Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid drivers license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.
Strong interpersonal communication skills with the ability to engage patients and team members effectively.
Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment.
Multi-lingual skills are a plus but not required.
Prior home care experience is beneficial.

As part of the OdySea Aquarium animal team, this position has the potential to work with diverse animals including (but not limited to) California sea lions, Asian small-clawed otters, African penguins, reptiles, and/or small exotic land animals. MAJOR DUTIES AND RESPONSIBILITIES:

REQUIRED EXPERIENCE:
Bachelor's degree in Psychology, Biology, Zoology, Marine Science or related discipline preferred, equivalent alternate experience accepted.
Minimum 1 year paid experience working with and training marine and/or terrestrial animals at an AZA accredited or equivalent facility.
SCUBA certification.
Excellent public speaking and showmanship experience.
PHYSICAL AND MEDICAL REQUIREMENTS:
Must be able to lift 50 pounds.

Job Title & Role Description:
The Care Specialist is a key member of the team responsible for delivering chronic care management to high-complexity patients. The Care Specialist primarily works in patients' homes and communities (90% of the time) and engages in virtual care (10% of the time). This role involves direct outreach to patients, guiding them through the enrollment process, facilitating virtual appointments with providers, and ensuring that patients adhere to care plans. By building relationships, coordinating care, and providing education, the Care Specialist plays a critical role in promoting health, preventing unnecessary hospitalizations, and improving patient outcomes.
Skills Required:
Minimum of 2 years of relevant work experience (e.g., Community Health Worker, Peer Support Specialist, Medical Assistant, etc.).
High school diploma or GED required.
A valid driver's license, auto liability insurance, and reliable transportation to travel within the assigned territory.
Experience in chronic care management or working with chronically ill/elderly patients.
Technologically proficient with basic computer skills (typing, using EMR systems).
Experience with motivational interviewing, trauma-informed care, and care coordination.
Strong interpersonal communication skills with the ability to engage patients and team members effectively.
Ability to prioritize tasks, manage schedules, and work independently in an unstructured environment.
Multi-lingual skills are a plus but not required.
Prior home care experience is beneficial.

A day in the life:

Aid Veterinary Technicians (VT) with booked appointments, walk-ins, and work-ins, including recording observations/treatment plans in the patient file, and aid in the performance of medical treatments under doctor supervision.

Description: The Veterinary Technician Assistant provides support and assistance to Internal Medicine, Cardiology, and Radiology with a focus on patient care, team safety, client satisfaction, and practice efficiency. Working under the supervision of the Director of Operations, Director of Veterinary Technicians, and the Veterinarians, a Veterinary Technician Assistant must be able to effectively and proactively communicate and perform the following duties that include but are not limited to:
Provide nursing care for patients.
Ensure proper use and safekeeping of medications and supplies.
Set-up, breakdown, clean, and ensure proper maintenance of medical equipment.
Maintain accuracy of electronic medical records and other paperwork.
Represent the practice professionally when interacting with pet owners, referring hospitals and departments, vendors, and co-workers.
Be knowledgeable and proficient with proper patient handling, positioning and restraint techniques.
Collect samples and perform diagnostic testing (i.e. blood work, urinalysis, radiographs, etc.)
Administer medications and other therapies through various techniques (i.e. oral, subcutaneous, intramuscular, and intravenous).
Be prepared to assist in an emergency situation (i.e. triage, CPR, and anesthetic complications).
Accurately fulfill prescription requests by filling medications in hospital, electronic submissions, and verbally communicating with outside pharmacies.
Carefully handle medications for proper inventory control.
Maintain cleanliness throughout the hospital.
Aid veterinarians and technicians in achieving greater efficiency by relieving them of technical work and administrative detail.
Provide written and verbal communication to pet owners including answering questions, obtaining patient history, and relaying diagnostic results and treatment plans.
Provide coverage for the Front Desk by performing various CSR duties as needed.
Operate within OSHA standards with a focus on safety for patients, pet owners, and co-workers.
