We are opening up a brand new women's RRS to provide client-centered care and recovery-oriented treatment within a therapeutic environment. We are looking for innovative staff for our licensed Substance Abuse Treatment programs, which are open to both veterans and the community. The primary goal of this position is to assist patients in obtaining necessary services by providing information, coordinating referrals, and following through with specific treatment plan goals. WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY: Provides supportive case management necessary for individuals to access medical, social, vocational, educational, substance abuse and/or mental health treatment supports and community-based recovery services. Assists clients with researching and accessing needed services. Conducts referrals and follow-up monitoring, and participates in discharge planning process. Communicates with other service providers and family members, with appropriate releases, to coordinate the client's next level of treatment services. Schedules appointments and supports attendance at scheduled treatment and human services appointments. Coordinates with other providers to monitor clients' health status and participation in treatment. Monitors treatment progress and implementation of the client's treatment plan. Conducts outreach activities to market Veterans Inc. programs and engage new referral sources. Maintains audit ready client files and documentation and facilitates client groups, as needed. Attends and participates in all meetings as required. Performs special projects and other duties as assigned. WHAT YOU MUST HAVE: Minimum of Bachelor's degree in Human Services, Social Work, Psychology, or related field from an accredited institution. Master's degree in related field highly preferred. Minimum of two (2) years of case management, substance abuse treatment counseling experience required. Demonstrated knowledge of military culture, experience in working with veteran population and an understanding of veterans-related needs and issues preferred. Two (2) years of experience facilitating groups or workshops preferred Must have strong computer skills with intermediate knowledge of Microsoft Office Suite. WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY: Performs initial intake and assessments of all veterans for acceptance into Veterans Inc. programs. Develops individualized service plans for homelessness prevention and or rapid rehousing based on veterans input, interests, and needs. Coordinates veterans' participation in the agency programs. Assists veterans with researching and accessing needed services including VA benefits, employment training/placement , educational support, counseling, housing, medical services and other public benefits such as health care services, daily living services, personal financial planning services, transportation, fiduciary and payee services, legal services, child care, housing counseling services, and temporary financial assistance. Tracks and reports case management progress through case notes and conferences with Program Manager and coordinates with all funding sources. Completes record keeping/documentation responsibilities and compiles data for monthly reporting into the agency database. This position collaborates as a part of an interdisciplinary treatment team within substance abuse treatment programs. The position provides guidance and counseling support as needed by overseeing and accompanying the patient in various program activities to ensure conformance with program schedules, standards, and policies. The hourly base rate starts at $20.00 an hour. There is a $2 shift differential for working 2nd or 3rd shifts. WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY: Works with the patient and treatment team to develop substance abuse treatment goals and interventions, based on client's substance abuse/mental health history and current needs. Provides support, resources, education, and both internal and external referrals for services. Oversees client activities to ensure compliance with program activities and agency policies. Provides guidance as needed. Facilitates psychoeducational client groups and group activities. Provides crisis intervention and able to follow specific procedures in emergency situations. Attends and participates in all meetings and maintains audit-ready patient records and program-specific forms in a timely manner, as required. The Program Manager is responsible for general oversight, administrative supervision of program staff, day-to-day operations, development, implementation, and continuous improvement and evaluation of the Clinical Stabilization Service (CSS) Program. WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY: Provides administrative supervision and management of designated staff including scheduling and approving time sheets, annual reviews, and working with HR regarding employee relations, training and development and disciplinary measures. Develops a strategic plan to implement a comprehensive mental health and substance use treatment program at Independence Hall. Develops the policies and procedures that will support the program and reflect the agency's philosophy and objectives. Works with the Business Department to ensure all program standards are met to maintain licensure by the Massachusetts Department of Public Health, compliance with BSAS Standards of Care, and CARF Behavioral Health accreditation. Collaborate with HR and the Business Department to manage financial performance of the program by monitoring and resolving budgeting issues. Works with Executive management to market and promote the program within the community by developing business relationships, referral networks and outreach linkages. Develops referral process for internal and external services for acute, rehabilitation, support care, medical maintenance, and continuing care for mental health and substance use disorders Identify mental health and substance use assessment tools and how they should be administered. Develops a formalized process for implementing, tracking, and analyzing the screening, brief interventions, and referrals to treatment. Develops and launches a patient engagement initiative, including outreach to de-stigmatize the utilization of counseling services, and works to enhance the "Culture of Care" and understands how to treat clients with dignity and respect, and de-escalate upset clients. Responsible for maintaining minimum staffing 24/7 in order to assure client safety and adherence to state licensing requirements, including all critical staff on all shifts. Maintain a strict policy and practice of non-abandonment of client care, and acts as the final line of coverage in the event of unavailability of replacement staff. WHAT YOU MUST HAVE: Minimum of a Bachelors Degree in related field (Social Work, Counseling, Psychology), Master's Degree preferred. Minimum of two years clinical, behavioral/mental health or substance abuse counseling required. Link Veterans to needed care including mental health, substance use, and primary care as well as services that address social determinants of health. This role is a part of a MassHealth initiative that provides qualified MassHealth enrollees with a service benefit called Behavioral Health Community Partners (BH CP). This initiative is led by the Boston Health Care for the Homeless Program, who has partnered with community-based providers to form the Social Determinants of Health BH CP Consortium to serve eligible MassHealth enrollees in the greater Boston area. Job Responsibilities: Work as part of an interdisciplinary BH CP team and coordinate the care for enrollees. Work closely with nurse care managers to engage enrollees regularly (several times a month) in face to face interactions to assist them in connecting to vital services and progress towards the goals outlined in their Person-Centered Care Plan. Work with the nurse care managers to conduct timely Comprehensive Assessments. Maintain weekly communication with the BH CP team via data sharing platform to improve coordination of information sharing/collaboration between enrollee and BH CP teams. Ensure timely documentation into data platform regarding enrollee progress on Care Plan goals, all enrollee encounters and care coordination notes. Assist nurse care managers and BH CP teams with arranging enrollee appointments for services needed and other related assessments and accompany enrollees to appointments as needed. Triage and troubleshoot care need issues for enrollees and work with nurse care managers to address needs. Participate in team case conferences and advocates for enrollees' needs. Participate in BH CP training and learning collaborative. Remind enrollees about appointments, filling prescriptions, etc. Work with BH CP team on the development and implementation of Patient Centered Care Plan. Request and send medical records for care coordination purposes to providers, Accountable Care Organization and Managed Care Organizations as needed. Collaboration with both community and medical partners to coordinate care services to meet participant need. Attend staff trainings and meetings as required. Qualifications Required Qualifications: BA in Human Services related field. Two years' experience with Veteran population, chronic mental illness, substance use disorders, and/or homelessness. Excellent customer service skills and the ability to communicate professionally with employees and enrollees both on the phone and in person. Proficiency with database systems. Strong crisis intervention skills. Case management experience preferred. Flexible self-starter with strong analytical skills.
