Provide urgent care services as needed, ensuring timely intervention for acute health issues. Collaborate with interdisciplinary teams to coordinate patient care and optimize treatment strategies. Utilize electronic health record systems such as Cerner for documentation and patient management. Educate patients and their families on disease prevention, health maintenance, and treatment options. Monitor patient progress and adjust treatment plans as necessary, focusing on infection control and chronic disease management. Participate in hospice palliative medicine initiatives to support patients with serious illnesses. Qualifications Master?s degree in Nursing or related field with a focus on Nurse Practitioner training. Current Nurse Practitioner certification and state licensure. Experience in geriatrics, urgent care, or senior care preferred. Proficient in using electronic medical records systems (Cerner experience is a plus). Strong skills in patient assessment and management of chronic conditions. Knowledge of durable medical equipment (DME) protocols is advantageous. Excellent communication skills with a compassionate approach to patient care. Ability to work collaboratively within a healthcare team while maintaining a high standard of professionalism. Writes, faxes and/or phones in prescriptions for patients. Liaisons between RI physician and outside consulting physicians regarding patient care. Appropriately select diagnostic tests. Accurately interpret test results including labs, X-rays, and complete documentation. Synthesize information accurately from PE, test results, written forms, EMR and patient verbal report. Integrate the patient?s PMHx and PSHx into decision making regarding orthopedic treatments. Appropriately order pharmaceuticals (antibiotics, anticoagulants, pain management), braces, therapies, weight-bearing status.
