A program senior leadership role responsible for the Admissions Department's leadership, strategy, customers, analysis and knowledge management, workforce, operations and results.
Serves as an Admissions standard, policies, procedures, and industry subject-matter expert and resource.
Position structure
Reports to and managed by the Program's GM
Leadership
Develop nurse, liaison, program scheduler, and other admissions staff leadership skills through utilization of Admissions Training, coaching, mentoring, and modeling in remote and face-to-face interactions on weekly, daily, monthly, quarterly, and annual basis, and as-needed
Collaborate with National, Division, Region, and Program leadership to successfully improve program performance, quality, and growth
Responsible for program's successful rollout and sustainability of national, divisional, and regional initiatives within their program
Lead by example through field coaching, account development, and community activities
Created a positive culture of growth and development
Strategy
Ensure Admissions Department has active and an actionable strategic plan documented in the system
Ensure program Admission Department actively collaborates with other program departments in development of strategies to ensure alignment
Customer
Develop a customer-service oriented Admissions Department by identifying and responding to the customer's needs and wants by recognizing the customer is the center of VITAS business
Review and provide guidance on program-related customer complaints.
Support and develop quality improvement initiatives based on internal and external survey results and time in field
MAKM (Measurement, Analysis, and Knowledge Management)
Admissions-focused process improvement project management for the program

Program oversight and ensured adherence to standards and performance in the areas of:
Training: Field Coaching, manage completion of required and ad hoc trainings, analyze need based on utilization of data tools, complete ongoing professional development, and is capable of group presentations (internal and external)
Staffing: Demand-based scheduling, productivity management, account development during working hours, and schedule creation
Recruitment: Monitor and manage initiatives, regular evaluation of FTE efficiently and with efficacy, ensure timely posting and hiring
Retention: develop, implement, and monitor retention initiatives, provide ongoing staff support, provided regular feedback through field coaching as part of required time in field, and implement company retention activities per standard
Operations
Compliance with AM required duties as here and within training materials
Audits: Daily, weekly, monthly, quarterly and annual
Monthly program, region, division and national meetings
Budget and operation expense and performance management
Results
Support a collaborative approach in achieving program, region, division and national Admissions related goals, KPIs and metrics.
Experience:
5+ years nursing preferred
3+ years in a leadership/management role preferred
Hospice admissions experience preferred
Ability to work on various assignments simultaneously
Knowledge of industry policies and practices
Ability to motivate and develop others
Ability to communicate tactfully, both verbally and in writing, with program leadership, patients, families, managers, coworkers, and vendors to resolve problems and negotiate resolutions
Proficient with technology with ability to adapt to new products and comfortable learning new technologies (i.e. Excel, MS Teams, OneNote, SharePoint, PowerBI, SharePoint, etc.)
Demonstrates experiences of developing strategies and solutions to improve Admissions performance
Ability to assimilate and analyze large amount of data/information to uncover business, quality and training needs within a data set
Education:
Bachelor's degree preferred in nursing or business from an accredited college or university or the international equivalent
Nurse preferred to be licensed in the State of which the programs they cover
Certification & Licensure:
Current RN license preferred

Patient Food Service

Transport patient trays to wings/pavilions for delivery by clinical staff
Prepare and deliver late trays
Breakdown patient trayline after meal service; work with cook to manage leftovers
Wash pots/pans from serving line; clean and sanitize trayline equipment
At designated times, pick up carts with dirty trays from meal service
Wash patient dishes
Follow guidelines for trash, dirty linen and recyclable collection
Sweep and mop floors
Accurately complete all documentation as specified
Provide backup for menu aide, if trained, for preparation and completion of menus and tallies
Menu Aide (if specified)
Accurately maintain dietary patient census, assure correct menu for diet order
Interact with patients and families in the daily selection of menu choices
Accurately tally patient menu selections for hot and cold foods
Catering Service
Set up and break down catering events as assigned
Cafeteria Service
Set up cafeteria line with food/beverages and supplies for meal service needs
Serve in cafeteria according to portion and service standards
Operate POS system; reconcile cash drawer
Clean and sanitize cafeteria serving areas, tables and chairs
Miscellaneous
Answer phone per protocol and make simple diet changes as indicated
Other duties and functions as assigned based upon patient, team and agency needs
SUCCESS CAPABILITIES:
Must be able to read, write and carry out instructions
Minimum of one year healthcare food service experience preferred

Position Specific Responsibilities
• Organizes, tracks, and builds a complete medical record for appropriate patient transition to the home health providers; and confirms start of care (SOC) for each patient referred.
• Responsible for identifying all post-acute care needs and collaborating with the Care Transition Coordinator.
• Collaborates with all Compassus/Ascension at Home intake teams.
• Responsible for completing the transition of care, including submission of complete referral, and, securing outside agency placement if Compassus/Ascension at Home is unable to service the patient.
• Organizes and prioritizes large volumes of information.
• Displays exceptional customer service to our patients/families/referral sources.
• Handles confidential and non-routine information.
• Manages and creates general correspondences, memos, charts, tables, graphs, business plans, etc.

Education and/or Experience
Certificate of an accredited program of Practical or Vocational Nursing as approved by the state required OR Associate or Bachelor's degree in Nursing.

Prior experience with electronic medical records preferred.

Recruit, train, and retain Administrative, Direct Care, Vigil, and Bereavement volunteers.
Facilitate volunteer orientation and annual training requirements.
Ensure completion of health screening requirements (e.g., TB tests, health questionnaires).
Serve as liaison between volunteers and staff to promote strong communication.
Represent the volunteer program at interdisciplinary team meetings and in the community.

Certified Nursing Assistant highly preferred.

WHAT YOU WILL DO:
Patient Food Service
Pull cold items for patient meal service per tally and setup on patient trayline
Set up hot food on patient trayline
Serve hot and cold foods per meal ticket selections and per serving standards
Transport patient trays to wings/pavilions for delivery by clinical staff
Prepare and deliver late trays
Breakdown patient trayline after meal service; work with cook to manage leftovers
Wash pots/pans from serving line; clean and sanitize trayline equipment
At designated times, pick up carts with dirty trays from meal service
Wash patient dishes
Follow guidelines for trash, dirty linen and recyclable collection
Sweep and mop floors
Accurately complete all documentation as specified
Provide backup for menu aide, if trained, for preparation and completion of menus and tallies
Menu Aide (if specified)
Accurately maintain dietary patient census, assure correct menu for diet order
Interact with patients and families in the daily selection of menu choices
Accurately tally patient menu selections for hot and cold foods
Catering Service
Set up and break down catering events as assigned
Cafeteria Service
Set up cafeteria line with food/beverages and supplies for meal service needs
Serve in cafeteria according to portion and service standards
Operate POS system; reconcile cash drawer
Clean and sanitize cafeteria serving areas, tables and chairs
Miscellaneous
Answer phone per protocol and make simple diet changes as indicated
Other duties and functions as assigned based upon patient, team and agency needs
SUCCESS CAPABILITIES:
Must be able to read, write and carry out instructions
Minimum of one year healthcare food service experience preferred
Equivalent education/experience may substitute for minimum qualifications except when there are legal requirements, such as a license/certification/registration

We're looking for a compassionate Hospice Chaplain to provide non-denominational spiritual and emotional support to patients and families facing end-of-life care. As a valued member of our interdisciplinary team, you'll help ensure that no one walks this path alone.
What You'll Do as a Hospice Chaplain:
+ Provide spiritual and emotional support to patients, families, and caregivers in alignment with their beliefs and values.
+ Offer non-denominational, inclusive care that respects all faiths, spiritual traditions, and cultural backgrounds.
+ Serve as an integral member of the interdisciplinary team, participating in patient care conferences and collaborating with nurses, social workers, aides, and medical staff.
+ Assess spiritual needs of patients and families and develop individualized spiritual care plans.
+ Conduct visits, prayers, blessings, and rituals as appropriate and requested by the patient or family.
+ Assist families during times of grief, loss, or transition, and provide bereavement support and counseling.
+ Build relationships with local faith leaders and community resources to support patients' and families' unique spiritual needs.
+ Maintain timely and accurate documentation of spiritual assessments, interventions, and outcomes.
About You
The candidate must meet one of the following educational requirements:
+ Bachelor's degree in theology, religion, human services, counseling, psychology, or sociology (or as required by state-specific regulations)
+ A minimum of one unit of ACPE-accredited Clinical Pastoral Education (CPE)
Note: Degrees from accredited institutions are preferred. If a degree is from a non-accredited college or university, AVPO approval is required prior to hire.

Compensation may vary within the salary range provided based on several factors including but not limited to a candidate's location, experience, education, skills, licensure, certifications and department equity. Gentiva provides associates with a comprehensive benefits and total rewards package, of which base pay is just one piece.
Salary Range: $25.40 - $29.85
