Job Description: Looking for Baylor Homecare RN

For: either Friday, Saturday, Sunday or Saturday, Sunday, Monday 3 days per week, paid for 5 to provide skilled services to our patients in the community.

As part of a person centered care model, this role will provide skilled nursing care to clients in home and community based settings ensuring the delivery of outcome based, high quality, cost effective care. This role will collaborate with all team members to achieve exceptional outcomes.

Responsibilities


  • Provide compassionate direct or indirect nursing care to homecare patients and families throughout the service area
  • Provide nursing services to include assessment, treatment of symptoms, preventative measures, education and time of death care, according to the plan of care
  • Work across homecare teams to provide daily visits to meet patient and staffing needs in patient homes, long term care facilities and assisted living facilities
  • Complete 5 assigned visits per day or as otherwise determined with clinical leadership in accordance with patient acuity
  • Appropriate and timely follow-up with physicians and other clinicians to meet patient and family needs
  • Compliance with clinical documentation, complete and accurate orders in the Electronic Medical Records system

Requirements

  • Current NC license as Registered Nurse (RN)

  • Maintain a positive culture that is built upon commitment to patients, families and co-workers
  • Demonstrate uncompromising respect for patients, families and co-workers
  • Excellent nursing assessment and problem solving skills
  • Exceptional customer service

As a Registered Nurse (RN), you will :

  • Provide in-home direct patient care according to the provider's prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery.
  • Assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.)
  • Administer medication, insulin, and IV/fluids; documenting thoroughly.

  • Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
  • Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.

To qualify for a Registered Nurse (R N) with us :

  • Education: Associate degree in Nursing (AND) or Associate of Applied Science in Nursing (AAS) required
  • Licensure: Current unrestricted license to practice as a Registered Nurse (RN) in the state associated with this position
  • Current CPR/AED/BLS/First Aid certification
  • Reliable transportation to/from care sites and/or work locations
  • Practical trach and/or ventilator experience preferred, not required
