    The Senior Case Manager is responsible for the coordination and delivery of quality direct case management services for Chamblet and Revision Family Home. The Senior Case Manager will provide consultation to interdisciplinary team members on issues related to supporting residents' housing, development and facilitation of treatment plans with residents, and assisting the Program Director with weekly case review. The Senior Case Manager reports to the Program Director for weekly supervision and programmatic assignments. Location: Chamblet Family Home in Dorchester provides life-stabilization services, intensive housing placement services and vocational case management services to homeless families and to pregnant women. Our program helps families increase self-sufficiency, gain healthy nutritional knowledge, search for more permanent housing and build job-readiness skills. Essential Functions: Act in accordance with Victory Programs, Inc.'s Standards of Excellence & Philosophy of Care at all times including maintaining professional ethics, boundaries and client confidentiality. Provide weekly collaborative case management services to 14 families to work collaboratively to assist the residents in addressing any barriers they are facing surrounding housing, finances, vocation and/or education, physical and mental health, childcare, and legal issues. Assess and provide referrals to external providers as needed. Develop and maintain service linkages and databases to support residents including developing relationships with local service providers and housing providers. Accompany clients to housing, legal, and medical appointments as needed. Develop and monitor service plans as required by EOHLC as well as complete timely reporting of housing assessments, case notes, and relevant data into the HMIS/ETO system and the residents' case files. Develop, implement, and facilitate workshops, community meetings, and resident events in conjunction with the Shelter Manager and Program Director as required by the funder and agency. Work closely with all Case Managers to provide resources and support with developing and implementing service plans for residents and stabilization families. Facilitate weekly case review and complete required documentation. Complete weekly HMIS/ETO audits and implement plans for timely completion of assessments in conjunction with the Program Director. Assist with program coverage as needed by maintaining building security, adhering to all program policies and protocols of the residences, and completing all required documentation in the program shift logs and staff communication logs. Non-Essential Functions: Attend supervision, staff meetings, clinical meetings, in-service training, and agency workshops. Participate and support the agency in program events, State House rallies, fundraising and advocacy events, rallies, walks and any additional functions. Transport clients using agency vehicles. Perform any other duty or task that may be requested from time to time by your supervisor or their designee. Qualifications: Master's Degree in Social Work, LADCI, or in another human services field preferred. Minimum of three years' experience in housing and client services is required. Knowledge of family shelter services or residential services and understanding of harm reduction philosophy required. Experience advocating for clients with government assistance agencies, housing authorities, private landlords or realtors preferred. Openness to harm reduction philosophy. A valid Massachusetts driver's license with a safety driving points of 00, 99, 01, 02, or 03. Ability to work independently and cooperatively as a member of a professional team. Strong organizational, written, and verbal communication skills. Strong computer skills, including Google office suites and Internet use. Demonstrated ability to work successfully in multicultural settings with diverse client groups, including people with mental health concerns and substance use disorder.     The Bilingual Housing Search Case Manager will provide high intensity housing search to help clients overcome barriers and access affordable, safe homes, to better their health outcomes. These supports are designed to increase housing retention. The Bilingual Housing Search Case Manager to the Program Director of Housing Search and Stabilization Services for weekly supervision and programmatic assignments. Victory Housing Division provides life-stabilization services, intensive housing placement services and case management services to homeless families and individuals. Our program helps families and individuals obtain permanent, safe housing solutions, increase self-sufficiency, and gain skills which support housing stability. Location: 29 Stanhope Street, Boston MA Essential Functions: Act in accordance with Victory Programs, Inc.'s Standards of Excellence always, including maintaining professional ethics, boundaries and client confidentiality. Ensure that policies and procedures of the programs are followed. Work with a caseload of 30-35 clients. In an intensive 12-18-month process, the search specialist will work to determine appropriate housing possibilities and create an individualized housing service plan including short- and long-term goals associated with stable, affordable housing. Work with clients to set priorities and take steps toward reaching these goals. Work with clients to identify possible barriers to obtaining housing. Discuss criminal, credit and eviction history. Request CORI and credit reports with all clients willing to do so. Proactively plan strategies to appeal housing denials and present mitigating factors. Attend all housing related appointments with clients, including interviews, appeal hearings, apartment viewings and lease signings. Work with clients on accessing, obtaining, applying for resources related to housing search and support for maintaining housing such as income support, health insurance support, identification documents etc. Maintain communications with housing authorities, landlords and private management companies responsible for administering affordable housing options. Contact these providers on a regular basis to remain informed of available housing, open waiting lists, eligibility requirements and application processes. Work closely with clients on all phases of the subsidized housing process, including completing and following-up on housing applications, researching available apartments, accompanying clients to all related housing appointments such as interviews, apartment viewings, and lease signing. Formulate and document individualized housing service plans for clients. Assist clients in creating goals that are achievable, concrete, and measurable and time limited. Maintain contact with other internal and external client providers, including, but not limited to counselors, case managers, and housing providers. Maintain timely and accurate records of all client work, completing all program required assessments. Maintain clear, concise and accurate client files and data-based records. Attend weekly team meetings, weekly supervision, monthly clinical supervision meetings. Attend All Staff meetings, monthly staff trainings, program collaborative meetings, attend staff trainings when offered. The Driver/Recovery Support Specialist supports the program by providing direct care to the clients, running groups, ensuring that the clients take their medication, driving clients to appointments, and assisting with service planning. The Driver/Recovery Support Specialist reports to the Program Manager. Essential Functions: Provide quality, empathetic customer service to the clients in the program. Drives clients to appointments as needed. Understand and implement a harm reduction philosophy. Provide a supportive and trauma-informed environment to women in recovery who have dual diagnoses (mental health and substance use). Ensure that when clients take medication, all policies and procedures are followed for assisting and monitoring the clients' self-administration of medication. Perform interventions with clients utilizing a motivational interviewing/strength-based perspective. Responsible for medication documentation, including: counting controlled medication, documenting incoming medication and medication removal, signing off on medication as it is taken by the clients.     The Milieu Engagement Advocate is responsible for the coordination of educational and peer support programming for Boston Living Center members as well as the delivery of services that support ongoing engagement and retention in services. The Milieu Engagement Advocate reports to the Milieu Engagement Manager for support and Supervision. Location: The Boston Living Center is a non-residential community center/resource program in Boston's Back Bay for individuals living with chronic health conditions (e.g. HIV/AIDS), substance use disorder, those experiencing homelessness, food insecurity and/or those at risk of experiencing street-based trauma. Location: The JP Recovery Center is a ‘recovery-oriented sanctuary anchored in the heart of the community,' delivering peer-to-peer recovery support services, and providing a safe, supportive, healing space for individuals in recovery. The Center uses a volunteer/member base, guided by peer staff, to deliver services and provide multiple pathways to recovery. Essential Functions: Act in accordance with Victory Programs Standards of Excellence & Philosophy Care, including maintaining professional ethics, boundaries, and client confidentiality, and working from a strength-based perspective. Provide an ethical framework for service delivery, with guidelines, policies, procedures, and practices consistent with peer values. Provide regular orientation on the basics of a peer model for all members and staff. Provide structured, varied program offerings and accessible calendars including Recovery Capital-building activities, Social Events & Fun activities both at the center and out in the community. Provide Outreach & Advocacy activities with regular opportunities for members to participate in community events, where they can “give back”, share their lived experiences and reduce stigma as they outreach to folks in or seeking recovery, and inform others about the center. Conduct Community Meetings that welcome all to attend and learn about the center while providing members opportunities to receive updates, vote, volunteer, share ideas, provide feedback, and direct programming and activities. Offer multiple Pathways to Recovery including an array of existing and emerging recovery support programs and groups. Provide ongoing Volunteer and Peer Leader recruitment, orientation, and skills development. Provide supervision, mentoring and regular volunteer recognition. Oversee member Management, including maintenance of member contact information and enrollment in ESM to ensure assertive engagement and re-engagement with members. Responsible for implementation of a recovery-oriented data collection management tool for enhanced membership data and program management. Develop and distribute marketing materials in the community to promote services. Collaborate and partner with community organizations, leaders and local treatment and recovery support service providers, coalitions and task forces, career centers and/or employment/housing organizations and criminal justice departments to promote the availability of the center, represent the recovery community, advocate for recovery, and ensure linkages to resources, supports, programs and services that benefit and address life needs of the membership. Enforce all policies and procedures of Victory Programs, Inc. Work with the finance department to complete budgets, approve accounts payable, and monitor financial statements to stay within budget. Work collaboratively with all staff to provide individualized services to all clients. Provide regular supervision to the Program Manager and Outreach Manager. Ensure that there is adequate staff coverage. Provide on-call support as needed. Non-Essential Functions: Attend weekly staff meetings, trainings, seminars, workshops and rallies as requested by the supervisor. Represent the program(s) in the community and to designated funders, as requested by the supervisor. Participate and support the agency in program holiday events, State House rallies, fundraising and advocacy events, HIV/AIDS rally and walk, and any additional functions. Identify alternative funding sources supportive of the program mission. Perform other duties as deemed necessary by your supervisor or his/her designee. Required Qualifications: The successful candidate is someone who has accepted their substance use history, has learned to live with and manage the daily challenges of recovery, and has attained a level of personal growth and healing that gives them the understanding, insight and motivation to help others. High school or equivalent. Completion of Recovery Coach Supervision training. Must demonstrate an understanding and belief in the recovery process and Recovery Oriented Systems of Care Model. Strong leadership skills including professionalism; active listening, critical thinking and effective delegation. Must have knowledge of multiple pathways to recovery support systems, including but not limited to 12-step support groups, faith-based groups, harm reduction services, SMART Recovery, Life Ring; and providing direct service. Must be independent and self-directed but able to work collaboratively and lead or manage a team. Ability to act with integrity and compassion and foster a culture of mission, trust, respect and accountability for outcomes. Ability to demonstrate cultural awareness and competency, including the ability to understand, respond and work effectively and with sensitivity with staff and members from diverse cultures and backgrounds. Must be CPR certified (may be obtained upon hire) Completed TB screening (may be obtained upon hire) Three work related professional references; at least 2 from manager or supervisor. Preferred Qualifications: Bachelor Degree in the social services field and a minimum of three years of management experience. Certified Addiction Recovery Coach (CARC) certification. Experience working within and/or leading a Peer Participatory Model program. Strong meeting, training, facilitating and project management skills. Demonstrated competence in service coordination and networking with other human service and health care providers. Strong understanding of non-profit organizations and the ability to implement/establish best practices. Able to work creatively within a budget and find ways of more effective management resources.     The Housing Stabilization Case Manager is responsible for providing housing search, advocacy and stabilization support to individuals exiting substance use treatment through the Mayor's Office of Housing / Rapid Re-Housing program. The program is designed to provide a pathway to increased housing retention through rental assistance and supportive services intending to lead to retention in permanent housing. The Housing Stabilization Case Manager reports to the Program Manager of the Mayor's Office of Housing / Rapid Rehousing program. Location: Paths to Victory serves clients at our office in the South End and by meeting clients where they are in the Boston area. Paths to Victory provides housing access, housing stabilization, and wrap-around services to those experiencing literal homelessness and those exiting substance use treatment. We assist clients with moving into housing, accessing rental assistance, and connecting with all necessary supportive services. This program serves as a bridge to obtaining permanent, safe housing, increasing self-sufficiency, and gaining skills which support stable housing and health.
