The LPN/LVN administers nursing care according to the plan of care to terminally ill patients admitted to the VITAS program, in accordance with the Nurse Practice Act in the State where the VITAS program is located.
JOB RESPONSIBILITIES:
Provides direct skilled bedside nursing care.
Evaluates patient/family needs and assists in the preparation and execution of the interdisciplinary plan of care for each patient on the team.
Performs selected acts in the care of the terminally ill under the supervision of a registered nurse; such acts include the administration of treatments and medications.
Evaluates effectiveness of treatments and monitors patient's reactions.
Reports significant findings or changes in the patient's condition to the registered nurse, Team Manager or Physician. Documents reports in clinical notes.
Maintains a clean and safe environment for the patient and family.
Maintain accurate documentation including but not limited to clinical notes on interdisciplinary charts.
Evaluates patient/family response to care.
Utilizes a wide variety of supportive and technical nursing skills in carrying out specific medical orders for administration of medication, performing specific treatments, monitoring diagnostic, therapeutic, and supportive measures.
Assists the interdisciplinary team in the designing and delivering of interdisciplinary plan of care.
Establishes therapeutic relationship with patients/families and invites their participation in the plan of care to ensure, as much as possible, their being able to function as independently as possible.
Initiates and performs nursing protocols based on patient's needs relating to physical comfort and mental well-being which include, but are not limited to, positioning, mobilizing, bowel and bladder elimination, skin and wound care, personal hygiene and communication.
Participates in patient/family teaching during daily care relative to understanding the illness and its treatment; maintaining of activities of daily living; self-care and self- medication, prevention and recognition of complications and preparation for discharge and follow-up.
Plans and carries out a program of professional growth including, but not limited to, participating in regularly scheduled in-service programs and appropriate community educational offerings.
Cooperates with education and research activities as assigned.
1. Participates in Nurse LPN/LVN on-call rotation/schedule as assigned in order to meet the needs of patients and families.
2. Is willing to work on flexible schedule, i.e., weekends or rotating shifts.
JOB REQUIREMENTS
Must have and maintain transportation to be used for work with required liability insurance and license.
QUALIFICATIONS
1. Graduate of an approved school of Practical/Vocational nursing.
2. Currently licensed to practice nursing in the state where the VITAS program is located.
3. At least one year of satisfactory nursing experience, at least one year of which is an LPN/LVN in a hospital on an adult medical surgical unit.
4. Equivalent experience may be considered.
RESTRICTIVE GUIDELINES IN CARE DELIVERY

Position Specific Responsibilities
Implements/follows the nursing care plan for each patient.
Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
Initiates preventive and rehabilitative nursing procedures as appropriate.
Observes signs and symptoms and reports to the physician and Registered Nurse Case Manager (RNCM) reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
Evaluates with the Home Health RNCM the effectiveness of the LP/VN's nursing service to the patient and family under the guidance of the RNCM.
Maintains accurate and complete records of observation, treatments, and care of patients.
Participates in medical record audits as assigned.
Attends staff meetings, patient care conferences, and in-services as scheduled.
Is responsible for submitting any changes in schedule to the Director of Clinical Services/Clinical Supervisor on a daily basis; participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical and speech therapists.
Prepares clinical and progress notes and submits them within 24 hours of visit.
Assists the physician and Home Health RNCM in performing specialized procedures.
Prepares equipment and materials for treatments.
Observes aseptic techniques as required.
Assists in patient learning and appropriate self-care techniques.
Ensures that all care is provided with respect for patient rights.
Reports all grievances and complaints made by patients or families to the appropriate persons.
Reports all allegations of patient abuse and/or misappropriation of patient's property as appropriate.
Follows standard precautions and infection control procedures.
Reports all accidents and incidents observed.
Identifies and responds appropriately to emergency situations.
Observes the safety needs of the patients.
Provides documentation of patient's visit on the clinical record accurately, timely, and appropriately per agency policies.
Develops treatment appropriate to patients' disease progression in conjunction with the Home Health RNCM.
Addresses patients' care needs with families and significant others as appropriate.
Maintains confidentiality of necessary information.
Maintains proper documentation for billing per policy.
Interfaces with patients, families, and staff to ensure customer satisfaction.
Communicates effectively with patients, families, and other healthcare providers.
Regular attendance is an essential function.

Education and/or Experience
Certificate of an accredited program of Practical or Vocational Nursing as approved by the state required.
Two (2) years of experience as an LPN in a clinical care setting preferred.
Experience in skilled nursing, preferably in hospice, home health care, oncology or emergency care preferred.
Skills
Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.

Certifications, Licenses, and Registrations
Active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse in the state(s) of employment required.

Provide prescribed medical treatment and personal care services to terminally ill persons in collaboration with the transdisciplinary team in a variety of settings. Coordinate the implementation of the Plan of Care for each patient utilizing the Nursing Process.
WHAT YOU WILL DO:
Responsible to practice in accordance with HWR clinical standards of practice, policies and procedures
Assess physical, psychosocial, and spiritual needs of the patient and family/caregivers.
Provide intermittent skilled nursing services to the patient under the patient's physician's direction.
Monitor and evaluate pain and symptoms and provide information and assistance to the patient and family to manage those problems.
Serve as a liaison to the patient's physician(s) when needed; provide information to the physician based on knowledge relating to the patient's needs.
Provide emotional support to the patient and family/caregiver.
Refer patients and/or family members to other services when appropriate and follow-up as necessary.
Provide instruction (health teaching) to the patient and family.
Maintain and promptly complete all necessary plans and reports related to hospice care.
Participate in care conferences and bereavement follow-up.
Participate in case review, quality improvement activities and other meetings as assigned.
Plan and clinically supervise activities of HNA as related to patient care.
Participate in the on-call rotation as scheduled.
Participate in discharge planning in collaboration with other team members.
SUCCESS CAPABILITIES:
Licensed in State of Ohio
Experience preferred in hospice care, home health, oncology and/or pain and symptom management.
Current CPR certification
Must have and maintain a valid driver's license, good driving record and auto accident insurance at a level acceptable to HWR and/or HWR's insurer
Hospice and Palliative Care Certification desired. Will be required within first two years of hire.
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration.

S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the company, among all departments, and all external stakeholders. The Home Health Licensed Practical/Vocational Nurse administers skilled nursing care to patients on an intermittent basis in their place of residence. This is performed in accordance with physician orders and plan of care under the direction and supervision of the Home Health Registered Nurse Case Manager. S/he services are furnished in accordance with agency policies.
Position Specific Responsibilities
* Implements/follows the nursing care plan for each patient.
* Provides nursing services, treatments, and diagnostic and preventive procedures as assigned.
* Initiates preventive and rehabilitative nursing procedures as appropriate.
* Observes signs and symptoms and reports to the physician and Registered Nurse Case Manager (RNCM) reactions to treatments, including drugs and changes in the patient's physical or emotional condition.
* Teaches and counsels the patient and family/significant others regarding the nursing care needs and other related problems of the patient at home.
* Evaluates with the Home Health RNCM the effectiveness of the LP/VN's nursing service to the patient and family under the guidance of the RNCM.
* Maintains accurate and complete records of observation, treatments, and care of patients.
* Participates in medical record audits as assigned.
* Attends staff meetings, patient care conferences, and in-services as scheduled.
* Is responsible for submitting any changes in schedule to the Director of Clinical Services/Clinical Supervisor on a daily basis; participating in patient care conferences to discuss the need for involvement of other members of the health team, such as physical and speech therapists.
* Prepares clinical and progress notes and submits them within 24 hours of visit.
* Assists the physician and Home Health RNCM in performing specialized procedures.
* Prepares equipment and materials for treatments.
* Observes aseptic techniques as required.
* Assists in patient learning and appropriate self-care techniques.
* Ensures that all care is provided with respect for patient rights.
* Reports all grievances and complaints made by patients or families to the appropriate persons.
* Reports all allegations of patient abuse and/or misappropriation of patient's property as appropriate.
* Follows standard precautions and infection control procedures.
* Reports all accidents and incidents observed.
* Identifies and responds appropriately to emergency situations.
* Observes the safety needs of the patients.
* Provides documentation of patient's visit on the clinical record accurately, timely, and appropriately per agency policies.
* Develops treatment appropriate to patients' disease progression in conjunction with the Home Health RNCM.
* Addresses patients' care needs with families and significant others as appropriate.
* Maintains confidentiality of necessary information.
* Maintains proper documentation for billing per policy.
* Interfaces with patients, families, and staff to ensure customer satisfaction.
* Communicates effectively with patients, families, and other healthcare providers.
* Regular attendance is an essential function.
* Performs other duties as assigned.
Education and/or Experience
* Certificate of an accredited program of Practical or Vocational Nursing as approved by the state required.
* Two (2) years of experience as an LPN in a clinical care setting preferred.
* Experience in skilled nursing, preferably in hospice, home health care, oncology or emergency care preferred.
Skills
* Mathematical Skills: Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals. Ability to compute rate, ratio, and percentage.
* Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from leaders, team members, investors, and external parties. Strong written and verbal communications.
* Other Skills and Abilities: Ability to understand, read, write, and speak English. Articulates and embraces hospice philosophy.
Certifications, Licenses, and Registrations
* Active and unencumbered Licensed Practical Nurse or Licensed Vocational Nurse in the state(s) of employment required.
Physical Demands and Work Environment: The demands of this role necessitate a team member to effectively perform essential functions. * Initiates and performs nursing protocols based on patient's needs relating to physical comfort and mental well-being which include, but are not limited to, positioning, mobilizing, bowel and bladder elimination, skin and wound care, personal hygiene and communication.
* Participates in patient/family teaching during daily care relative to understanding the illness and its treatment; maintaining of activities of daily living; self-care and self- medication, prevention and recognition of complications and preparation for discharge and follow-up.
* Plans and carries out a program of professional growth including, but not limited to, participating in regularly scheduled in-service programs and appropriate community educational offerings.
* Cooperates with education and research activities as assigned.
1. Participates in Nurse LPN/LVN on-call rotation/schedule as assigned in order to meet the needs of patients and families.
2. Is willing to work on flexible schedule, i.e., weekends or rotating shifts.
JOB REQUIREMENTS
Must have and maintain transportation to be used for work with required liability insurance and license.
QUALIFICATIONS
1. Graduate of an approved school of Practical/Vocational nursing.
2. Currently licensed to practice nursing in the state where the VITAS program is located.
3. At least one year of satisfactory nursing experience, at least one year of which is an LPN/LVN in a hospital on an adult medical surgical unit.
4. Equivalent experience may be considered.
RESTRICTIVE GUIDELINES IN CARE DELIVERY
1. Does NOT make initial patient nursing care assessments.
2. Supervised by a Registered Nurse
3. Reports to the applicable Team Manager
