Job Description:
The Registered Nurse (RN) provides hospice care and is experienced in nursing, with emphasis on community health education/experience. The professional nurse builds from the resources of the community to plan and direct services to meet the needs of individual and families within their homes and communities. Patient Care
Responsible for providing comprehensive care to hospice patients.
Completes an initial assessment of patient and family to determine hospice needs. Provides a complete physical assessment and history of current and previous illness(es). Admits to appropriate level of service/ care.
Provides professional nursing care by utilizing all elements of nursing process.
Assesses and evaluates patient's status by:
Writing and initiating plan of care
Regularly re-evaluating patient and family/caregiver needs
Revising the plan of care as necessary
Uses health assessment data to determine nursing diagnosis.
Develops a care plan that establishes goals, based on nursing diagnosis and incorporates palliative nursing actions. Includes the patient and the family in the planning process.
Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician in the physician's plan of care.
Counsels the patient and family in meeting nursing and related needs.
Provides health care instructions to the patient as appropriate per assessment and plan.
Assists the patient with the activities of daily living and facilitates the patient's efforts toward self-sufficiency and optional comfort care.
Has working knowledge of state, federal, local and accreditation regulations for the delivery of hospice services.
Completes, maintains and submits accurate and relevant clinical notes regarding patient's condition and care given. Records pain/symptom management changes/outcomes as appropriate.
Communicates with the physician regarding the patient's needs and reports changes in the patient's condition; obtains/receives physicians' orders as required.
Communicates with community health related persons to coordinate the care plan.
Teaches the patient and family/caregiver self-care techniques as appropriate. Provides medication, diet and other instructions as ordered by the physician and recognizes and utilizes opportunities for health counseling with patients and families/caregivers. Works in concert with the other hospice Interdisciplinary Team Members.
Provides and maintains a safe environment for the patient.
Assists the patient and family/caregiver and other team members in providing continuity of care.
Works in cooperation with the family/caregiver and hospice Interdisciplinary Team Members to meet the emotional needs of the patient and family/caregiver.
Additional Duties
Participates in on-call duties as defined by the on-call policy.
Ensures that arrangements for equipment and other necessary items and services
are available.
Supervises ancillary personnel and delegates responsibilities when required.
Assumes responsibility for personal growth and development and maintains and upgrades professional knowledge and practice skills through attendance and participation in continuing education and inservice classes.
Fulfills the obligation of requested and/or accepted case assignments.
Actively participates in quality management program.
Meets daily average productivity standard set by the organization.

JOB DESCRIPTION SUMMARY
The Office-Based RN Case Manager is a central clinical leader responsible for the oversight, coordination, and communication of hospice care for both home-based and facility-based patients. Operating from the central hospice office, this RN ensures timely implementation and monitoring of individualized plans of care (POCs), facilitates communication between interdisciplinary teams, and supports families, caregivers, and facility staff. This role is essential to ensuring regulatory compliance, timely documentation, and seamless coordination across all levels of hospice care delivery.
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES
Core Responsibilities
1. Clinical Oversight & Plan of Care Management
Initiate, review, and revise hospice POCs
Ensure interventions are carried out by field staff
Track documentation to confirm symptom management and visit completion
Maintain up-to-date documentation in the EMR (HospiceMD)
2. Communication & Coordination
Serve as the primary point of contact for families, facilities, marketers and providers
Notify parties before and after changes in condition or orders
Conduct wellness calls for new admissions, imminent cases, and random check-ins
Monitor Google Chat rooms and follow up on issues to ensure loop closure
Review EOS reports from night/weekend shifts, post to patient chats, and follow up
3. Documentation & Compliance
Document all care coordination and communication activities
Ensure hospice and facility documentation align (meds, DME, MAR, 485s)
Prepare and submit recert narratives notes in advance to field RN.
Ensure all notes, plans, and IDG documentation are survey-ready
4. Resource & Order Management
Submit and follow up on medication and DME orders
Confirm delivery from pharmacy and vendors
Work with staff and couriers to resolve pharmacy billing/authorization issues
Coordinate deprescribing discussions with pharmacists and families
5. Team Integration
Attend and prepare for weekly IDG meetings
Provide handoffs and updates to night/weekend on-call staff
Support field team with triage, follow-up, and case management guidance
Daily Tasks
Home Patients
Review prior shift charting and communications
Conduct wellness and symptom check-in calls
Update care plans and coordinate field visit follow-through
Monitor medication/DME delivery and order status
Document updates in the chart and patient chat rooms
Facility Patients (SNF, ALF, B&C)
Call facility nurses for overnight updates and visit coordination
Review meds, MARs, wound care, and order follow-through
Ensure documentation is delivered/uploaded as needed

Qualifications

If making patient visits implements current nursing practice following a patient assessment and an approved plan of care initiated by the Registered Nurse.
If making patient visits provide accurate and timely documentation of patient services to reflect the plan of care.
If making patient visits assess and provide patient and family/caregiver education and information pertinent to diagnosis and self plan of care.
If making patient visits participates in coordination of hospice services, appropriately reporting the identified needs for hospice interdisciplinary group members. Examples include hospice aide, OT, PT, MSW, ST, Dietitian or Hospice Manager.
If making patient visits use equipment and supplies effectively and efficiently.
If making patient visits provides appropriate pain/symptom management. Evaluates patient's response to treatments/medications.
Participates in personal, professional growth and development. Also participates in organization's quality management program.
Performs other duties as assigned by the Hospice Director.
Actively participates in Quality management program.

All clinical staff are expected to participate as needed in variable shifts, including weekend rotation per staffing needs and departmental holiday rotation.
Qualifications:
Current and active California Licensed Vocational Nurse (LVN) license.
Minimum one year experience required, three years' experience preferred. Community health/hospice or medical/surgical experience is preferred.
Current CPR certification.
Licensed driver with insured automobile - in accordance with state and/or organization requirements.
Complies with accepted professional standards and practice.
Demonstrates good verbal and written communication, and organization skills.
Understands philosophy of hospice concept. Also understands needs of the terminally ill.
Bilingual - Spanish preferred, but not necessary.
Physical Requirements:
Ability to lift 50 lbs.
Ability to push/pull 50 lbs.
Ability to stoop, bend, and squat.
Ability to walk up one flight of stairs.
Ability to walk one mile.
Ability to sit for one hour, minimum.
