Start times will vary depending on the dock out times provided by the customer, there are no specific shifts, this is a 24/7 operation. This account is fast past and time commitments are essential for the customer. All condo trucks are freightliners equipped with automatic transmission. Drivers will be required to go with a mentor for 3 - 5 days. Trucks cannot be taken home, all trucks need to be parked and the Walmart DC. Drivers must have reliable transportation. This position is responsible for performing any and all of the tasks to receive, store, inventory and ship merchandise in a manner that meets company standards for accuracy, safety, security, productivity and quality.
Responsibilities:
1. Offload inbound merchandise, verify count and SKU, stack on pallets, label pallets, process documents, and relocate merchandise to proper location
2. Receive and process UPS inbound shipments and supply orders. Pick store supply and UPS orders and prepare for shipment
3. Move merchandise from receiving dock to proper location, process letdown sheet and re-label merchandise if necessary
4. Perform cycle counts, investigate and resolve inventory discrepancies
5. Pick orders for shipment
6. Move merchandise to staging area, verify SKU and quantity check for damage, and verify that order is complete and process documents
7. Verify merchandise order is accurate and complete, stack and wrap merchandise, load in trailer and process documents
8. Ensure job assignments are completed accurately and free of damage. Performance will be evaluated through periodic, random audits, and measured against the company accuracy standards
9. Operate forklifts, pallet jacks, and other material handling equipment as assigned
10. Conduct job duties in a manner that promotes safety. Complies with all OSHA and MSDS standards. Operates all equipment in a safe manner. On a daily basis, inspects equipment before use. Uses Personal Protective Equipment (PPE) as required. Maintains a clean, neat, and orderly work area
11. May perform other duties as assigned
Qualifications:
Experience and Education: High School Diploma or equivalent preferred

Professional Certifications: None

Other knowledge, skills or abilities:
• Ability to lift 50 to 75 pounds continuously throughout the shift, standing, bending, squatting and/or walking for extended periods throughout the shift
• Ability to read, speak and understand the English language on a basic level
• Ability to count accurately and perform basic math
• Ability to take initiative to correct issues without direction, offer solutions to problems and support a team effort
• Knowledge of operating any or all of the following: RF terminal and bar code scanner, 2-way radio, computer terminal or PC, lift truck, pallet jack, dock door leveler, stretch wrap machine

This position is responsible for performing any and all of the tasks to receive, store, inventory and ship merchandise in a manner that meets company standards for accuracy, safety, security, productivity and quality.
Responsibilities:
1. Offload inbound merchandise, verify count and SKU, stack on pallets, label pallets, process documents, and relocate merchandise to proper location
2. Receive and process UPS inbound shipments and supply orders. Pick store supply and UPS orders and prepare for shipment
3. Move merchandise from receiving dock to proper location, process letdown sheet and re-label merchandise if necessary
4. Perform cycle counts, investigate and resolve inventory discrepancies
5. Pick orders for shipment
6. Move merchandise to staging area, verify SKU and quantity check for damage, and verify that order is complete and process documents
7. Verify merchandise order is accurate and complete, stack and wrap merchandise, load in trailer and process documents
8. Ensure job assignments are completed accurately and free of damage. Performance will be evaluated through periodic, random audits, and measured against the company accuracy standards
9. Operate forklifts, pallet jacks, and other material handling equipment as assigned
10. Conduct job duties in a manner that promotes safety. Complies with all OSHA and MSDS standards. Operates all equipment in a safe manner. On a daily basis, inspects equipment before use. Uses Personal Protective Equipment (PPE) as required. Maintains a clean, neat, and orderly work area
11. May perform other duties as assigned

Experience and Education: High School Diploma or equivalent preferred

JOB SUMMARY: As a liaison between GMR Communication Centers and our contract facilities, you will work to build trusting relationships and assure that patient movement is a seamless process performed in a safe, timely, and appropriate manner. Under the direction of the Operations Manager, the Transportation Coord (TC) will receive process and coordinate medical and non-medical transports as required for local healthcare systems and skilled nursing facilities. The TC will work closely with non-GMR personnel and interact directly with clinical and social work staff at various facilities.

ESSENTIAL FUNCTIONS/DUTIES:
• Assist hospital staff with Level of Service (LOS). determination to help both the Hospital Staff and the Ambulance Crew understand why patient is moving at an appropriate LOS in real time.
• Ensure that all appropriate paperwork has been filled out and is complete and correct. While you will not fill out Hospital responsible paperwork, you should make sure it is in order and ready for the crew prior to transport (Ex: Physician Certification Statement (PCS) and any other paperwork that should accompany patient).
• Assist hospital team with patient movement to designated discharge unit.
• Coordinate and order Ambulance/CCT(s) for all applicable discharges.
• Assist in the elimination of delays to keep patient movement a seamless process.
• Provide information regarding transportation services to physicians and nursing personnel in hospital and healthcare facilities throughout the state.
• Provide patients with detailed information regarding the transport and advise transportation staff on medical needs of the patient during their transport (requires knowledge of the patient's medical records).
• Utilize information received from healthcare provider, patient, or family to coordinate medical and non-medical transportation.
• Recommend the appropriate unit(s) to respond based upon the type of call, priority, and availability of unit(s).
• Ask pertinent questions to determine type of transport needed, billing information, and destination of transport.
• Communicate verbally and in writing with GMR Dispatch centers and customer communications centers to ensure on-time performance and quality customer service.
• Suggest routes for medical and non-medical units to follow.
• Interact with GMR dispatch centers to ensure proper unit utilization.
• Report unusual incidents to duty supervisors as appropriate.
• Prepare timely and accurate paperwork as required.
• Serve as the Liaison between GMR and healthcare facilities.
• Assist with billing-related questions.
• Perform other duties as assigned

QUALIFICATIONS:
• 1 Year of related experience providing dispatching and/or transportation services to hospital patients.
• 2 Years of EMS industry experience preferred Education:
• High School Diploma or Equivalent
• Bachelor's Degree in related field preferred Skills:
• Must be familiar with local geography
• Must be proficient with Microsoft Office Applications
• Must Type 30 WPM
• Must have effective oral, written, and interpersonal communication skills

WORKING CONDITIONS AND MENTAL/PHYSICAL REQUIREMENTS:

The dispatch office is in a controlled temperature setting and under fluorescent lighting. Work is performed in a confined environment and can be highly stressful at times and plays a critical role in Public Safety delivery. Must take information from callers who may be excited, abusive, incoherent, ETOH and/or hysterical.
Environment requires handling multiple tasks and prioritizing while dealing with frequent interruptions.

PHYSICAL DEMANDS:
This position requires frequent sitting in front of computer monitors, with frequent and simultaneous using the phone, radio and computers, and monitoring multiple computer screens. This position requires the ability to work in close quarters and wear a telephone headset. This position requires occasional to moderate bending and twisting of the neck. This position may require occasional lifting of no more than 10 lbs. This position requires frequent simple grasping of both the left and right hands, as well as frequent computer typing. This position may require occasional pushing or pulling or reaching above or below shoulder level. This position requires close vision (clear vision at 20 inches or less), color vision (ability to identify and distinguish color), and the ability to adjust focus.

PRINCIPAL DUTIES AND RESPONSIBILITIES:

All duties and responsibilities are expected to be performed in accordance with Fresenius Kidney Care policy procedures standards of nursing practice state and federal regulations.

  • Performs all essential functions under the direction of the Clinical Manager (CM) and with the guidance from the Educator Preceptor or a CAP RN 2 3 4 or 5 for the first 6 months of employment.

  • Expected to complete requirements to advance to the next CAP level for which they qualify within 24 months from date of hire.

  • Performs ongoing systematic collection and analysis of patient data pre - during - post hemodialysis treatment for assigned patients and documents in the patient medical record adjusts or modifies the treatment plan as indicated and notifies Team Leader Charge Nurse Clinical Manager or Physician as needed.

  • Recognizes aspects and implications of patient status that vary from normal and reports to or collaborates with appropriate health team members for input.

  • Directs and provides in collaboration with direct and ancillary patient care staff all aspects of the daily provision of safe and effective delivery of chronic hemodialysis therapy to assigned patients.

  • The position provides direct patient care that regularly involves heavy lifting and moving of patients and assisting with ambulation. Equipment aids and/or coworkers may provide assistance. SUPERVISION:

  • Assigned oversight of Patient Care Technicians/LPNs as a Team Leader or designated Nurse in charge after the following:

    • Successful completion of all FKC education and training requirements for new employees plus

    • Must have a minimum of 9 months experience as an RN plus

    • Successful completion of 3 months experience in chronic hemodialysis.

EDUCATION:

  • Graduate of an accredited School of Nursing.

  • Current appropriate state licensure.

  • Must meet the practice requirements in the state in which he or she is employed.

EXPERIENCE AND REQUIRED SKILLS:

  • Entry level for RNs with less than 2 years of nephrology nursing experience within the last two years or for RNs with a BSN and less than 1 year of nephrology nursing experience within the last two years.
  • Minimum of 9 months experience as a Registered Nurse (preferred).
  • Chronic hemodialysis experience (preferred).
  • Successful completion of FKC Education and Training requirements for new employees.
  • Successful completion and maintenance of CPR BLS Certification.
  • Meet all conditions of employment including Ishihara's Color Blindness Test.
  • Successful completion of all annual required education/training/competencies.

JOB SUMMARY: As a liaison between GMR Communication Centers and our contract facilities, you will work to build trusting relationships and assure that patient movement is a seamless process performed in a safe, timely, and appropriate manner. Under the direction of the Operations Manager, the Transportation Coord (TC) will receive process and coordinate medical and non-medical transports as required for local healthcare systems and skilled nursing facilities. The TC will work closely with non-GMR personnel and interact directly with clinical and social work staff at various facilities.

ESSENTIAL FUNCTIONS/DUTIES:
• Assist hospital staff with Level of Service (LOS). determination to help both the Hospital Staff and the Ambulance Crew understand why patient is moving at an appropriate LOS in real time.
• Ensure that all appropriate paperwork has been filled out and is complete and correct. While you will not fill out Hospital responsible paperwork, you should make sure it is in order and ready for the crew prior to transport (Ex: Physician Certification Statement (PCS) and any other paperwork that should accompany patient).
• Assist hospital team with patient movement to designated discharge unit.
• Coordinate and order Ambulance/CCT(s) for all applicable discharges.
• Assist in the elimination of delays to keep patient movement a seamless process.
• Provide information regarding transportation services to physicians and nursing personnel in hospital and healthcare facilities throughout the state.
• Provide patients with detailed information regarding the transport and advise transportation staff on medical needs of the patient during their transport (requires knowledge of the patient's medical records).
• Utilize information received from healthcare provider, patient, or family to coordinate medical and non-medical transportation.
• Recommend the appropriate unit(s) to respond based upon the type of call, priority, and availability of unit(s).
• Ask pertinent questions to determine type of transport needed, billing information, and destination of transport.
• Communicate verbally and in writing with GMR Dispatch centers and customer communications centers to ensure on-time performance and quality customer service.
• Suggest routes for medical and non-medical units to follow.
• Interact with GMR dispatch centers to ensure proper unit utilization.
• Report unusual incidents to duty supervisors as appropriate.
• Prepare timely and accurate paperwork as required.
• Serve as the Liaison between GMR and healthcare facilities.
• Assist with billing-related questions.
• Perform other duties as assigned

QUALIFICATIONS:
• 1 Year of related experience providing dispatching and/or transportation services to hospital patients.
• 2 Years of EMS industry experience preferred Education:
• High School Diploma or Equivalent
• Bachelor's Degree in related field preferred Skills:
• Must be familiar with local geography
• Must be proficient with Microsoft Office Applications
• Must Type 30 WPM
• Must have effective oral, written, and interpersonal communication skills

WORKING CONDITIONS AND MENTAL/PHYSICAL REQUIREMENTS:

The dispatch office is in a controlled temperature setting and under fluorescent lighting. Work is performed in a confined environment and can be highly stressful at times and plays a critical role in Public Safety delivery. Must take information from callers who may be excited, abusive, incoherent, ETOH and/or hysterical.
Environment requires handling multiple tasks and prioritizing while dealing with frequent interruptions.

PHYSICAL DEMANDS:
This position requires frequent sitting in front of computer monitors, with frequent and simultaneous using the phone, radio and computers, and monitoring multiple computer screens. This position requires the ability to work in close quarters and wear a telephone headset. This position requires occasional to moderate bending and twisting of the neck. This position may require occasional lifting of no more than 10 lbs. This position requires frequent simple grasping of both the left and right hands, as well as frequent computer typing. This position may require occasional pushing or pulling or reaching above or below shoulder level. This position requires close vision (clear vision at 20 inches or less), color vision (ability to identify and distinguish color), and the ability to adjust focus.

MENTAL REQUIREMENTS:
The position requires the ability to compare, analyze, communicate, coordinate, evaluate, instruct, and negotiate. This position requires strong interpersonal skills.
