The Team Manager is the leader of the Patient Care Team that ensures that patients and families receive continuity and quality of care from admission to discharge or transfer to bereavement. Salary Range: $130,000- 140,000/ Annually
Patient Care Services
Directs all patient care services for the team, including volunteer and bereavement, and ensures that services are rendered as integrated components of the interdisciplinary plan of care.
Collaborates with both the team physician and the attending physician as needed and acts as the clinical resource for the interdisciplinary team
Participates in all potential alive discharges to ensure effective discharge planning
Oversees the team schedule and coordination of services to insure coverage for all patients on the team.
Fills in and provides patient care within their scope of practice during times of need
Staff Supervision and Management
Interviews, selects, trains, supervises, evaluates and dismisses team staff in conjunction with the Patient Care Administrator
Assures staff competence and performance levels through making field visits, evaluating documentation and providing team based in services on Vitas values, hospice principles and practices and specific team growth needs.
Acts as a resource and mentor for staff re clinical issues, documentation, inter- and intra- team problem solving and appropriate customer service behavior
Oversees staff and volunteer schedules, scheduling and territory assignments to assure that workload is distributed equitably and that all staff are able to meet overall productivity expectations.
Reviews and approves payroll, assuring that employees have documented accurate time, mileage and additional expenses.
Participates in on-call rotation.
Quality Improvement/Regulatory Compliance
Monitors clinical records and all team activities to ensure that they are accurate, complete and meet required standards in accordance with VITAS policy and all regulatory bodies.
Participates in performance improvement activities to respond to service issues and challenges, and documentation issues
Participates in and complies with QAPI program.
Assists as requested by program senior management team with surveys and requests from regulatory bodies and intermediaries
Performs required chart reviews to ensure quality care is being delivered.
Customer Service/Sales/Marketing
Assures that service issues with pa tients/families or internal customers are resolved promptly and satisfactorily.
Analyzes customer service issues on team to identify causes and works with individual team members as well as entire team to improve performance.
Actively participates in potential revocations or service concerns
Regularly visits LTC and Contract Bed facilities to assure care plan integration, customer satisfaction and to discuss criteria for determining appropriateness.
Participates in professional, voluntary or community service organizations
Cost Containment
Verifies treatments, staffing and supplies.
Monitors resources and utilization of clinical operations including but not limited to labor, pharmacy, medical supplies, equipment and mileage.

Professional Development

QUALIFICATIONS
Reliable transportation with appropriate license and insurance coverage for driver and passengers.
Knowledge of the principles and practices of primary medical and nursing care, with at least a working knowledge of oncologic nursing and palliative care.
Basic understanding of the interpersonal dynamics operative within the working relationships of an interdisciplinary health care team, and knowledge of the process of dying and bereavement.
Working knowledge of VITAS information system (Vx) and the ability to utilize management reports.
Two years successful supervisory experience or equivalent in a health care organization.
EDUCATION

Company: Compassus The Care Transition Associate performs a variety of patient-centered administrative tasks to support care coordination within acute care hospital hubs. This care transition work is focused on post-acute care needs for patients requiring home health and hospice . Support patients and families with their specific discharge plan and plan of care for home health and hospice referrals. The Care Transition Associate ensures a high level of customer service and connection between the hospital's patients and post-acute service providers by assisting patients and the clinicians who provide care to access appropriate post-acute care and effectuate a timely start of care.
This role will support care coordination activities furnished within the Providence hospital and develop opportunities for educational outreach that will enhance service awareness and access. Additional duties assigned may include collaboration with local Service Line leadership in essential meetings, reporting on tactical plans, and training/orientation.
The Care Transition Associate position is salaried and will not receive any bonus or compensation related to assisting with admissions to the JV home health agency or hospice. The Care Transition Associate may be assessed for success of achieving Value-Based Enterprise measures.

Ensures services and equipment ordered are appropriate based on clinical criteria and patient diagnosis. Ensures required documentation is present in the patient chart. Obtains accurate charting and MD orders as needed to ensure appropriate insurance coverage.
* Meets with Clinical Liaisons, hospital discharge planners, physician groups and other referral sources maintaining excellent customer relationships, ensuring satisfaction, providing updated materials, and informing of any updates on new or changes in services. Serves as a resource for inpatient care teams for patients discharging from the hospital to home health or hospice service lines.
* Provides in-person education materials and consultation to patients and families regarding Home Health and Hospice. Coordinates delivery of equipment to the patient residence with Home Health Agency's or Hospice's HME and DME vendors as appropriate to avoid hospital discharge delays.
* Communicates clearly to physician and provider offices verifying orders, following provider, pertinent medical and other information applicable to Home Health and Hospice clearly and concisely.
* Provides administrative support for Clinical Liaison team.
* Efficiently navigates EMR; enters patient data and appropriate documentation ensuring a high level of accuracy. Schedules starts of care for patients as needed.
* Responsible for providing necessary documentation to internal and external agencies to ensure patients receive Home Health and/or Hospice care ordered at hospital discharge. Communicates with patient and family to verify pertinent data.
* Supports coordination of referral and reimbursement including notification of clinical teams for any follow up needed on orders and reported changes in patient condition.
* Demonstrates and maintains up to date knowledge and understanding of community resources and payer source criteria for Home Health and Hospice services. Maintains strong knowledge base on all DME equipment and supplies.

WHAT YOU'LL DO
As a hospice social worker, you will provide emotional and logistical support to patients and their loved ones during an important and vulnerable time. You will ensure VITAS patients reach the end of life with their affairs in order, honoring their wishes for finances, funeral arrangements, and more. Their loved ones will trust you to help them navigate the grieving process.
Whether you're granting an end-of-life wish or simply listening to a patient's stories at their bedside, your actions-large and small-will make a meaningful difference in the lives of patients and their families.
WHERE YOU'LL WORK
Each day is different for our social workers. You will spend most of your time caring for hospice patients wherever they call home, which may be a private residence, or an assisted living/skilled nursing facility.
About once a week, you'll come to the office to meet with the rest of your interdisciplinary team: the RNs, physicians, chaplains, and fellow nurses who work together to provide 360-degree support to the patients and families we serve.

JOB REQUIREMENTS
* Ability to travel per job requirements.
* Thorough knowledge of community resources or the desire/ability to obtain this knowledge.
* Knowledge of key psychosocial issues prevalent with terminally ill patients and their family/caregivers, including the knowledge of effective psychosocial inter mentions to address those issues.
* Capacity to relate and work well with others as a member of an interdisciplinary team.
* Excellent listening and communication skills.
* Ability to be an effective patient and family advocate- both within the patient/family unit, within the core IDG, and within community agencies and the like.
QUALIFICATIONS
* Masters of Social Work required from a school accredited by the Council on Social Work Education, unless grandfathered in per the 2008 Medicare Hospice Condition of Participation
* At least one year social work experience in a health care setting.
