Duties Description Hutchings Psychiatric Center is recruiting for a UtilizationReview Coordinator to analyze and evaluate quality, effectiveness, and efficiency of psychiatric and medical services provided to individual diagnosed with mental illness. Candidates from outside or within State Service can apply.
As a UtilizationReview Coordinator, your duties will include:
* Reviewing clinical records for completeness and accuracy to ensure compliance.
* Collecting and analyzing data related to the utilizationreview process and preparing reports on findings, patterns, and trends.
* Reviewing medical records of new admissions, placement decisions, length of stays, and discharge plans against established criteria to determine if necessary and effective.
Minimum Qualifications Candidates from outside or within State Service can be considered for hire under the Hiring for Emergency Limited Placement - Statewide (NY HELPS) program as a UtilizationReview Coordinator if they have:
* Bachelor's degree in a human services field and 3 years of utilizationreview experience in a health care facility regulated by Centers for Medicare and Medicaid Services UtilizationReview Standards, or
* Possession of a license and registration to practice nursing and 2 years of post-licensure nursing experience, one of which must have included utilizationreview and/or discharge planning

Candidates within State Service can be considered for a competitive class appointment if they are reachable on the current UtilizationReview Coordinator eligible list.

Day to Day Responsibilities:
Conducts utilization management activities in accordance with Utilization Management policies and procedures. Responds to calls, conducts certification reviews, facilitates the discharge planning and referral process and coordinates denials with physician advisors for all assigned programs.

Job Description:
Performs specified patient care activities and related services necessary in quality care of the home nutritional support and intravenous therapy patient.
QUALIFICATION REQUIREMENTS
Graduate of a N.L.N. accredited school of nursing required
Current license as a Registered Nurse in the state in which you are currently practicing required
Minimum of one (1) year of experience in IV therapy with a strong background in nutritional support and/or immunoglobulin therapy required
In lieu of above experience, two (2) years of recent experience in an acute care setting or apprentice for a period of 3 months for all home visits with an experienced R.N. may be accepted
Must possess or acquire a current CPR certificate within the first 30 days of employment
Must possess good technical skills and strong knowledge base in clinical nutrition and IV therapy, and must be able to function independently within a team framework
Must possess excellent communication and organizational skills
Must be dependable in attendance and job performance
Must have valid driver's license in state of employment
QUALIFICATIONS PREFERRED

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
Maintains standards and policies set forth by BioMatrix Specialty Infusion Pharmacy.
Performs initial patient evaluation as it pertains to nursing care in accordance with BioMatrix Specialty Infusion Pharmacy policy and procedures.
Assists referring physician and/or institution in planning and implementing patient care in order to insure a smooth transition from hospital to home.
Provides the patient/significant other with information regarding the concept of home infusion therapy and the impact of such therapies.
Performs patient training in self-management of infusion therapies including the provision of necessary teaching materials.
Conducts patient visits in the home/clinic to perform the following:
Physical assessment
Nutritional assessment
Therapeutic monitoring to evaluate and manage potential mechanical, infectious, and metabolic complications.
Peripheral IV starts/re-starts
Obtaining labs via peripheral veni-puncture or central venous access device.
Assessing patient's ability to perform procedures and instruction of specific procedures.
Performing therapy specific nursing procedures (i.e., port access, CVC dressing changes, etc.)
Product delivery
Maintains thorough and appropriate documentation in the patient medical records of all visits and procedures performed as well as phone contacts with the patient, physician, or other members of the health care team.
Participates in patient care conferences.
Assists Director of Nursing in conducting special projects.
Participates in the agency Performance Improvement Program and Patient Safety Committee.
Actively participates in and follows agency Infection Control Program.

A Registered Nurse for Home Infusion inserts, monitors and removes an IV used to give medications, blood products and nutrition to a patient. When a patient needs blood levels checked, an IV nurse performs a veinous phlebotomy. On-Call:
Participation in the on-call rotation is a requirement of this position. It is anticipated that an incumbent in the position would be on-call approx. once every eight (8) weeks for two (2) weekdays per month and one (1) weekend.
Job Description:
A Registered Nurse for Home Infusion inserts, monitors and removes an IV used to give medications, blood products and nutrition to a patient. During the IV therapy, the nurse monitors the patient for adverse reactions and ensures that the tubing, bandages and needle stay in place. When a patient needs blood levels checked, an IV nurse performs a veinous phlebotomy. During the entire IV therapy process, the nurse obtains sets of vital signs, completes the physical assessment and review of systems, documents all patient-related activities and records the medications they use.
Provides and coordinates all aspects of staffing services to the patient with the assistance of the Company Directors, Nursing Coordinator and the Director of Nursing. Communicates with the patients to determine problems and/or solutions regarding their treatment. The Per Diem RN also conveys collected documentation from patient to all necessary internal and third-party external staff regarding scheduling and other pertinent patient information.
Performs a variety of duties both clinical and nursing notes documentation. This position requires skill in planning, organizing, scheduling and coordinating the delivery of patient care by all staff personnel including but not limited to complete documentation of nursing care and submission of nursing notes.
QUALIFICATION REQUIREMENTS
Nursing Degree from an accredited college or university required
IV certification & experience in central line care and management required
