Job Description:
Under the direction and supervision of the Vice President of Operations, the Intake Specialist - Non-Clinical will be an integral member of the Intake Department and will contribute to increasing VNA Health's revenue and reputation by converting referrals to successful admissions by providing excellent customer service and maintaining high standards of quality in work product. The Intake Specialist - Non-Clinical will respect and promote the mission, values, and vision of VNA Health.
Responsibilities:
* Assess, triage, and process referrals to successful admissions in a timely, accurate, and professional manner.
* Process in accordance with VNA Health's and Intake's policies/protocols and CMS regulations.
* Obtain required information from referral sources (physicians, discharge planners, case managers, and other facility staff) and follow their policies/procedures.
* Collaborate with VNA Health departments to meet relevant requirements, protocols, and regulations.
* Communicate with patients/families to educate about and facilitate for successful home care services.
* Meet or exceed goals regarding referral-to-admit conversion rate and face-to-face encounter documentation.
* Enter all pertinent data in EMR and other electronic and manual systems with a high degree of accuracy, timeliness, and completeness.
* Respond to all inquiries (phone, email, mail, fax, in-person) of internal and external customers in a timely, accurate, and professional manner.
* Demonstrate comprehensive working knowledge of VNA Health's programs and services, including associated regulations and payment sources, and communicate them effectively to constituents.
* Maintain strong proficiency in external data systems (such as ProAccess, Wave, Aiden, etc.).
* Comply with applicable Joint Commission standards.

Qualifications:
* Bachelor's degree preferred.

The Bereavement Counselor is responsible for providing bereavement counseling and educational services for VNA Health clients as directed by the Hospice Director. The Bereavement Counselor respects, represents, and promotes the mission, values and vision of VNA Health.
ESSENTIAL JOB DUTIES AND RESPONSIBILITIES
The incumbent serves as the Bereavement specialist for VNA Health's interdisciplinary Hospice team(s), and for Community clients as delegated by the Social Services Supervisor. The incumbent:
1. Provides mail, phone, and in-person follow up to bereaved Hospice families as appropriate.
2. Provides grief counseling (individual, family, and group) to Hospice and Community clients as appropriate.
3. Provides a clinical assessment and plan for bereaved clients, when appropriate.
4. Provides information and referral for bereaved clients, when appropriate.
5. Engages with clients in a timely and professional fashion.
6. Completes appropriate and required documentation in a timely fashion.
7. Handles and prioritizes multiple tasks simultaneously, while delivering Bereavement services in an efficient, professional, and compassionate manner.
8. Demonstrates a good understanding of customer service and quality management principles.
9. Assists with Bereavement program planning and development as requested by the Social Services Supervisor.
10. Functions independently, but seeks and accepts clinical supervision and program guidance from the Social Services Supervisor.

For patients who plan to return to work, the occupational therapist may perform work capacity evaluation and may refer to specialized vocational training in accordance with Organization policy.
Develops a treatment program and establishes goals for improved function. Communicates plan of care to referring physicians and other Organization professionals.
May teach new skills or re- train patients in once familiar daily activities that have been lost due to illness or injury, in accordance with Organization policy.
May fabricate splints and instruct patients in the use of various types of adaptive equipment to improve function.
May train patients in the use of prosthetic and/or orthotic devices.
Maintains appropriate clinical records, clinical notes, and reports to the physician any changes in the patient's condition. Submits these records in accordance with Organization policy.
Maintains contact/communication with other personnel involved in the patient's care to promote coordinated, efficient care.
Amends and contributes to in-services, case conferences, and other meetings as required by Organization policy to ensure coordinated and comprehensive plans of care for the patients of the Organization.

Prepare, print, all materials required for case conference meetings
Support bi-weekly case conferences (every other Thursday), including:

Job Summary:
The Medical Social Worker (MSW) contracted or employed through the organization is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. Inspects and maintains appearance of all physical space surrounding the Loan Closet, including the area designated for recycling and debris.
Maintains the wash rack in a like-new appearance on a daily basis.
Performs routine maintenance and repair of DME equipment. e. Maintains the DME stock in an organized manner.
Provides written instructional materials for proper use and safety of equipment.
Refers appropriate clients to PT Manager, as needed, for proper fit and use of equipment.
Checks equipment for cleanliness and working condition prior to loaning out. In conjunction with the Facilities Engineer, regularly inspect office and DME to ensure it is in proper working order.
Cleans and sanitizes incoming equipment each day.
DME must be must be washed and dried in the cleaning room and then replaced in inventory. No equipment is to be taken outside to dry in the sun or placed in the parking lot.
Uses protective clothing and devices when performing tasks that involve the handling of infectious waste, blood or body fluids.
Sends equipment out for repairs, as necessary, or arranges for disposal or donation.
Keeps clean and dirty equipment in designated areas. 4. Receives and processes all donations of equipment.
Inputs data daily to record loans.
Contacts clients on overdue equipment.
Follows policy and procedures and suggests improvements.
Works with volunteers and other staff to ensure DME availability to the community

We are currently seeking to fill the Hospice Chaplain position.
-Position to be performed in Kern, Tulare County, and surrounding areas.
-Compensation $30-$35/ hr (negotiable, based on experience, etc.).
-1 year of experience in Hospice preferred.
About the Position
The Hospice Chaplain is responsible for the provision of spiritual care services to patients and
families/caregivers of Compassionate Care Hospice Central CA, either directly or through
coordination of care with other spiritual counselors.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
1. Assures spiritual assessment of patients and families/caregivers in the hospice program
and appropriate services are rendered in a timely manner.
2. Facilitates the development of the individualized the plan of care by participating in the
comprehensive assessment to meet identified spiritual needs.
3. Provides direct spiritual care to patients and families/caregivers.
4. Serves as liaison and support to community chaplains and spiritual counselors.
5. Maintains records of spiritual care services utilization and related activities for quality
assurance, program development, and policies and procedures review and revision.
6. Documents direct services and ongoing communication with community chaplains and
spiritual counselors.
7. Attends patient care conference and interdisciplinary group meetings as a member of the
interdisciplinary group.
8. Provides consultation, education, and support to the interdisciplinary group on spiritual
care.
9. Recruits community chaplains, spiritual counselors, and spiritual care volunteers adequate
to meet patient and family/caregiver needs by developing community contacts and offering
education through congregations about hospice care.
10. Provides for funeral or memorial services for patients as requested.
11. Plans periodic memorial services to meet the needs of personnel, volunteers, and
community clergy/spiritual counselors working with the hospice team.
12. Assists in supervision of spiritual care volunteers when assigned to patients/
families/caregivers.
13. Actively participates in quality assessment performance improvement teams and activities.
14. Accepts and performs other related duties and responsibilities as required.

POSITION QUALIFICATIONS
1. Graduate of accredited seminary or school of theology (BD, MDiv or equivalent theological
degree). Experience working with death and dying, individuals/family/caregiver.
2. Ability to work as member of interdisciplinary group.
3. Comfortable in an interfaith setting.
4. Ability to accept different lifestyles, cultures, beliefs, and values.
5. Ability to network with community clergy and congregations.
6. Knowledge of and commitment to hospice philosophy of care.
7. If making home visits, must be a licensed driver with an automobile that is insured in
accordance with state and/or organization requirements and is in good working order.
8. Hospice/experience preferred.

Job Description:
The Registered Nurse (RN) provides hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. The RN respects and promotes the mission, values and vision of VNA Health.
Patient Care
Responsible for providing comprehensive care to hospice patients.
Completes an initial assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care.
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient's status by:
Writing and initiating plan of care
Regularly re-evaluating patient and family/caregiver needs
Revising the plan of care as necessary
Uses health assessment data to determine nursing diagnosis.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
Has working knowledge of state, federal, local and accreditation regulations for the delivery of hospice services.
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
Communicates with community health related persons to coordinate the care plan.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the other hospice Interdisciplinary Team Members.
Provides and maintains a safe environment for the patient.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
Additional Duties
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services
are available.
Supervises ancillary personnel and delegates responsibilities when required.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
Fulfills the obligation of requested and/or accepted case assignments.
Actively participates in quality management program.
Meets daily average productivity standard set by the organization.
