Job Summary:
The Primary Care Advanced Practice Provider (APP) collaborates with a multidisciplinary healthcare team in an ambulatory Primary Care and Internal Medicine practice setting to provide care to individuals and families across the lifespan. The APP has didactic education, clinical competency, and national certification in the identified area of clinical practice, enabling the delivery of preventative healthcare for individuals and families and the assessment, diagnosis, and treatment of acute and chronic illness. Primary Care APPs demonstrate an ongoing commitment to patient-centered care and partner with individuals and families to promote engagement and shared decision-making to ensure optimal health, well-being, and self-management of disease processes. KEY RESPONSIBILITIES

We are looking for experienced APPs who are comfortable working independently and willing to build bridges as we develop relationships with new teams. Our regional hospital ICUs have between 10-14 ICU beds. They serve a mixed patient population, with the majority of patients being medical. The goal staffing model is 24-hour coverage with one day and one night APP (traditional 13-hour shifts) at each hospital; however, during the initial period the APP will cover both ICUs but home based at VTHH.

The nurse practitioner (NP) within the acute care hospital setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. Department Summary:
VPEC has two locations: The Vanderbilt Clinic (TVC) and One Hundred Oaks (OHO), both are fully staffed clinics with 11 Nurse practitioners, 5 registered nurses and multiple MAs and PSRs. The staff at either location performs: vitals, images, sleep apnea screening and a complete review of systems and history and physical on patients undergoing elective surgeries. The nurse practitioners performs these visits in person, while the registered nurses use a virtual phone call model. We can also arrange for lab work and x-rays to be done at the time of the VPEC visit or through a local primary care doctor for our phone call patients. We gather any outside testing and can order electrocardiograms, cardiac consults, stress tests and echocardiograms if felt needed by the attending anesthesiologist.
Patients are usually referred to the clinic by one of our surgeons for medical optimization prior to the day of surgery. This process allows our team of Nurse Practitioners to ensure that preoperative patients are properly evaluated, educated and that all necessary testing and patient information are in hand before procedures. This is a free evaluation to assess the patients overall risk for surgery. This better ensures patient safety and helps prevent surgical delays and cancellations on the day of surgery. Once the patient evaluation is completed at VPEC, it becomes part of the patients electronic medical record and is accessible by any Vanderbilt medical provider.

We are seeking a dedicated and skilled PhysicianAssistant (PA) to join our organ recovery team. The PA will play a critical role in the surgical recovery of organs for transplantation, collaborating with transplant surgeons, donor hospitals, and the recovery team to ensure high-quality procurement procedures.
Department Summary:
The Division of Hepatobiliary Surgery and Liver Transplantation participates in a comprehensive medical-surgical-radiologic hepatobiliary referral service, emphasizing diagnosis and medical management of liver and bile duct diseases with a variety of treatment strategies, including liver resection, RF ablation, alcohol injection, chemoembolization, portosystemic shunting, TIPS, laparoscopic procedures, and biliary bypass, as well as liver transplantation.
The liver transplantation program is part of the Vanderbilt Transplant Center, which represents a major commitment to the expanding and important field of transplantation. The heart, lung, bone marrow, kidney, and pancreas transplantation programs closely interact with one another. The division also participates in a comprehensive diagnostic and treatment program in hepatobiliary disease, offered by the Vanderbilt Clinic. This multidisciplinary group evaluates patients to establish a diagnosis, assess the severity of the disease and develop a treatment plan.

Preferred Qualifications:
* At least 5 years of experience as a PhysicianAssistant
* Strong preference for PA's with experience in organ procurement, cardiac surgery, or abdominal surgery
KEY RESPONSIBILITIES
* Perform or assist in surgical organ recovery procedures under the supervision of transplant surgeons.
* Provide comprehensive clinical assessments and manage donor physiology pre-recovery.
* Document and report findings to transplant centers and organ procurement organizations.
* Ensure accurate and timely completion of required donor medical records and recovery documentation.

POSITION SHIFT
* Nights 7p-7a - All positions will work some weekends and holidays.
The nurse practitioner (NP) or physicianassistant (PA) within the acute care hospital setting works in collaboration with, and is an integral member of, a multidisciplinary health care team. The NP or PA functions primarily as a hospitalist in an inpatient setting, managing the care of acute/complex chronically ill patients. The NP or PA is typically on a service aligned team managing the patient care from step down to discharge. The NP or PA may have responsibilities for same patient population during follow up outpatient visits. The NP or PA has a strong foundation in evidenced based practice/education and provides an environment of safe quality care. The NP or PA demonstrates an advanced level of medical and nursing knowledge, clinical/technical competence, sound clinical judgment, professional communication skills, timely and compliant documentation, and is responsible for professional development and competency validation.
KEY RESPONSIBILITIES
* Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care.
* In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patient's overall care, identifying expected outcomes for diagnoses.
* Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
* Forms differential diagnoses and treats acute and chronic conditions.
* Prescribes therapeutic interventions both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes.
* Utilizes evidence-based, approved practice protocols in planning and implementing care;
* Initiates appropriate referrals and consultations;
* Provides specialty specific consultation services upon request and within specialty scope of practice;

KEY RESPONSIBILITIES
* Assessment of Health Status
* Performs and documents in the medical record a complete history and physical examination for acute and complex chronically ill patients.
* Orders and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated.
* The nurse practitioner may serve as the first responder for both nursing and house staff in the event of patient status changes, orders, and in situations requiring procedural and resuscitative interventions.
* Assesses for risks associated with the care of the acute and complex chronically ill patient including: medication side effects, immobility, impaired nutrition, fluid and electrolyte imbalance, immunocompetence, invasive interventions and diagnostic procedures.
* Coordinates daily rounds with the Attending physician teams, presents 24 hour patient report, collaborates in plan of care, and follows assigned patient population throughout the day.
* Diagnosis
* Collaborates with interdisciplinary health care team in making diagnoses of acute and complex chronic conditions.
* Manages diagnostic tests through ordering, interpretation, performance, and supervision.
* Formulates differential diagnoses by priority.
* Diagnoses complications and orders appropriate interventions
* Formulates Plan of Care
* Identifies expected outcomes from diagnosis, and formulates and documents a plan of care to address complex acute and chronic health care needs.
* Utilizes evidenced based practice guidelines or protocols in an individualized, dynamic plan of care that can be applied across the continuum.
* Implements and modifies plan of care
* Prescribes diagnostic strategies and therapeutic interventions both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes.

The Vanderbilt Colon and Rectal Surgery program and its highly skilled surgeons offer a range of approaches for colorectal cancer and other conditions of the colon and rectum. Department Summary:
Vanderbilt's Department of Plastic Surgery is one of eight departments of the Section of Surgical Sciences. Plastic surgery was originally one of several specialties under the original Vanderbilt Department of Surgery in 1925. Today our Department is a vital service of Vanderbilt University Medical Center. On the SICU at VUMC, we care for critically ill adult patients 18 years and older who are recovering from various surgical interventions and procedures. Our unit is unique because of the diversity of our patient population. We care for liver transplants, emergency general surgery, vascular, thoracic, orthopedics, surgical oncology patients, and frequently care for trauma and OB patients. This provides endless opportunities for learning and skillset expansion.
