This case manager position is part of the Supportive Services for Veterans and Families (SSVF) Program. The Rapid Rehousing & Homeless Prevention Case Manager is responsible for delivering supportive services to veterans in the areas of homelessness prevention, rapid rehousing, outreach, and employment counseling/placement services for veterans enrolled in Veterans Inc. programs. This position will also provide case management and outreach services to assist veteran clients and their families enrolled in Veterans Inc Programs.
WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
Performs initial intake and assessments of all veterans for acceptance into Veterans Inc. programs.
Develops individualized service plans for homelessness prevention and or rapid rehousing based on veterans input, interests, and needs. Coordinates veterans' participation in the agency programs.
Assists veterans with researching and accessing needed services including VA benefits, employment training/placement , educational support, counseling, housing, medical services and other public benefits such as health care services, daily living services, personal financial planning services, transportation, fiduciary and payee services, legal services, child care, housing counseling services, and temporary financial assistance.
Tracks and reports case management progress through case notes and conferences with Program Manager and coordinates with all funding sources.
Completes record keeping/documentation responsibilities and compiles data for monthly reporting into the agency database.
Conducts outreach activities to market Veterans Inc. programs and engage new referral sources.
WHAT YOU MUST HAVE:
Bachelor's degree in Human Services, Social Work or other related field required.
Minimum of two years' experience in Employment/Training, Vocational Rehabilitation, Counseling, Outreach/and or Services to the Homeless, or related field, is required.
Computer proficiency in Microsoft Office Suite.
Must be able to pass a CORI and background investigation.
Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
Must have a valid driver's license.

What You'll Do The Case Manager for the Washington, DC Temporary Assistance for Needy Families (TANF) Workforce Development program will serve as the main point of contact for Customers (TANF recipients referred to Fedcap by the District of Columbia's Department of Human Services). The Case Manager will play a key role in coordinating all services to ensure that Customers are making meaningful progress toward preparing for and obtaining employment. Case Managers shall develop trusted working relationships with assigned TANF Customers and shall monitor Customer engagement and progress up through the first year of a Customer's employment. Case Managers shall coordinate multiple activities with Fedcap staff and other providers, as well as assist Customers in reporting work participation.
Your responsibilities will include:
Case Coordination and Relationships
Carry a case load of approximately 50 unemployed and unenrolled program participants
Conducted at minimum 25 participant interactions a week including bi-weekly coaching sessions and/or weekly non-compliance/re-engagement attempt meetings
Coordinate and develop relationships with all external Providers (including Secondary Service Providers, District Agency Providers, School, Employers, and Community Providers) as well as Teammates and ensure that participants are making meaningful progress toward employment.
Review DHS-administered assessments, develop and amend participants' Detailed Individual Responsibility Plans (DIRPs), and help participants overcome barriers to program engagement and participation.
Develop trusted working relationships with assigned TANF participants and monitor participant engagement and progress up through participants first year of employment.
Provide counseling, intervention, and program action planning and delivery.
Facilitate the successful utilization of job readiness training, rapid job searching, job retention services, and/or education services.
Document activities, participation, compliance, and employment progress in the Electronic Case Management (ECM) systems.
Stress urgency and ownership, guiding participants through a learn-by-doing process during their service planning and delivery.

Achieve 80% Work Participation Rate (WPR) on case load by collecting, verifying and processing timesheets and/or paystubs for entire case load weekly
Complete non-compliance attempts weekly for the 20% of the case load not meeting WPR including phone calls, emails, letters and home visits and re-engaging those participants who are responsive or confirming sanction status to DHS for those that have not.
Coordinate multiple activities outlined in the participant's DIRP and assist participants in reporting work participation.
Ultimately responsible for the engagement of Participants and works with all staff to identify appropriate activities, resources, and supports for participants.
Performance Management and Reporting
Consistently meet or exceed performance targets and KPIs, including job placements, employment retention, school completions, case closure due to earnings, customer satisfaction and audit scores.
Ensure 100% accuracy in internal data systems, including timely recording of employer activity, jobseeker interactions, and placement outcomes
Work closely with internal teams to maintain service consistency and accountability, improving overall efficiency in services
Responsible for capturing and recording employment information and then working closely with the Customer to maintain and obtain job promotions and/or salary increases on the job.
Diligently verify and report all aspects of Customer milestone achievements to DHS.
Assist in compiling data for reports, including but not limited to, Customer timesheets and placement and retention reports.

Observe all policies and procedures and attend all required training and certifications as scheduled.

The Case Manager is also responsible for completing documentation accurately and in a timely manner. This position will be based on-site at Chestnut Crossing but will spend a significant amount of time in the community working directly with participants and will regularly travel to MHA's Main Office in Chicopee.

Responsibilities:

GENERAL SUMMARY

ESSENTIAL JOB FUNCTIONS*

The Case Manager provides outreach and support services to clients with psychiatric, or substance use disorder diagnoses to enable them to utilize clinical treatment services and other supports. The Case Manager assists clients to attain his/her clinical treatment plan goals in outpatient and other levels of care and works to mitigate barriers to success.
Responsibilities include:
Provide service coordination and linkages to the community.
Provides outreach services to clients, delivering mobile services in a number of community or institutional settings including inpatient units, homes, day programs, etc.
Assist clients in obtaining benefits, housing and health care as needed.
Provide crisis support and participate in the development, revision and utilization of crisis prevention and/or safety plans.
Encourages and facilitates utilization of clients' natural support systems as well as recovery-oriented, peer support and self-help support and services.
Will assist in data collection and tracking for grant evaluation.
Requirements:
Bachelor's degree from an accredited educational institution in the Human Service field • Ability to pass certification for CPR, First Aid
Valid driver's license; reliable transportation for business use
Ability to counsel and support clients with psychiatric and/or substance use disorder diagnoses
Demonstrated knowledge of trauma-informed care
Lived experience relative to psychiatric and/or substance use disorder preferred.

Position Summary
The Case Manager provides case management to homeless individuals being referred for housing placement in a variety of settings. Key Responsibilities
Engaging in activities to provide outreach, engagement, and assessment to potential program applicants. Gather necessary supporting documentation, complete intake assessments and enter necessary data into program database.
To support the participant across a range of services, the case manager will develop and maintain collaborations and partnerships with entities including but not limited to homeless networks, Continuum of Care, and state and local agencies which serve homeless individuals including drop-in centers.
Actions taken to support the implementation of the Individualized Action Plan and to monitor the progress made towards Individualized Action Plan goals.
Linking participants to a range of state and community-based services including, but not limited to, mental health; substance use; primary health care; income benefits; housing; legal assistance; education; peer support and job training.
Develop crisis and pre-crisis plans with the program supervisor and clinical director to provide intervention and stabilization in the event of a psychiatric crisis.
As the participant nears meeting the established goals, the case manager will work with the person and his/her treatment team to develop a discharge plan and identify a projected disenrollment date.

Position Summary
The Case Manager provides trauma-informed, person-centered support to individuals in substance use and/or mental health recovery who have a history of homelessness. This role involves assisting participants as they transition from homelessness into their own studio apartments within a congregate supportive housing site. Once housed, the Case Manager continues to provide ongoing support to help participants achieve their goals-such as stabilizing income, strengthening coping skills, accessing medical care, and pursuing employment. The Case Manager is also responsible for completing documentation accurately and in a timely manner. This position will be based on-site at Chestnut Crossing but will spend a significant amount of time in the community working directly with participants and will regularly travel to MHA's Main Office in Chicopee.

Key Responsibilities
Coordinate all documentation required for program entry, including but not limited to: homelessness verification, substance use assessments, eligibility screenings, and obtaining IDs, birth certificates, social security cards, income verifications, etc.
Complete initial Adult Comprehensive Assessments, updates, and service plans.
Collaborate as part of a multidisciplinary team to develop Individualized Action Plans (IAPs), utilizing participant voice, choice, strengths, and needs.
Implement strategies outlined in the IAP and document progress as required.
Develop, monitor, and revise safety plans in collaboration with participants and the team.
Adjust support based on individual needs.
Assist participants with the transition to stable housing.
Maintain strong communication with staff at Chestnut Crossing.
Support tenants in navigating responsibilities such as apartment maintenance, communication with property management, and understanding housing regulations.
Support participants in seeking and maintaining employment, vocational training, or education.
Educate and assist participants in accessing public benefits and resources (e.g., Social Security, EAEDC, SNAP, PVTA, Fuel Assistance).
Encourage community integration by helping participants access public spaces, support groups, recreational activities, and vocational programs.
Promote skill development in areas such as ADLs, money management, transportation, and social participation.

This case manager position is part of the Supportive Services for Veterans and Families (SSVF) Program. The Rapid Rehousing & Homeless Prevention Case Manager is responsible for delivering supportive services to veterans in the areas of homelessness prevention, rapid rehousing, outreach, and employment counseling/placement services for veterans enrolled in Veterans Inc. programs. This position will also provide case management and outreach services to assist veteran clients and their families enrolled in Veterans Inc Programs.
WHAT YOU'LL BE RESPONSIBLE FOR DAY TO DAY:
Performs initial intake and assessments of all veterans for acceptance into Veterans Inc. programs.
Develops individualized service plans for homelessness prevention and or rapid rehousing based on veterans input, interests, and needs. Coordinates veterans' participation in the agency programs.
Assists veterans with researching and accessing needed services including VA benefits, employment training/placement , educational support, counseling, housing, medical services and other public benefits such as health care services, daily living services, personal financial planning services, transportation, fiduciary and payee services, legal services, child care, housing counseling services, and temporary financial assistance.
Tracks and reports case management progress through case notes and conferences with Program Manager and coordinates with all funding sources.
Completes record keeping/documentation responsibilities and compiles data for monthly reporting into the agency database.
Conducts outreach activities to market Veterans Inc. programs and engage new referral sources.
WHAT YOU MUST HAVE:
Bachelor's degree in Human Services, Social Work or other related field required.
Minimum of two years' experience in Employment/Training, Vocational Rehabilitation, Counseling, Outreach/and or Services to the Homeless, or related field, is required.
Computer proficiency in Microsoft Office Suite.
Must be able to pass a CORI and background investigation.
Compliance with Veterans Inc. COVID-19 Vaccination Policy. Religious and Medical Exemptions are considered.
Must have a valid driver's license.

OVERVIEW
The SIPS Case Manager is responsible for providing high quality, intensive case management services to a diverse group of participants. The SIPS model places chronically homeless individuals directly into housing without requiring them to meet specific clinical or behavioral outcomes first. Case management services include regular home-visits, coordination with a network of community-based providers, and assistance with applications for housing and public benefits.
This is a hybrid position where you will work a minimum of two (2) days per week in the office at 1411 Tremont Street Boston, MA. Hybrid work schedule may change, and you will be notified by your supervisor.
RESPONSIBILITIES MAY INCLUDE:
The statements below are intended to describe the general nature and scope of work being performed by this position. This is not a complete listing of all responsibilities, duties and/or skills required. Other duties may be assigned.
•
As needed, provide information and referral services, and brief counseling around immediate client concerns and/or barriers to housing, including but not limited to: financial instability, legal issues, mental health issues, substance abuse, un- or under-employment, health care, and housekeeping;
•As needed, obtain resources and services as requested and/or needed to address clients' needs, including but not limited to: housing applications, negotiation and mediation with property owners and utility companies, fuel assistance, and shelter information;
•As needed, administer homelessness prevention funds, including Residential Assistance for Families in Transition (RAFT), by conducting intakes, screening participants for program eligibility, and meeting with heads of household directly to complete paperwork and determine an appropriate course of action: either administering funding or providing counseling and referral services, or both;
•As needed, assist clients with short term on-going housing search including but not limited to, contacting prospective landlords and/or management companies, accompanying clients to view and apply for units, attending appointments at housing agencies and other search activities as they present;
•As needed, assist participants with move-in transition supports including but not limited to; furniture and furnishings assistance, budget support, and other resources and services as needed;
QUALIFICATIONS:
High School Diploma or GED required
At least 1 -2 years' experience in case management or intake and assessment with homeless households or households at risk of homelessness;
Familiarity with affordable housing and housing subsidies as well as populations with substance use and/or mental health challenges
Experience with crisis intervention regarding issues such as homelessness and affordable housing access;
Knowledge and experience with service agencies and resources for low-income and/or homeless individuals and families;
Willingness to work as part of a team to promote the goals of the program and organization;
Sensitivity to the needs of the homeless, low-income, and diverse populations;
Ability to work in a busy, diverse team setting;
Excellent time management, organizational, communication, and writing skills
PREFERENCE GIVEN TO
Candidates with multilingual skills (verbal and written). We are seeking candidates that speak and write English and at least one of the following languages (for interpreting and translation): Vietnamese, Cape Verdean Creole, Portuguese, Cantonese, Mandarin, Toisanese, Haitian- creole, and Spanish.
