Responsibilities
Responsible for directing quality management efforts to ensure continuous improvement and organizational excellence, adherence with all regulatory, compliance, accreditation, and legal issues, and promote a safe environment for all employees, patients and visitors. Manage risk issues and oversees the credentialing function for medical staff. Accountable for the development, management and coordination of the hospital-wide Performance Improvement Program designed to improve clinical outcomes and the patient experience. Responsible for maintaining compliance with CMS Conditions of Participation, the Joint Commission standards, COLA/CLIA standards and HIPAA Rules and Regulations. Functions as the Risk Manager for organization as well as the Facility Compliance Officer.
Required Skills:
* An active clinical license in the state of employment is required in one of the following fields: Nursing, Physical Therapy, Occupational Therapy, Speech Language Pathology or Respiratory Therapy.
* Minimum three (3) years of work experience or equivalent, in quality/risk management activities required.
* Knowledgeable in the principles of performance improvement.
Additional Qualifications/Skills:
* Bachelor's or Master's degree preferred. Management experience preferred.
* Certified Professional Healthcare Quality (CPHQ) certification preferred.

Job Description
The Executive Director is responsible for the overall leadership, direction, growth and culture of the home health and hospice agency. Responsibilities
Responsible for educating, problem solving and building relationships with referral sources, obtaining referrals, completing eRehab or LTRAX assessments, and participating in planning, organizing and executing the Business Departmental plan. Frequent day travel with reliable transportation is required. The individual will be responsible for establishing and maintaining effective working relationships with their assigned accounts.
Functions as a key source of market intelligence providing direct communication between the community, referral sources and physicians to their Vibra Hospital Team. Often serves as the "face of Vibra" in the community and is responsible for developing a positive, professional image conducive to building long lasting relationships with our customers.
Required Skills:
* Current, valid, and active license to practice as a Registered Nurse, Licensed Practical/Vocational Nurse, Licensed Clinical Social Worker, Occupational Therapist, Speech Language Pathologist, Physical Therapist or Respiratory Therapist required in the state where support is provided.
* Recent healthcare experience in the acute care hospital, rehabilitation setting or home health care agency/company required.
* Current, valid and active driver's license required.

Additional Qualifications/Skills:
* Previous clinical screening experience is preferred.
