The ACCS Housing Coordinator promotes community integration and independence among adults with psychiatric conditions by assisting with identifying, accessing, and maintaining suitable housing. The ACCS Housing Coordinator will work to with each Person Served to ensure housing support is expedient and will do so by developing and maintaining relationships with local property managers, landlords, housing authorities.
In the role you will use recovery-oriented, person-centered approach to help Person Served throughout the process of securing and/or maintaining housing. You will also provide outreach services to each Person Served whether they currently reside in shelters, on the street, in temporary housing/respite or residential Group Living environments to identify their housing needs and preferences.

The LTSS Supervisor provides team oversight of care coordination and clinical care management for MassHealth Members with complex medical and behavioral health needs who are enrolled in an Accountable Care Organization (ACO) or Managed Care Organization (MCO) plan. The Supervisor collaborates with the Community Partner team and the clinical staff of each Enrollee's ACO/MCO's plan to minimize duplicative efforts, promote integrated care, ensure quality and continuity of care, and support the values of person-centered planning. The Supervisor is at the helm of organizing and coordinating resources and services in response to the Enrollees' healthcare needs across multiple settings, and inclusive of both LTSS and SDH needs. This role drives assessment and care planning, care transitions, health and wellness coaching, as well as community and social services connections across a multi-disciplinary care team for CCP enrollees.

Essential Functions of Position:
Works cooperatively with staff and program participants as assigned to implement objectives and therapeutic activities that support Service Plan goals. Ensures that the participants' needs are met and communicated with program staff and treatment team according to regulations and prescribed need.
Coordinates participant's personal care schedule with program staff to meet all the needs of the program. Performs personal care functions for individuals with disabilities, including changing, toileting, feeding, and laundry. Uses adaptive equipment and mechanical lift systems competently as per training and supervision to facilitate the movement and lifting of physically disabled individuals for personal care assistance.

Trains participants to maximize independent functioning in self-care and activities of daily living related to personal care, including personal hygiene, grooming, and eating. Participates in the development and implementation of Day Habilitation Service Plans and IDT as a member of the Day Habilitation team.
Reports medical and health concerns to the Healthcare Supervisor and Program Director. Provides emergency interventions as needed.
Delivers services to program participants with respect and dignity in accordance with human rights training, DDS regulations for human rights, and Bay Cove standards.
Provides participant coverage for other groups and activities as assigned.
Demonstrates proficiency with the use of electronic database, email, electronic record keeping and on-line training systems. Provides required documentation for participant activities and personal care records for each day.
Other duties as assigned by the Program Director, Day Hab Service Manager or R.N.
Requirements for the position:
High School degree
Certification as Nurse's Aide is preferred.
One year of job experience in a training program working with this population and/or Associate Degree in human services related field preferred
Maintain required training certifications including CNA, CPR, and First Aid.
Must have had a TB screening within the past 12 months, & required every year.
Ability to lift 30 pounds. Ability and physical capacity to perform physical care tasks to assist people with mobility impairments as needed, utilizing safety protocols and approved techniques.

Job Summary: The Safety Specialist promotes health, wellness and safety in our low threshold services for adults with histories of homelessness, most of whom are impacted by substance use and misuse, co-occurring mental health challenges, and complex health conditions. The Safety Specialist directly supports Bay Cove's mission by working on behalf of persons served, staff, and all community stakeholders in a proactive and professional manner. This staff member is critical to providing a welcoming and inclusive environment for persons who live in our permanent housing programs and whose work reflects a commitment to health, racial and housing equity.
By providing respectful and responsive supports and by working collaboratively with all stakeholder, this staff member implements hare reduction and trauma informed supports to pesos who have often been marginalized and stigmatized by others. The Safety Specialist brings great value to our tenants and team members by offering practical assistance in a calm, positive, and respectful manner. In such environments, people can work to build or rebuild their lives, and enjoy the benefits of a safe and affordable home. The Safety Specialist severs as a community diplomat and represents the CCC and Bay Cove as a good neighbor and responsive community partner.

Essential Functions of Position:
Welcomes and logs in all guests, visitors, and community partners.
Reinforces expectations and policies for tenants and guests that support the safety and benefit of all tenants, staff, and neighbors. Ensures that Narcan is administered in the event that a person over doses and waits with the person until First Responders are onthe scene. From the front desk, monitors cameras with all staff to support tenants' needs. Ensures that health and safety systems (security camera, Automated External Defibrillator, door alarms, smoke detectors in common kitchens, First Aid kits, CPR masks, Narcan, fentanyl test strips, etc.) are all in good supply and in working order. Intervenes in crisis situations using good judgement, communication skills, de-escalation techniques, and calling upon emergency services as needed.
Demonstrates dependability and reliability.
Must be able to work all shifts including weekend and holidays as scheduled.
Attends all required training.
Communicates in real-time with Program Director and staff at the CCC.
Maintains privacy and confidentiality of tenants' personal and private health conditions in accordance with state and federal law.
All other duties as requested by the Program Director.

Compensation:
