The Home Health RN provides skilled, compassionate nursing care to patients in their homes in accordance with physician orders, agency policy, and Medicare Conditions of Participation. The RN is responsible for comprehensive assessments, developing and managing individualized care plans, and collaborating with the interdisciplinary team to ensure quality outcomes. Primary Responsibilities
Patient Care & Clinical Oversight
Conduct comprehensive assessments and develop individualized care plans.
Deliver skilled nursing services including medication management, wound care, and patient education.
Monitor and adjust care plans based on patient status and needs.
Collaborate with physicians and the care team for coordinated care.
Educate patients and caregivers to promote independence and safety.
Case Management & Documentation
Manage a primary caseload efficiently and cost-effectively.
Maintain accurate, timely clinical documentation in compliance with regulations.
Participate in IDT meetings and case conferences.
Supervise Home Health Aides in accordance with Medicare guidelines.
Professionalism & Communication
Communicate effectively with patients, families, providers, and colleagues.
Uphold patient rights, privacy, and confidentiality.
Act as a resource and mentor to peers and students as needed.
Participate in performance improvement, staff meetings, and continuing education.
Regulatory Compliance & Safety
Adhere to PA licensure laws, Medicare regulations, and agency policies.
Follow infection control, safety, and emergency procedures.
Participate in on-call rotation as assigned.
On-Call & After-Hours Care
Participate in evening, weekend, and holiday on-call rotation as assigned.
Secondary Responsibilities
Provide visits to hospice patients as needed.
Participate in agency-wide initiatives and committees.
Perform other duties as assigned.
Physical Requirements and Working Conditions
Able to work in varied home settings and conditions.
Requires walking, standing, lifting (up to 50 lbs), bending, and driving.
Must have reliable transportation, a valid PA driver's license, and current auto insurance.
Qualifications
Graduate of an accredited RN program; BSN preferred.
Active RN license in the Commonwealth of Pennsylvania.
Minimum 1 year of nursing experience; home health or acute care preferred.
CPR certification required.

The VNA of Hanover & Spring Grove is seeking a dedicated and compassionate Registered Nurse to join our interdisciplinary team in a dual-capacity role supporting both Home Health and Hospice services. This position offers a unique opportunity to provide skilled care during daytime home visits while also responding to emergent Hospice needs during evening hours.
Position Overview:
The Home Health and Hospice RN will manage a small Home Health caseload from 12:00 PM to 4:00 PM, followed by providing Hospice support from 4:00 PM to 8:00 PM. Evening responsibilities include completing hospice consents, administering IV medications, and responding to urgent patient needs. The ideal candidate will demonstrate strong clinical judgment, excellent communication skills, and the ability to work independently while collaborating within a team-oriented environment.
Key Responsibilities:
Clinical Care & Case Management
Perform comprehensive nursing assessments and develop individualized care plans
Provide after-hours hospice services including consents and IV administration
Deliver patient and caregiver education on disease progression and symptom management
Monitor and report changes in patient conditions and adjust care accordingly
Collaborate with physicians and interdisciplinary teams for coordinated care delivery
Documentation & Communication
Maintain timely, accurate clinical documentation in the electronic health record (EHR)
Communicate professionally with patients, families, and team members
Participate in care conferences and case reviews

Effectively manage caseloads and prioritize care delivery

Supervise Home Health and Hospice Aides as required

WHERE YOU'LL WORK
Each day is different. You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility. Most VITAS nurses make 4-6 visits each day.
Once a week, you'll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and nurses, who collaborate to provide holistic support for the patients and families we serve.

4. Registered Nurse, RN, completes initial and ongoing comprehensive assessments including physical, psychosocial, spiritual, and bereavement in order to provide palliation of end-of-life symptoms
5. Registered Nurse, RN, appropriately identifies/utilizes resources, referrals such as PT, OT, Home Health, Dietary, Pharmacy, Supplies, Home Medical Equipment, and other visits/consults
6. Registered Nurse, RN, completes documentation at the time of the event and at the location of care unless otherwise directed
7. Registered Nurse, RN, provides education and training to patients, families, caregivers and referral sources in the care of the patient, including pain management
8. Registered Nurse, RN, participates in patient care meetings as assigned
9. Registered Nurse, RN, participates in weekly team meetings
10. Registered Nurse, RN, provides effective communication/shift reports through written, verbal, reporting and body language
11. Registered Nurse, RN, participates in bereavement support

i. All factors pertinent to the patient's safety, including the competence of the Hospice Aide/Homemaker have been assessed
ii. Determination that this activity can be delegated safely to a Hospice Aide/Homemaker
19. Registered Nurse, RN, provides nursing care including nursing tasks that may be delegated to other roles such as LPNs, HAs and Volunteers
20. Registered Nurse, RN, participates in and monitors patient and family care compliance activities
21. Registered Nurse, RN, professionally manages/coordinates the hospice plan of care
22. Registered Nurse, RN, provides and follows infection control practices

+ Prior experience in triage nursing, emergency care, or crisis response is a plus

The AIM Palliative RN will collaborate with an interdisciplinary team including Nurse Practitioners, Social Workers, and Primary Care Providers. Key Responsibilities:
+ Conduct in-home and telephonic nursing assessments and screenings for palliative care patients
+ Complete risk stratification assessments and determine frequency of visits based on patient needs
+ Deliver patient and family education on disease progression, medications, nutrition, and end-of-life care
+ Collaborate with NPs and PCPs to identify changes in patient condition and escalate care appropriately
+ Coordinate care transitions and facilitate hospice referrals when needed
+ Accurately document clinical findings, assessments, care plans, and education in the Electronic Medical Record (EMR)
+ Support the development and regular updates of individualized Palliative Care Plans
+ Communicate lab results, order DME, and manage prescriptions in accordance with state RN scope of practice

About You
Required Qualifications:

Obtains Treatment authorization and admit patients to the agency.
Performs a comprehensive clinical evaluation.
Informs the physician of patient needs and obtain orders for any required changes.
Initiates the plan of care.
Teaches, supervises, and counsels the patient and family members regarding the nursing care needs and other related problems of the patient at home.
Performs any direct care necessary per physician orders.
Ensures patient is referred to appropriate agencies for continued supplemental services.
Completes Home Health OASIS assessment appropriate for patient visit timepoint.
Documents all activities in the Electronic Medical Record per agency policy.
Distributes a written start of care report to all care team members.
Meets established productivity targets by regularly checking in with Supervisor regarding any scheduling changes or additional assignments needed.
Requirements
Graduate of an accredited school of nursing
Current RN license in good standing to practice in the Commonwealth of Pennsylvania
Prior home health nursing experience
Maintains clinical and professional competence
CPR certified
Driver's License and Car Insurance in good standing
Reliable vehicle
Possesses the ability to follow oral and written instructions
Possesses the ability to communicate effectively with all members of the care team
Possesses good organizational skills
Proof of COVID-19 Vaccination or an approved Medical or Religious exemption

Job Description
We are currently seeking a dedicated and passionate Clinical Liaison Registered Nurse (RN) to join our team at KeystoneCare. This vital role involves acting as an advocate for patients within an acute care setting until they are safely discharged to their homes. The Clinical Liaison RN is responsible for generating referrals for home health, palliative care, hospice, waiver, and private duty services by building strong relationships with case managers and social workers. In addition to meeting with patients and their families, you will ensure a seamless transition of care throughout the discharge process, guiding them through their options and verifying eligibility for care.

Primary Job Responsibilities:
Act as the primary referral contact between assigned hospital partners and KeystoneCare.
Establish and maintain professional relationships with case managers, discharge planners, and social workers.
Conduct clinical assessments of potential referrals to ensure appropriateness against eligibility criteria.
Facilitate a smooth transition of care from the hospital to home by assessing and communicating all patient needs prior to discharge.
Conduct regular, planned visits to assigned facilities and remain available by phone during business hours.
Promote Home Health, Palliative Care, Hospice, and Private Duty services within the designated market.
Achieve or exceed targets for referral volume and growth.
Maintain knowledge of all assigned hospitals and stay updated on changes and trends in the healthcare marketplace.
Requirements
Job Requirements:
Valid RN license in the state of Pennsylvania.
3+ years of experience in a clinical care coordinator, liaison, or case manager role preferred.
Self-motivated with strong time management and communication skills.
Clinical knowledge of disease entities common to Home Health and Hospice.
Valid Driver's License and a reliable vehicle are required.
