The LPN/LVN administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the NursePractice Act in the State where the VITAS program is located.
JOB RESPONSIBILITIES:
Provides direct skilled bedside nursing care.
Evaluates patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
Performs selected acts in the care of the terminally ill under the supervision of a registered nurse; such acts include the administration of treatments and medications.
Evaluates effectiveness of treatments and monitors patient's reactions.
Reports significant findings or changes in the patient's condition to the registered nurse, Team Manager or Physician. Documents reports in clinical notes.
Maintains a clean and safe environment for the patient and family.
Maintain accurate documentation including but not limited to clinical notes on interdisciplinary charts.
Evaluates patient/family response to care.
Utilizes a wide variety of supportive and technical nursing skills in carrying out specific medical orders for administration of medication, performing specific treatments, monitoring diagnostic, therapeutic, and supportive measures.
Assists the interdisciplinary team in the designing and delivering of interdisciplinary plan of care.
Establishes therapeutic relationship with patients/families and invites their participation in the plan of care to ensure, as much as possible, their being able to function as independently as possible.
Initiates and performs nursing protocols based on patient's needs relating to physical comfort and mental well-being which include, but are not limited to, positioning, mobilizing, bowel and bladder elimination, skin and wound care, personal hygiene and communication.
Participates in patient/family teaching during daily care relative to understanding the illness and its treatment; maintaining of activities of daily living; self-care and self- medication, prevention and recognition of complications and preparation for discharge and follow-up.
Plans and carries out a program of professional growth including, but not limited to, participating in regularly scheduled in-service programs and appropriate community educational offerings.
Cooperates with education and research activities as assigned.
1. Participates in Nurse LPN/LVN on-call rotation/schedule as assigned in order to meet the needs of patients and families.
2. Is willing to work on flexible schedule, i.e., weekends or rotating shifts.
JOB REQUIREMENTS
Must have and maintain transportation to be used for work with required liability insurance and license.
QUALIFICATIONS
1. Graduate of an approved school of Practical/Vocational nursing.
2. Currently licensed to practicenursing in the state where the VITAS program is located.
3. At least one year of satisfactory nursing experience, at least one year of which is an LPN/LVN in a hospital on an adult medical surgical unit.
4. Equivalent experience may be considered.
RESTRICTIVE GUIDELINES IN CARE DELIVERY
1. Does NOT make initial patient nursing care assessments.
2. Supervised by a Registered Nurse
3. Reports to the applicable Team Manager

As a home care registered nurse (RN), you will ensure hospice patients are comfortable and forge compassionate, meaningful connections with the people in their lives.

WHERE YOU'LL WORK
Each day is different. You will spend most of your time caring for hospice patients wherever they call home, whether that means a private residence or an assisted living/skilled nursing facility. Most VITAS nurses make 4-6 visits each day.
Once a week, you'll visit the local program office to meet with an interdisciplinary team, including social workers, physicians, chaplains, and nurses, who collaborate to provide holistic support for the patients and families we serve.

What You'll Do as a Hospice RN / RN Case Manager:
• Be the primary point of contact for patients and families, guiding them through every step of their hospice journey.
• Deliver and document skilled, hands-on nursing care based on each patient's individualized Plan of Care-in their home, assisted living, or wherever they call home.
• Perform initial and ongoing assessments to monitor patient condition and adjust care as needed.
• Collaborate with an interdisciplinary team-including physicians, LPNs, CNAs, social workers, chaplains, and others-to build and refine personalized care plans.
• Provide emotional, physical, and spiritual support not just to the patient, but to their loved ones as well-offering education, comfort, and bereavement guidance.
• Teach caregivers how to care for their loved one safely and confidently, offering both practical training and emotional reassurance.
• Communicate important updates in patient condition to the care team, ensuring timely and appropriate interventions.
• Maintain accurate, up-to-date clinical documentation and follow infection control and compliance standards.
• Support the organization's quality improvement and scheduling initiatives to ensure care is consistent, compassionate, and efficient.
• Help patients and families understand and navigate topics such as:
• Medication administration
• Hospice philosophy and services
• Symptom and pain management
• End-of-life processes and expectations
About You:
Qualifications - What You'll Bring:
• Active RN license in the state of employment (or eligible to obtain).
• 1+ year of RN experience in hospice, home health, ICU, oncology, geriatrics, or related nursing fields.
• A genuine hospice heart: compassionate, empathetic, and patient-centered.
• Comfortable providing care in diverse settings, including private homes and facilities.
• Strong communication and critical thinking skills in emotionally sensitive situations.

We are seeking a compassionate and experienced Registered Nurse (RN) to join our Advanced Illness Management (AIM) Palliative Care Team. General management, supervision and leadership of the Hospice Home Care Transdisciplinary Teams that support our East Side; knowledge of and commitment to organizational mission, culture, management principles, policies and fiscal stewardship; Hospice representative to medical community and external providers; ensure the delivery of comprehensive, coordinated quality clinical care and service in homes.
WHAT YOU WILL DO:
Coordination of clinical staff in relation to program and clinical services; participation in their selection and evaluation process.
Evaluate facilitators of Care Conferences for Hospice Care Teams; provide guidance to the Team Leaders in identification and resolution of problems and concerns in patient/family care.
Supervise and develop Team Leaders.
Oversee continuing compliance with Medicare and other licensure regulations.
Maintain open communication and networking with all hospice community offices and coordinators.
Participate in quality improvement and utilization management processes pertinent to Hospice services.
Participate in on-call services as administrator on call.
Fiscally responsible for managing the allocation of resources. To monitor utilization of resources and report monthly variances as requested.
Participate in further program planning, development and enhancements in coordination with the Chief Clinical Officer and Professional Relations.
Develop plans to operationalize programs and projects for the Home Care Programs. This will include on-going monitoring, evaluating, utilizing quality improvement tools and system processes.
Conduct presentations and in-services in the community and Nursing Facilities to enhance education of hospice services.
Assist the Chief Clinical Officer with special projects and other duties as assigned.
Participate in defining budgeted centers annually.

Our Clinical Agents (Telephone Triage RN's) provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team in a variety of settings. WHAT YOU WILL DO:
Answer escalated calls from families, hospital staff, nurses' pharmacies, patients etc and providers to begin assessing situation and gaining understanding of the patients specific care needs and begin advising care.
Triage inbound calls for multiple health care professionals and families and provide intervention (be it advise or dispatch)
Demonstrate calm and attentive presence, expert administration of care and a unwavering spirit of compassion
Responsible for relaying updated or new provider contact information to staff and management.
Accurately document inbound/outbound patient information in agency's EMR and associated systems
Provide a next level medical advice to supporting agents by being in a ready status in the Five9 system to assist.
Perform other duties as assigned
SUCCESS CAPABILITIES:
High School Diploma or Equivalent required.
Graduate from an accredited school of professional nursing or a school accepted for candidacy status from an accrediting body. A Registered NurseLicense (RN) in the State of Ohio
2-4 years of clinical support of triage or call experience in medical setting
Prefer a minimum of 3 years recent clinical experience in a physician's office, hospice or palliative care work, home health, critical care and or emergency room
Relevant experience includes direct patient triage, clinical phone triage, demonstrated excellence in clinical assessment using nursing process, or adult/pediatric nursing care in a physician or hospital setting
Excellent verbal and written communication skills

WHEN DO ON-CALL NURSES MAKE A DIFFERENCE?
Your schedule is typically either 8p - 8:30a with an expectation to work every other weekend and every other holiday. This position includes a $3,000 sign-on bonus for FT.
WHAT YOU WILL DO:
