The Patient Service Representative provides customer service to all customers, both over the phone and at the check-in area, for all arriving and departing patients. This position is responsible for scheduling future appointments both in person and via the telephone, registering new patients in the EHR, checks registration forms for completeness and accuracy, and scans registration into patient EHR. Job Responsibilities: * Accurately register new patients in the OOH electronic medical record (EMR) system, checking to see if the patient was previously registered and verify demographic and insurance information for accuracy. * Promptly and courteously, greet arriving patients and guests. * Promptly check patient in, including, but not limited to accurately updating insurance, yearly demographic information (registration form) and collecting any payments required from the patient. Promptly check patient out. * Process payments received via mail, phone, or in-person. * Schedules future appointments as needed, at both checkout window and over the phone. * Refer patients to Patient Accounts Office if financial plans are necessary. * Scans all completed patient paperwork into patient EMR in a timely manner. * Maintain phone etiquette; answer phones, transfer calls, take accurate messages and assign appropriately, document prescription refills and assign to correct provider. * Monitors appointment schedules * Respond to Patient/Physician inquiries * Reschedule patient appointments when providers call in sick or schedule time off. * Direct patients that need assistance with their health insurance to the embedded Facilitated Insurance Enroller. * Call appointment reminders for physicals and all new patient appointments and change appointment status to "confirm" when done. Call failed appointment reminder calls to attempt to confirm appointment. * The ability to communicate effectively with patients and staff from all departments. * Attend regular staff meetings, trainings, and other meetings as requested. * Maintain a safe, clean and orderly work area at the front desk and waiting room. * Schedule interpreters (internal or external) for patient appointments. * Balance cash receipts at the end of shift and prepare appropriate reports. * Make sure the building is unlocked in the morning, and locked at night with the alarm set by the last employee to leave the building. * Adheres to all organizational policies and procedures. * Any other reasonable requests from management. The Clinical Performance and Quality Manager is responsible for the overall development, coordination, monitoring and presentation of mandatory trainings, ongoing staff development and performance of the Oak Orchard Health Medical Department. The Clinical Performance and Quality Manager will directly report to the Director of Nursing and oversee the Nurse Supervisors. This position will ensure the standardization of workflows, training and competency requirements. Additionally, optimization of clinical standards will be audited monthly and coached consistently to ensure quality measures are met within the organization. The position requires experience in education program development, preferably in the health care environment, with demonstrated ability to present educational information. Candidates must be able to communicate effectively with all levels of the organization and build a strong rapport with those around them. Must be comfortable with technology, specifically eCW and Microsoft Office, and demonstrate a high degree of organizational skills and take initiative to problem-solve. Coordinate with HR to enroll staff into BLS training classes. * Collaborate with Director of Nursing and Chief Medical Officer to develop role-specific training plans, competency checks for clinical skills and point of care testing, and in-service content for nursing staff. * Complete quarterly performance audits with reporting on quality measures and partner with DON to review and troubleshoot areas for improvement. * Provide training for all Nurse Supervisors, LPNs and MAs on nursing skills and point of care testing machines (maintenance, testing and quality controls), and complete competency checks for nursing skills and machines used at the point of care annually. * Provide training to RNs on nursing skills and point of care testing as well as basic RN skills such as document review, TOC processing, INR tracking/management and Chronic Care Management (CCM). * Provide training for all Nurse Supervisors, including ensuring VFC/VFA compliance and training, monthly duties and Nurse Supervisor audits. * Provides leadership for the Nurse Supervisor team including support of the nursing team, troubleshooting day-to-day operations, payroll processing and supporting daily needs of the nursing team. * Provide the first week of in-person orientation for all nursing staff, including role-specific training on EMR, workflows and point of care testing machines and completion of competency checks for nursing skills and machines used at the point of care. * Create training materials for each trainee, including skills and information binders that nurses can add to as they grow with Oak Orchard. * Partner with the DON to confirm site-based compliance of the medical team with governing bodies such as Patient Centered Medical Home (PCMH) and The Joint Commission (JCO). * Travel to satellite locations for both support of the locations and to conduct training and skills assessments. * Identify individual nursing needs to address opportunities for improvement and ensure optimal nursing retention and reduce turnover. * Travel to all locations on a monthly basis to meet with nurse Supervisors and ensure workflow compliance. * Attend meetings as required and participate in continuing learning and education. * Any other reasonable task as assigned. Coordinate with HR to enroll staff into BLS training classes. · Collaborate with Director of Nursing and Chief Medical Officer to develop role-specific training plans, competency checks for clinical skills and point of care testing, and in-service content for nursing staff. · Complete quarterly performance audits with reporting on quality measures and partner with DON to review and troubleshoot areas for improvement. · Provide training for all Nurse Supervisors, LPNs and MAs on nursing skills and point of care testing machines (maintenance, testing and quality controls), and complete competency checks for nursing skills and machines used at the point of care annually. · Provide training to RNs on nursing skills and point of care testing as well as basic RN skills such as document review, TOC processing, INR tracking/management and Chronic Care Management (CCM). · Provide training for all Nurse Supervisors, including ensuring VFC/VFA compliance and training, monthly duties and Nurse Supervisor audits. · Provides leadership for the Nurse Supervisor team including support of the nursing team, troubleshooting day-to-day operations, payroll processing and supporting daily needs of the nursing team. · Provide the first week of in-person orientation for all nursing staff, including role-specific training on EMR, workflows and point of care testing machines and completion of competency checks for nursing skills and machines used at the point of care. · Create training materials for each trainee, including skills and information binders that nurses can add to as they grow with Oak Orchard. · Partner with the DON to confirm site-based compliance of the medical team with governing bodies such as Patient Centered Medical Home (PCMH) and The Joint Commission (JCO). · Travel to satellite locations for both support of the locations and to conduct training and skills assessments. · Identify individual nursing needs to address opportunities for improvement and ensure optimal nursing retention and reduce turnover. · Travel to all locations on a monthly basis to meet with nurse Supervisors and ensure workflow compliance. · Attend meetings as required and participate in continuing learning and education. · Any other reasonable task as assigned. The Registered Nurse is responsible for a team-based nursing approach to managing patient needs. This RN will provide care coordination, advocacy, coaching and education for patients that are at risk for hospital admissions or ER visits. This nurse will also assess, manage and support patients, provide appropriate documentation and work collaboratively with the entire health care team. Direct patient care will give the RNs the opportunity to work to their full scope of patient care. Our goal as an organization is to have a team of RNs at each location working together to give our patients the best quality care for both chronic conditions and preventative measures, as well as improve quality measures for Oak Orchard Health Job Responsibilities: * Identify patients with high risk for adverse health outcomes, establish relationships with these patients to enable effective intervention and support. Create goals with patients and develop plan of care. * Identify barriers of care to high risk patients, implement care plans with patients to provide self-management and support interventions. Optimize patient access to needed services through insurance and other benefits. * Facilitate effective transition of care through the provision of comprehensive follow up to in-patient and skilled nursing admission when discharged home. Provide patient education in regard to discharge instructions and medication questions. * Promote health maintenance in high risk population patient panel, and participate as a member of the Care Team. Work with outside agencies and providers to share patient information for best health outcomes. * Assess incoming phone calls and walk-in patients needing triage. * Assess and provide follow up for patient ED and UC visits. * Assist in population health efforts focusing on preventative care and chronic condition management. * Monitor patients on chronic anticoagulation therapy. * Work in RN capacity to facilitate community outreach efforts such as vaccine clinics and direct patient care in the clinical setting. * Provide culturally competent interventions based on patient assessment and identified cultural needs. * Perform other duties as assigned. Requirements Skills / Qualifications: * Excellent communication skills and ability to form collaborative partnerships across all service settings * Working knowledge of the provision of health care in a variety of settings * Knowledge of community resources preferable * Ability to engage and build relationships with patients * Ability to prioritize workload and assess the need for collaboration with the health care team * Basic computer literacy skills, with competence in Microsoft Office products such as Work, Excel, Outlook and PowerPoint, and experience working in an electronic health record (EHR) * Ability to assimilate new information and technologies into daily work * Bilingual desired but not required * Flexibility in schedule - some evenings and weekends may be required Education/Experience: * Graduate of an approved school of nursing with RN license to practice in NYS.
