The Detroit Medical Center (DMC) Registered Nurse (RN) is a member of the Patient Care Services Team, and is responsible for the satisfactory completion of nursing care by the nursing team. The RN manages and provides patient care activities for a group of patients and their families through application of independent judgment, communication and collaboration with all team members including ancillary and support services. The role of the Registered Nurse at the DMC encompasses leadership, partnership, collaboration, teaching and supervision.
The DMC RN:

The Communication Specialist will staff the Communication Center in the emergency department at Detroit Receiving Hospital The Communication Center is located in the emergency department at Detroit Receiving Hospital. The Communication Center will function to centralize and streamline communication between DMC hospital sites and the surrounding area. The Communication Center will house all outside communication equipment utilized to receive notification of critical patient transports, disasters, or mass casualty events. The Communication Center will accommodate the Personal Computer-Based Emergency Management (EM) System utilized during a major incident within Michigan for tracking patient movement from the scene to the hospital. The goal of the Communication Center is to expedite incoming patient transfers from external facilities and communicate to the ED staff patients condition prior to arrival.

1. Expedites and coordinates all transfers from external facilities.
2. Coordinates and activates STEMI process, stroke team notification, major communicable disease response team, chempack team, decontamination team, all communications from outside agencies regarding acts of bio-terrorism/decontamination/code yellow; activates trauma pager.

Job Summary

The Graduate Nurse (GN) is a member of the patient care delivery team. Summary Description:
Coordinates the oversight of surgical instruments and supplies between the OR and Central Sterile Processing (CSP) to ensure that requested items are available and to help facilitate efficient case flow. Responsible for assisting in special projects and process improvement initiatives when needed. Promotes customer service and satisfaction through training, education, communication and resolution. Perform duties in accordance with hospital, TJC, CMMS, FDA, AORN, AAMI and other appropriate hospital and department specific policies and procedures.
General Duties:
1. Assists the Operating room staff in preparing the OR suite for surgery by verifying instrument trays are available prior to case start times.
2. Assists in the transportation of sterile instruments and supplies to the operating room using appropriate standards and techniques. Perform package integrity checks on all items to make sure that the package and/or wrapper is not compromised.
3. Keeps track of case flow during the day, monitor instrument needs and notify CSP personnel in advance of any possible addition / emergent instrument needs. Must be able to resolve issues or offer alternatives when necessary.
4. Assist in removing soiled case carts or un-sterile instrumentation and scopes from the operating room to be returned to CSP for processing.
5. Assist with planning and coordination of instrument turnarounds with the CSP staff. Help communicate to the surgical team essential instrumentation information that may affect or delay surgery start time.
6. Verifies the instrument needs for the next day and that they have been cleaned, wrapped, sterilized and available for use.
7. Demonstrate a working knowledge and understanding of surgical instruments and be able to identify when instruments and broken, damaged or not functioning as designed. Report these issues to the CSP management team.
8. Communicate to the OR coordinators suggestions related to surgeon preference card changes based upon set usage to help ensure availability.
9. Working with the OR staff identifies instrumentation related to tray additions and reconfigurations, building surgeon specialty sets, etc. Report recommendations to the CSP Manager.
10. Responds to problem calls from Surgery throughout the day as required.
11. Perform required daily audits of completed and post-op case carts. Provide completed audit sheets to the CSP data entry clerk.
12. Participates in education of health care providers related to receipt, inspection, inventory, storage & distribution of instrumentation and medical supplies in Processing, function of specialty instrumentation, problem resolution of frequently encountered instrument function issues and instrument maintenance program.

Residency Program Coordinator Job Description
Under limited supervision, provides a high level of diversified administrative support in assisting the Graduate Medical Education (GME) Office in the management and direction of the residency training program. Coordinates the educational activities of the program for incoming and current residents. Assures program compliance with regulatory agency standards and maintains essential documents required for program viability. Functions as liaison between the GME office, residents, departments, attending physicians, administration, outside institutions and Program Director(s).
PRINCIPAL DUTIES AND RESPONSIBILITES:
1. Assists leadership with day-to-day personnel and administrative support services of the department. Assists in scheduling, communications and preparing reports, conduct documentation reviews for errors and discrepancies, and media for meetings and conferences.
2. Resolves problems and refers problems of an unusual nature to direct supervisor and program leadership.
3. Assists with program communication and compliance with accrediting body requirements and remain informed on updates from the accrediting body. Assists with the program's annual update, as required by the program's accrediting body.
4. Maintains and oversees time off and attendance records for residents and other healthcare personnel as needed.
5. Prepares, copies and distributes house staff rotation schedules, call schedules and lecture schedules. Coordinates rotation, vacation and call schedules.

12. Monitors program personnel and make adjustments to resident and faculty rosters in various databases.
13. Supervises collection of data fulfilling American Medical Association, American Osteopathic Association and other licensing and credentialing boards; prepares reports and compiles statistics.
14. Verifies training and processes letters of recommendation for current residents as well as past residents; prepares correspondence relative to program.
15. Plan and organize resident graduation which involves securing facility, identifying menu, generating invitations and developing graduation program.
16. Coordinates monthly didactic schedule, which includes identifying speakers, assisting with presentations and frequently communicating with attending physicians. Collects lecture evaluations and prepares summary reports, maintains lecture attendance records and documentation, corresponds with program faculty.
17. Prepare, distribute, report, and manage evaluations for the program, including residents and faculty.
18. Review budget analysis for accuracy. Track program expenditures aligned with the program's budget.
19. Coordinates the annual program evaluation, preparation and signing of contracts for new and existing residents and program directors via submission of contracts for CME credit approval for physician.
20. Tracks and maintains documentation of compliance requests for residents such as, but not limited to, BLS, HIPAA, OSHA, immunization records and personal health and malpractice insurance to assure confidentially and compliance.
21. Develops and maintains resident tracking software and maintains current data such as evaluations, examinations, schedules, procedure logs and conference attendance, scholarly activity and so forth.
22. Assists the Program Director in implementation of requirements for resident training programs in compliance with the program's accrediting body and arranges site visits by various institutions. Maintains a working knowledge of student and resident requirements and department/site resources.
23. May develop medical student rotation schedules, coordinates visiting medical student rotations and collects student evaluations. Corresponds with Michigan State University College of Osteopathic Medicine and Wayne State University School of Medicine as needed.

Summary Description:
Under general direction and according to established policies and procedures, develops and supervises the implementation of interdisciplinary treatment plans for patients in the inpatient setting at Rehabilitation Institute of Michigan. Performs continuous review of services to assure patient progress and maintains communication with families and professionals.
Coordinates interdisciplinary team meetings to discuss each patients rehabilitation program with team, patient/family, insurance case managers / representative and other appropriate individuals involved in patient treatment.
Recommends services for patients based on individual patient assessments, case documents and patient/family or insurance carrier preferences.

Job Description:
The RN Case Manager is responsible to facilitate care along a continuum through effective resource coordination to help patients achieve optimal health, access to care and appropriate utilization of resources, balanced with the patients resources and right to self-determination. The individual in this position has overall responsibility for ensuring that care is provided at the appropriate level of care based on medical necessity and to assess the patient for transition needs to promote timely throughput, safe discharge and prevent avoidable readmissions. This position integrates national standards for case management scope of services including: Utilization Management supporting medical necessity and denial prevention. Transition Management promoting appropriate length of stay, readmission prevention and patient satisfaction. Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care. Utilization Management: Balances clinical and financial requirements and resources in advocating for patient needs with judicious resource management. Assures the patient is in the appropriate status and level of care based on Medical Necessity process and submits case for Secondary Physician review per Tenet policy. Ensures timely communication of clinical data to payers to support admission, level of care, length of stay and authorization for post-acute services . Advocates for the patient and hospital with payers to secure appropriate payment for services rendered. Promotes prudent utilization of all resources (fiscal, human, environmental, equipment and services) by evaluating resources available to the patient and balancing cost and quality to assure optimal clinical and financial outcomes. Identifies and documents Avoidable Days using the data to address opportunities for improvement. Prevents denials and disputes by communicating with payers and documenting relevant information. Coordinates clinical care (medical necessity, appropriateness of care and resource utilization for admission, continued stay, discharge and post- acute care) compared to evidence-based practice, internal and external requirements. (30% daily, essential).
Transition Management: Completes comprehensive assessment within 24 hours of patient admission to identify and document the anticipated transition plan for patients. Integrates key elements of patient assessment, patient choice and available resources to develop and implement a successful transition plan. Identifies patients at risk for readmission and applies appropriate intervention including risk assessment and referral to Social Work services and/or Complex Case Review. May delegate the implementation of the transition plan to LVN/LPN or Assistant staff. And follows up to ensure the transition plan is completed timely and accurately. Ensures all elements of the transition plan are implemented and communicated to the healthcare team, patient/family and post-acute providers. Provides information to patients to make informed choices when community services per Tenet policy. Completes Final Discharge Disposition Form Assessment for Medicare patients per Tenet policy. Identifies and reports variances in appropriateness of medical care provided, over/under utilization of resources compared to evidence-based practice and external requirements. This priority includes documentation in the Tenet Case Management system to communicating information through clear, complete and concise documentation (30% daily, essential).
Care Coordination: Screens patients for factors that may affect the progression of care and intervenes as needed to promote timely and appropriate throughput. Conducts assessments and stratifies patients at risk for readmission or in need of Case Management services. Ensures the plan of care is clinically appropriate, consistent with patient choice and available resources. Ensures consults, testing and procedures are sequenced to support the patients clinical needs with timely and efficient care delivery. Ensures patient needs are communicated and that the healthcare team is mutually accountable to achieve the patient plan of care. Effectively collaborates with physicians, nurses, ancillary staff, payors, patients and families to achieve optimum clinical outcomes (15% daily, essential).
Education: Ensures and provides education to patients, physicians and the healthcare team relevant to the- Effective progression of care, Appropriate level of care, and Safe and timely patient transition. Provides patient and healthcare team education regarding resources and benefits available to the patient along with the economic impact of care options. Ensures that education has been provided to the patient/family/caregiver by the healthcare team prior to discharge (15% daily, essential).
Compliance: Ensures compliance with federal, state, and local regulations and accreditation requirements impacting case management scope of services. Adheres to department structure and staffing, policies and procedures to comply with the CMS Conditions of Participation and Tenet policies. Operates within the LVN/LPN scope of practice as defined by state licensing regulations. Remains current with Tenet Case Management practices (10% daily, essential).
Qualifications:
1. Graduate from an accredited school of nursing. Bachelors degree in Nursing or other health-related field, or equivalent combination of education and/or related experience.
2. Two years of acute hospital patient care experience. Acute hospital case management experience preferred.
3. License to practice as a Registered Nurse in the State of Michigan.
4. Accredited Case Manager (ACM) preferred.
5. Must complete Tenets InterQual education course within 30 days of hire (and at least annually thereafter) and pass with a score of 85 or better. Education provided to physicians, patients, families and caregivers
This individual's responsibility will include the following activities: a) complex psycho-social transition planning assessment and reassessment and intervention, b) assistance with adoptions, abuse and neglect cases, including assessment, intervention and referral as appropriate to local, state and /or federal agencies, c) care coordination, d) implementation or oversight of implementation of the transition plan, e) leading and/or facilitating multi-disciplinary patient care conferences including Complex Case Review, f) making appropriate referrals to other departments, g ) communicating with patients and families about the plan of care, h) collaborating with physicians, office staff and ancillary departments, i) assuring patient education is completed to support post-acute needs , j) timely complete and concise documentation in Case Management system, k ) maintenance of accurate patient demographic and insurance information, l) and other duties as assigned.
POSITION SPECIFIC RESPONSIBILITIES:
Transition Management
· Completes comprehensive assessment within 24 hours of patient admission to identify and document the anticipated transition plan for patients
· Integrates key elements of patient assessment, patient choice and available resources to develop and implement a successful transition plan
· Completes Complex/Psycho-social assessment and plan for patients identified as high risk for readmission.
· Provides psycho-social assessment and intervention for patients identified with identified needs including behavioral health, lack of support systems, financial barriers, end of life, and/or medication adherence.
· May delegate the implementation of the transition plan to LVN/LPN or Assistant staff. And follows up to ensure the transition plan is completed timely and accurately
· Ensures all elements of the transition plan are implemented and communicated to the healthcare team, patient/family and post-acute providers
· Provides information to patients to make informed choices when community services per Tenet policy
· Completes Final Discharge Disposition Form Assessment for Medicare patients per Tenet policy
· Completes timely, complete and accurate documentation in the Tenet Case Management system to communicating information to the care team and provide documents needed in the patient record(40% daily, essential)
Care Coordination
· Screens patients for factors that may affect the progression of care and intervenes as needed to promote timely and appropriate throughput
· Conducts assessments and stratifies patients at risk for readmission or in need of Case Management services
· Assists with adoption/abuse/neglect cases and reporting of appropriate cases to local, state and/or federal agencies
· Ensures the plan of care is consistent with patient choice and available resources
· Ensures patient needs are communicated and that the healthcare team is mutually accountable to achieve the patient plan of care
·Effectively collaborates with physicians, nurses, ancillary staff, payors, patients and families to achieve optimal outcomes(40% daily, essential).
Education
· Ensures and provides education to patients, physicians and the healthcare team relevant to the safe and timely patient transition
· Provides patient and healthcare team education regarding resources and benefits available to the patient along with the economic impact of care options
· Ensures that education has been provided to the patient/family/caregiver by the healthcare team prior to discharge
(10% daily, essential).
Compliance
· Ensures compliance with federal, state, and local regulations and accreditation requirements impacting case management scope of services
· Adheres to department structure and staffing, policies and procedures to comply with the CMS Conditions of Participation and Tenet policies
· Operates within the Social Work scope of practice as defined by state licensing regulations(10% daily, essential)
Qualifications:
Minimum Qualifications:
1. Graduation from an accredited school of nursing. Bachelor's degree in Nursing (BSN) preferred.
2. Licensed to practice as a Registered Nurse (RN) in the state of Michigan.
3. Three years of progressively more responsible clinical and/or quality improvement/process improvement experience, including at least one to two years of experience in health care quality management and the application of continuous improvement methodologies, tools, standards, and guidelines.
