Makes recommendations for services and care for at home based on an assessment of the patient's current condition Knowledge of patient medical, psychosocial, and environmental situation. Effectively communicates the care needs, services lines and other pertinent demographic and clinical information using Visiting Nurse's EMR Completes and submits all comprehensive documentation that supports the referral, and F2F when required. Ensures all referrals have a documented physician written or verbal order to start care. Establishes and maintains strong relationships with key stakeholders in each of the hospitals. Initiates a plan of treatment in collaboration with the physician, hospital staff and patient and effectively communicates to the patient and family. What You'll Do: Oversee daily activities of field clinicians, managing patient care delivery and staff assignments to ensure safe, high-quality outcomes. Lead, coach, and develop team members through regular feedback, performance evaluations, and mentorship. Coordinate patient plans of care with families, clinicians, and physicians. Ensure all clinical documentation meets regulatory, accreditation, and reimbursement standards. Review referral information, participate in start-of-care conferences, and guide clinicians to ensure plans reflect patient acuity and avoid unnecessary hospitalizations. Monitor OASIS assessments, case conferences, recertifications, and discharges. Conduct branch investigations to resolve patient concerns, grievances, or compliance audits. Utilize clinical dashboards and agency reports to track documentation, productivity, and quality metrics, intervening when standards aren't met. Collaborate with branch leadership to develop and implement strategic goals. Serve as a key resource on Medicare, Medicaid, third-party payer requirements, and best practices in care coordination and case management. Support new employee recruitment and onboarding, oversee orientation, and assist with hiring decisions. Coordinate complex care needs, such as infusions, wound cases, lab communication, and transitions to hospice or palliative care. Ensure adherence to infection control protocols, privacy practices, and compliance with local, state, and federal regulations. Schedule: Full-time Monday - Friday You're Perfect for the Position if you: Are a Registered Nurse graduate from an accredited Diploma, Associate, or Bachelor's program (BSN preferred). Hold current Georgia RN licensure, valid CPR certification, driver's license, automobile insurance, and reliable transportation. Have 1-2 years of home health or hospice leadership experience, or 3-5 years as an RN Field Nurse ready to advance into management. Understand the physical, social, and emotional needs of elderly, disabled, and terminally ill patients. Are knowledgeable about Medicare, Medicaid, and payer regulations affecting home health. Have advanced nursing skills, a collaborative approach, and strong observation, verbal, and written communication abilities. Possess excellent analytical, team-building, coaching, and conflict management skills. Demonstrate accountability in driving results, managing documentation standards, and promoting high clinical outcomes.
