Schedule: Sun-Thu 7am-3pm Salary: $39.50 This role is eligible for a $700 per month stipend for the candidate hired into this role who lives further than a 25-mile radius from this worksite. Responsible for completing all skilled nursing care needs of the individual; including but not limited to, wound care, GT/JT care, trach care and nutritional support. Monitors and observes clients for any change in status. Reports all issues in a timely manner to the PD, SPD (if needed), and Program Nurse Ensures that all the individual's care is completed using best-practice and providing the person with dignity and choice. Ensures that ADLs are performed, and assistive devices are in working order and available and ready for use. Performs individualized assessments with any change of status and provides skilled nursing interventions, as indicated. Supervision Clinically supervises C.N.A.s during designated shifts. The Restoration Center (RC) Licensed Practical Nurse (LPN) will offer medical support, care coordination, and rehabilitation options to those in crisis who would benefit from a diversionary level of care to stabilize and return to the community. The LPN is responsible for providing direct care and nursing care to meet identified individuals' health maintenance needs. The LPN works as part of a multidisciplinary team to support people at the RC around all domains of living and to support the achievement of goals identified by the individual.
