Properly codes Professional Inpatient, Physician outpatient and technical charges for multiple clinics to ensure accuracy and optimal reimbursement from all third-party payers
Minimum Qualifications
High school diploma or equivalent and two years of medical billing or related experience, or related training from an accredited agency. The coder must be able to code & QA Outpatient Technical and Professional in 4 or more specialties. CCA, CCS, or CCSP coding certification from AHIMA, or CPCA, CPC, CPCHA coding certification from AAPC required within one year of hire. A successful completion of General Compliance Coder testing within 6 months of hire is also required.
Preferred Qualifications
PB/HB, Revenue Cycle, Coding, Charge Capture, Medicare, CMS Preferred.

Abstracting: Reviews medical records to determine accurate required abstracting elements (facility/client specific elements) including appropriate discharge disposition.
Coding Quality: Demonstrates consistency in achieving or exceeding 95.5% coding accuracy in the selection of principal and secondary diagnoses ((including DRG, MCC & CC, SOI/ROM)) and procedures. Demonstrates accuracy and consistency in abstracting elements defined by per facility.
Coder Productivity: Meets and/or exceeds Conifer's inpatient coding productivity guidelines
Physician Queries: Demonstrates strong skills in creating appropriate and compliant physician retrospective coding queries.
Professional Development: Stays current with AHA Official Coding and Reporting Guidelines, CMS and other agency directives for ICD-10-CM and ICD-10-PCS coding. Completes mandatory coding education as assigned. Quarterly review of AHA Coding Clinic. Attends all required coding operations conference calls.
DNFB: Reviews held accounts daily for resolution in support of coding DNFB performance. Communicates barriers to leaders ( physician queries, missing documentation, second level review, DRG reconciliation, etc.) for appropriate follow-up and resolution.

Compensation
Pay: $27.30-$40.95 per hour. Compensation depends on location, qualifications, and experience.

Job Summary
To support World Class Service Lines, and with Documentation Excellence (DE) as the primary objective, the Clinical Coder IV reviews clinical documentation and diagnostic results as appropriate to extract data and apply appropriate codes for billing, internal and external reporting, research and regulatory compliance. An option to work as part of the clinical team and perform high level, service line based concurrent coding is also available. Essential Functions
Reviews medical records of high complexity to identify the appropriate principal diagnosis and procedure codes, all other appropriate secondary diagnoses and procedure codes. Assign and present on Admission, Hospital Acquired Condition and Core Measure Indicators for all diagnosis codes.
Facilitates appropriate MS-DRG for inpatient medical records and appropriate APC assignment for outpatient medical records using UHDDS and other facility guidelines.
Demonstrates the technical competence to use the facility encoder as it interfaces with the hospital mainframe and/or EMR in an on-site or remote setting.
Reviews charges and Evaluation and Management levels.
Demonstrates proficiency with Microsoft Office Applications and in using required computer systems with minimal assistance.
Abstracts coded data and other pertinent fields in the hospital electronic health record.
Ensures the accuracy of data input.
Meets established quality and productivity standards.
Facilitates peer review and training for all Acute Clinical Coders in the coding department. Provides support to management.
Stay abreast of coding principles and regulatory guidelines related to inpatient and/or outpatient coding.
Physical Requirements
Must be able to concentrate and sit for long periods of time while reviewing electronic health records. Daily and weekly deadlines must be met in a fast paced office environment and/or at home environment.
Education, Experience and Certifications.
High school diploma or GED required; Bachelors degree preferred. Advanced knowledge in Medical Terminology, Anatomy and Physiology and Pharmacology required. 4 years coding experience in acute care setting required. Current RHIA, RHIT, CCS, CPC-H, CPC or CIC required plus a passing score on the CHS Coding test.

LifeGift is currently looking for a Medical & Death Record Review Auditor an outstanding candidate with an auditing healthcare background. The ideal candidate will responsible for timely, systematic review of retrospective medical record data obtained from hospitals and used for determining donor potential and assessing hospital performance. The auditor provides the data to support LifeGift strategic plans to maximize donation potential and improve donation processes in each hospital.
Do you possess the attributes to be a successful Medical & Death Record Review Auditor and perform the following essential functions?
Works with director to create a schedule designed to complete medical record reviews and death record reviews in a timely manner
Works with hospital staff to acquire access to hospital death lists and other appropriate records, utilizing remote electronic access when available
Audits medical records thoroughly and accurately for assigned hospitals to ensure compliance with CMS standards for death record reviews
Performs an analysis of appropriate referrals for timeliness and eligibility for organ donation
Investigates discrepancies in reporting; resolving inaccuracies in data and reporting deviations that require further review or follow-up
Ensures accuracy in data collection, data entry, and data analysis related to medical record review and donor potential
Analyzes results of reports and identifies patterns and trends in data sets
Documents all pertinent information in LifeGift's EMR and quality control systems
Reports findings of medical record reviews on a regular basis with appropriate internal partners
Completes data for hospital dashboards in a manner that allows for timely reporting
Acts as a resource for the medical record review process, data collection, and data interpretation, providing ongoing communication and training as needed with key staff
Assists in defining new data collection and development of reporting resources

Associate's degree or equivalent from two-year college or technical school 3 years related experience and/or training in a clinical or quality assurance role preferred.
Medical terminology and medical records & procedures experience required.
Organ and tissue procurement and/or transplantation experience preferred.

The Inpatient Coder is responsible for coding discharged inpatient encounters. May work in collaboration with Clinical Documentation Improvement nurses. **Job Summary:**
Uses information technology to provide reporting and analysis in support of internal and external customer requests for information and service relating to all aspects of building/campus documentation. Assists in the maintenance of departmental databases that provide information regarding building drawings, surveys, manuals, occupants, and facilities inventory.
**Job Duties:**
+ Manages the creation of building floor plans for UTMB facilities using CAD. Walks buildings and sites for space inventory tasks and to field verify construction and survey drawings. Communicates and coordinates with construction project delivery teams to support planning and activation tasks.
+ Uses the CAFM system to maintain building, room, and departmental information and generate building and departmental reports. Coordinates updates to the CAFM system in collaboration with the Vendor.
+ Leads in the processing, data verification, and filing of space surveys by others. Communicates with customers to resolve inquiries. Assists with user training sessions.
+ Understands relational databases and can use SQL tools to analyze and report on space data. Partners with customers to facilitate oral and written communications, including the design and delivery of presentations.
+ Manages the electronic document management system (EDMS) for archiving construction documents. Inspects and catalogs delivered documents. Trains and assists staff in accessing the EDMS, printing electronic facilities drawings, and other documents as needed.
+ Designs and produces organization charts, diagrams, and posters for various groups as required.
+ Learns new computer software, as the position requires.
+ Performs related duties as required.
**Knowledge/Skills/Abilities:**
+ Basic to intermediate AutoCAD 2D drafting and ability to read construction documents.
+ Working knowledge of relational databases.
+ Strong communication skills, organizational skills, and attention to detail.
+ Experience with Archibus or other CAFM systems preferred.
+ Experience with MS Access / SQL / Oracle databases preferred

**Salary Range** :
$63,120.00 to $72,000.00, actual salary commensurate with experience.
**Work Schedule:**
Partial Remote to be determined by the department. Must be willing to travel to different job sites. Monday through Friday, 8 am to 5 pm, and as needed on occasion.
