Essential Job Functions:
* Engage clients who are admitted to the Central Receiving System to discuss the benefit of care coordination services with the intent to promote said services and obtain the client's consent for care coordination services post-discharge from acute care.
* Review Safety Plan/Wellness Toolkit upon discharge with client and their support system.
* Coordinate client treatment services.
* Arrange for appointments within 7 days of the discharge from acute care with verification within 1 business day post appointment.
* Provide follow-up phone call within 24 hours and 72 hours upon discharge.
* Provide care transfer check-ins? over a 90-day period to make additional referrals/linkages to both mental health and substance abuse services.
* Track all contact with client for the duration of the client's participation in the care coordination program.
* Assess client's risk of suicide during ?care transfer check-ins? and ensure client is linked with acute stabilization if necessary.
* Performs other related tasks as assigned.

Job duties include, but are not limited to:
Pet restraint, triage incoming patients, collection and analysis of lab samples, diagnostic imaging, management and nursing care of medical and hospitalized cases with varying degrees of stability, administration of medications, effective communication of treatment plans with clients, surgical preparation and assistance, and anesthetic management.

Job Description:
The Small Animal Hospital is recruiting a veterinary technician for the Surgical Oncology service. We are seeking a technician who is passionate about the opportunities the oncology specialty can provide in prolonging the lives of their patients, while extending compassion and empathy to their patients and loved ones. Technicians provide support by assisting veterinarians with exams and treatments, as well as providing insight, instruction, and demonstration to veterinary students. In particular, this position will assist the Surgical Oncology service. Access to the wide range of treatments and services offered through the Oncology Service makes this a great opportunity for those considering obtaining their specialty certification.
Example of Veterinary Technician Duties and Responsibilities include but not limited to:
• Assist with daily intake and discharge of Surgical Oncology cases
• Assist with patient evaluations or rechecks by assisting in patient restraint, obtaining and submitting samples, scheduling with other services for diagnostic or consultation purposes, and providing treatment or medications
• Perform general nursing care such as venipuncture, catheter placement, patient restraint, treatments, medication administration, and sample collection
• Document in electronic medical records
• Perform anesthesia and sedation procedures including patient monitoring and recovery
• Assist with creating an efficient transfer of surgery cases to anesthesia
• Assist surgeons and students with instrumentation and sterile technique
• Assist with minor surgical procedures, bandage changes, and suture removals
• Facilitate the admission of surgical oncology cases to the Progressive Care Ward or ICU
The service receives Monday - Friday. May require working earlier than 7:30am or later than 5:00pm
This is a full-time, benefited position and is considered essential personnel. A post-offer education verification, background check, and health assessment are required.
Expected Salary:
Non-credentialed $18-22, Credentialed $20-25
Preferred Qualifications
• Graduate from an accredited American Veterinary Medical Association program for veterinary technology OR
• Credentialed veterinary technician
