Supervises assigned residential program area to ensure the heath, safety and appropriate habilitation efforts on behalf of individuals served in accordance with VALLEYLIFE Policy and Procedures.
Position Responsibilities
Provides supervision and direction to program staff and ensures VALLEYLIFE Policy and Procedures are followed.
Fulfills responsibilities of behavior management programs, including program development, training of direct care staff, monitoring data on the effectiveness of the BTP program objective and attending all medication reviews.
Ensures program is individualized and appropriate for individuals served and meets contract requirements.
Completes all required areas of the Individual Service Plan (ISP) and attends meetings. Ensure ISP's are followed.
Maintains accurate client records and ensures all pertinent documentation is filed in each individual client's master file and back up files on campus.
Ensures that needs of individuals served are met by providing a safe, appropriately supplied and adequately staffed program.
Ensures all staff is trained on the needs and communication of each client.
Ensures each staff completes a clinical for medication administration within 14 days on the job.
Ensures timely orientation to group home including the completion of the client site on the first day in the home and the New Employee training within first two weeks on the job.
Ensures medication is administered per doctor's orders and medication is refilled as required to ensure adequate supply of medication.
Follow all policies and procedures, standards and licensing requirements.
Follow all policies and procedures regarding attendance, including punctuality and notification.
Participates in departmental meetings, inter-department meetings, committee meetings, and in-service training sessions as required.
Assist with move in of new clients, ensuring all staff is trained on the new clients needs within 24 hours of admission.
Maintains awareness of all rules and regulations and ensures compliance.
Ensures compliance with state rules and regulations are maintained with goal of 0 non-compliances each review.
Effectively manage household budgets; staying within budget limits.
Manage the client's funds according to the ISP client expenditures.
Coordinates communications of information regarding clients, to parents, guardians, Valley and other agencies serving the client.
Quarterly, completes and submits detailed Article 7 review.
Is on call for the group home 24 hours a day, 7 days a week.
Attend each annual physical, medication review, wheelchair evaluation, and any other medical appointment as deemed necessary to meet the needs of the clients.
If required, schedule staff and provide substitutes if necessary.
Performs additional duties as required.
Requirements
High School Diploma or equivalent. AA Degree is preferred.
Minimum 21 years of age.
Two years experience providing habilitation services to individuals with developmental disabilities; Director may waive if training program completed.
One year of supervisor experience is preferred.
Must have a valid Arizona Driver's License with a safe driving record as determined by insurance requirements.
Up to date on all required certifications.
Physical Demands
Ability to lift 40-60 pounds a distance of four feet, a frequency up to 20 times in an eight hour shift and 35 times in a twelve hour shift.

PRINCIPAL DUTIES AND RESPONSIBILITIES:
Patient Assessment / Care Planning / Counseling
As a member of the interdisciplinary team, assesses patients' psychosocial status, strengths and areas of need that may affect rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment.
Participates in care planning in collaboration with the patient and healthcare team to identify effective interventions that will help the patient meet rehabilitation, treatment goals, and improve quality of life.
Utilizes FKC patient education programs, established social work theory and methods, social work focused interventions, and quality of life measurement instruments as part of assessment and care planning to address barriers and meet patient treatment goals.
Provides monitoring and interventions for the patient to adjust to dialysis and achieve optimal psychosocial status and quality of life.
Provides supportive counseling services to patients as permitted within the scope of their clinical training and state license.
Provides educational and goal directed counseling to patients who are seeking transplant.
Provides information and assists the team and patient with referral to community resources (home health services, vocational rehabilitation, etc.) to facilitate optimal treatment outcomes.
Maintains current knowledge regarding local vocational/educational rehabilitation programs and assist patients with referral and access to vocational rehabilitation to enable them to remain employed, become employed or receive education.
Assesses patient awareness of advance directives; assists with accessing advance directive forms/information and facilitates discussion of advance directive wishes, if necessary, with the healthcare team and the patient's family/support persons.
In collaboration with the physician and nurse, participates in the discussion of patient DNR status in the facility to ensure patient and/or family understand and make an informed decision about their care.

Quality

Patient Education
Assesses patient knowledge of kidney disease for barriers that may affect adherence to treatment. Works with patient, family and health care team to provide education tailored to the patient's learning style, communication barriers, and needs.
With other members of the interdisciplinary team, provides appropriate information about all treatment modalities.
Facilitates the transplant referral process and collaborates with interdisciplinary team on transplant waitlist management.
Provides ongoing education to patient/family regarding psychosocial issues related to End Stage Renal Disease (ESRD) and all support services that are available.
Reviews patient rights and responsibilities, grievance information (company and network) and other facilities policies with patient and/or the patients' representative to ensure patients' understanding of the rights and expectations of them.
Collaborates with the team on appropriate QAI activities.
Patient Admission and Continuity of Care
Reviews Patient Rights and Responsibilities, Grievance Procedure & Important Numbers Handout, FKC Non-discrimination policy, DNR Statement (if applicable) and address any immediate needs/concerns.
Understands the referral and admission process and supports the clinic in regard to the patient needs for scheduling to maximize adherence and adjustment.
The Social Worker will interview the patient to identify root causes or concerns for the discharge request, (i.e. transfer to hospice, relocation, dissatisfaction with services or staff) and share causes/concerns with operational leadership.
Insurance and Financial Assistance
Collaborates and functions as a liaison for patient with Insurance Coordinators to address issues related to insurance.
In collaboration with Insurance Coordinators, provides information and education to patients about payment to dialysis (federal, state, commercial insurance, state renal programs, AKF HIPP, and entitlement programs).
Collaborates with the Insurance Coordinator of any changes to patient state that impacts insurance i.e. transplantation, discharged, loss of coverage, or extended travel.
Refers patients to patient billing solutions (PBS) department for questions/concerns in regard to treatment related bills
Staff Related
Assists with interview process and decision to hire new personnel if requested by SW Manager/Senior Manager.
Works with the administrative support staff to maintain updated patient resource lists (e.g. maintain updated list of transportation resources).
Provides training to staff pertaining to psychosocial topics as needed.
Contributes and participates with weekly team huddles. Discusses any urgent patient issues with staff.
Adheres to work defined caseload guidelines based on state regulatory requirements.
Performs other related duties as assigned.

Maintenance:
Vehicles: When applicable, test drives vehicle monthly; inspects vehicle lifts and seatbelts monthly; reports safety concerns immediately to supervisor or maintenance personnel; assures vehicle is washed and cleaned; trains staff in safe vehicle operation and safely transporting individuals; ensures routine maintenance is performed.
Living Environment: When applicable, schedules and monitors daily and seasonal housekeeping; oversees lawn and yard care. Maintains neat, clean and safe environment for individuals served.
Maintenance and Repair: When applicable, makes or arranges for minor repairs; reports to and schedules repairs by maintenance personnel as required; monitors environmental safety; monitors water temperature, refrigerator and freezer temperature, and safety alarms and extinguishers on a monthly basis; replaces furnace filters monthly.
Equipment: Monitors wheelchairs, lifts and other safety equipment; sees that repairs are made promptly and correctly in the event of faulty equipment.
Other:
Performs other duties and activities as required, including backfilling roles under your supervision.
MPA = Mobile Punch Authorized for timekeeping.
SUPERVISORY RESPONSIBILITIES
Supervises the Direct Support Professionals.
Manages assigned personnel. Completes performance evaluations, orientation, and training. Makes decisions on employee hires, transfers, promotions, salary changes, discipline, terminations, and similar actions. Resolves employee problems within position responsibilities.

POSITION SUMMARY
This position is responsible for providing scientific and clinical direction to the HLA clinical laboratory and reports to a designated member of the SQL/LSA senior leadership team and a Medical Director. This position is responsible for oversight of all licensed and unlicensed personnel in the HLA department and serves as a mentor and assists in the development of HLA Director in Training (DIT) candidates per ACHI. Responsible for providing technical interpretation of results, providing consultations with physicians, overseeing research projects, implementing new assays/assay troubleshooting, quality assurance, and reviewing SOPs. Assumes Clinical Consultant responsibilities and participates on site and system-level committees as assigned/required. This position will hold a faculty appointment with the University of Arizona Department of Pathology.
Performs all functions according to established policies, procedures, regulatory and accreditation requirements, as well as applicable professional standards.
MINIMUM QUALIFICATIONS
PhD in in a chemical, physical, biological, or clinical laboratory science.
Two (2) years appropriate specialty experience.

Join the growing team of physicians/clinician-scientists at Banner Sun Health Research Institute (BSHRI) that are setting international benchmarks in neurodegenerative clinical care, research and education in Alzheimer's Disease and related dementias, and Parkinson's Disease and other movement disorders. We're seeking a Neurology Advanced Practice Provider, to support our memory and cognitive disorders neurology subspecialty clinic. Ideal candidates will have the desire to work in a diverse clinical, research, and educational/training setting focused on memory/cognitive disorders. Our nonprofit mission is to make a positive impact on these patients through multidisciplinary care. This is an excellent opportunity for a dynamic NP/PA with experience in Neurology, Psychiatry, or geriatrics to join our highly specialized team which includes neurologists, geriatric psychiatrists, geriatricians, nurse practitioners, neuropsychologists, and care navigators. New graduates looking to be mentored in their early career are also encouraged to apply. If you have a passion for combining comprehensive interdisciplinary patient care, in a highly resourced, non-WRVU-driven outpatient setting, with clinical research/trials and teaching opportunities, this could be a great fit for you.
BSRHI is nationally and internationally recognized for cutting edge care that integrates research opportunities. BSHRI is just one of 30 NIH-funded national Alzheimer's Disease Research Centers in the nation. Additional funding resources include the National Institutes of Health, the Foundation for NIH, the Michael Fox Foundation, and other philanthropic foundations. BSHRI maintains collaborations with nationally and internationally recognized centers such as Johns Hopkins University, Boston University, Washington University in St. Louis, Brigham and Women's Hospital and Harvard Medical School.
Qualifications:
Current Arizona state NP or PA licensure
Neurology, psychiatry, or geriatric experience preferred but willing to train new graduates
Provides compassionate and empathetic care
Develops, refines, and implements effective healthcare strategies through lifelong learning
Diligent in maintaining accurate electronic medical records
Develops and maintains productive, professional relationships with physicians and staff to facilitate effective communication
