TREATMENT PLANS ARE COORDINATED WITH THE PHYSICIAN, THE PATIENT AND THE CAREGIVER TO HELP PATIENTS ACHIEVE THEIR OPTIMAL LEVEL OF FUNCTIONING AND INDEPENDENCE. THE PROGRAM PROVIDES HOME ASSESSMENTS, PATIENT EDUCATION AND SKILLED NURSING TREATMENTS AS ORDERED BY PHYSICIANS. A TELEHEALTH PROGRAM IS PROVIDED TO REMOTELY MONITOR VITAL SIGNS, WEIGHT AND KNOWLEDGE OF THE DISEASE PROCESSES FOR PATIENTS REQUIRING ADDITIONAL SUPPORT ON A DAILY BASIS. IN FISCAL YEAR 2018, THERE WERE 1,822 HOME CARE ADMISSIONS AND 29,743 HOME CARE VISITS WERE PERFORMED.
