JOB SUMMARY: Located at the LAC+USC Medical Center HUB Clinic, Case Manager is responsible for providing acute case management services, Individual Rehabilitative services, groups, referrals and linkage. The Case Manager will interact with the community at large and with specific allied agencies. Hours for this position are Monday to Friday 8:30-5:00 pm. Requirements ESSENTIAL DUTIES & RESPONSIBILITIES: Reporting to the program supervisor, the Case Manager is responsible for the following: Work collaboratively with members of inter-disciplinary teams and provide ongoing clinical consultation to DCFS and DMH. Develop relationships and ongoing communication with DCFS workers and other staff. Collaborate with DCFS Clinical Social Workers to ensure the child or youth is linked to the appropriate level of care services. Identify the strengths and needs of the youth and family. Identify natural supports that exist within the family structure and the community and provide linkage to additional resources. Provide Individual Rehabilitative Services and/or group services until linkage to ongoing mental health services is complete. Follow up with clients and families regarding linkage, resources, and services to ensure that their needs are being met. Complete all required documentation in accordance with regulations (e.g. DMH); and enter progress notes into the web-based Electronic Health Record software (CareLogic). Follow agency policies and procedures. Participate in VIP in-service training programs. Perform other duties as assigned. BASIC QUALIFICATIONS: Bachelor's degree and two (2) years documented experience in a mental health setting; OR Associates of Arts degree plus four years' experience in a relevant mental health setting; OR 6 years' experience in a relevant mental health setting. Knowledge in working with the DMH Integrated System (IS), IBHIS and Department of Healthcare Services Medi-Cal. Ability to exercise good judgment and act calmly in emergency situations. Bilingual in Spanish. Experience and ability to work in a fast-paced environment with good time management skills. Self-starter, independent worker with the ability to adapt to change in a fast working environment. Good written and verbal communication skills. Valid California driver's license, driving record and personal vehicle insurance acceptable to VIP's insurance carrier. ESSENTIAL DUTIES & RESPONSIBILITIES • Ability to work collaboratively with members of inter-disciplinary teams; and ability to provide ongoing clinical consultation to medical staff; • Demonstrated awareness of, sensitivity to and competence in dealing with cultural and socioeconomic diversity of client population; • Conduct mental health screenings/crisis intervention, integrating forensic interviewing techniques and formulating recommendations; • Educate patient and family regarding the diagnosis, mental health needs, and provide appropriate mental health referrals; • Develop relationships and ongoing communication with DCFS workers and other staff; • Participate in training on intervention techniques; • Complete all required documentation in accordance with regulations (e.g. DMH); • Maintain all appropriate records including crisis intervention files and/or incident reports, clinical notes and other records as required; • Follow agency policy and procedures; • Participate in VIP in-service training programs; and • Perform other duties as assigned. Participates in VNS Health meetings, including but not limited to, interdisciplinary team meetings, in-service and training programs, rehab services meetings and caseload reviews. Travels to patients' homes and/or other facilities with varying environments (e.g., elevated buildings, walk-ups, care facilities, single/multiple family homes, presence of pets, etc.) using approved transportation options to deliver direct care to the patient. Assists patients with therapeutic interventions and activities of daily living, which may require positioning, moving, transferring and lifting patients of varying body sizes and physical conditions, with and without assistance and as appropriate, instructing, demonstrating and verifying capabilities of family members and/or paraprofessionals. Ability to use and assist patient with use durable medical equipment as a component of the plan of care, including but not limited to wheelchairs, patient transfer lifts, ambulatory devices, slings, splints, orthoses, prostheses, and exercise/strengthening equipment. Provides care in accordance with Agency policies, practices, procedures and standards, including professional practice standards of care, which may require standing, stooping, sitting, crouching, twisting, reaching, bending and stretching to deliver patient care.
