variable kennel shifts evenings 3pm to 7pm, weekend rotations
The Animal Care Center at Cherry Way is looking for a part-time Pet Care Attendant and Pet Bather to help us provide the best care for pets staying with us.
The responsibilities associated with this position include:
Feeding dogs and cats and administering medication if needed
Cleaning cages
Walking dogs
Noticing and reporting any issues a pet may be having to our veterinary staff
Maintaining overall cleanliness of kennel, grooming, lobby, and outside of building
Bathing and drying dogs and cats to get ready for our groomers to complete the groom
We are looking for someone who loves animals, is attentive, caring, and hard-working. Must be reliable and a good team player. Our team is committed to delivering the highest quality care to our clients and their pets they entrust in our care.

Required Qualifications:
Undergraduate degree

The Community Health Worker is responsible for assessment, planning and implementing care strategies that are individualized by patient and directed toward the most appropriate, least restrictive level of care. This position is a field-based position with a home-based office. You will work from home when not in the field.
If you reside within Hamilton County, you will have the flexibility to work remotely* as you take on some tough challenges.
Primary Responsibilities:
Engage members either face to face or telephonically to assist with closing gaps in care, linking to necessary services and providing education about their health
Review available member services records and relevant documentation (e.g. utilization history, functional level, stratification information)
Conduct member health assessments that include bio-psychosocial, functional, and behavioral health needs
Utilize interviewing techniques and active listening to collect and retain member information and incorporating responses as they are presented to complete assessment
Identify member service needs related to health concerns
Identify urgent member situations and escalate to next level when necessary
Engage member to participate in the assessment process and collaboratively develop Health Action Plan based on their individual needs, preferences, and objective with nursing oversight
