasks via EHR when necessary to enlist the assistance of other team members.
Complete same day post-operative surgical calls.
Participate in trainings and meets requirements for Merit Based Incentive Payment System (MIPS) measures, HIPAA compliance and Protected Health Information.

Job Title: Visiting Nurse (RN) - Floating Territories

Annualized Salary: $75,000 - $110,000 (paid per visit)

All of our clinicians work in strategic "territories" to see clients (approx. 6 to 7 per day), and your goal in this position is to help out with patient load fluctuations across multiple territories.

Registered Nurse - RN Visit Nurse Responsibilities:
Assessing and re-evaluating the client's condition and needs as required.
Provide patient cares according to client's care plan and needs.
Teach the client and/or family members about cares and support skills.
Communicate regularly with RN Case Managers, Supervisors, and the Care Team.
Registered Nurse - RN Visit Nurse Qualifications:
Possess a current Registered Nurse license in the State of Minnesota.
Trained in skills necessary to provide nursing care to a wide variety of medical conditions.
Demonstrate basic computer knowledge and ability with an aptitude to learn company applications.
