A DAY IN THE LIFE OF A PERSONAL CARE ATTENDANT
Under the supervision of the Home Care Aide Staffing Manager, this position is responsible for assisting in the carrying out of a client plan of care. Responsibilities include but are not limited to:
Receives and carries out daily patient assignments.
Collects patient and family data and reports findings to supervisor.
Performs interventions and procedures as directed to ensure the safe effective quality of care for patients.
Performs a variety of patient care tasks including, but not limited to activities of daily living, collection of vital signs, ambulation/exercises, homemaking, etc.
Effectively communicates clinical information-both verbally and in writing-to patients, their representatives, and home care staff, ensuring timely and appropriate reporting.
QUALIFICATIONS:
High School diploma or equivalent required.
Must be 18 years of age.
Prior home care and/or hospice experience preferred.
Valid state driver's license, vehicle and automobile insurance required. Ability to successfully pass pre-employment driver record, criminal, adult and child abuse background check/review required. Must show proof of auto insurance at time of employment.

A DAY IN THE LIFE OF A HOSPICE REGISTERED NURSE (RN)
As a Hospice RN, you are responsible for serving as case managers and for providing individualized goal directed nursing care through the use of the nursing process in accordance with physician's orders. Responsibilities include but are not limited to the following:
Essential functions:
Utilizing the nursing process, assesses patient/care giver needs in order to provide quality care to assigned patients.
Develops and communicates a realistic plan of care for each patient.
Serves as Case Manager for assigned patients and actively manages case loads and visit times in accordance with agency productivity standards.
Implements medical plan of care, nursing interventions, and procedures as necessary to ensure the safe effective quality of care for patients.
Stays up-to-date on a variety of High-Tech skills; IV use and maintenance, wound VAC, complex wound care, etc.
In accordance with LPN scope of practice, works collaboratively with LPN's to ensure that the plan of care is followed, delegates tasks and ensures that patient needs are met.
Consistently reviews documentation, annotates visit notes and acts on recommendations in accordance with department policies and procedures.
Evaluates a variety of aspects of implemented plan of care including, but not limited to identified problems, care provided and patient's responses, to meet patient/care giver needs and ensure overall quality of care delivered.
Maintains up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information for use by the health care team to ensure on-going and continued quality of care.
Performs patient and care giver education to ensure informed patient decisions can be made, including, but not limited to, medication side effects, treatment options, discharge instructions, reportable conditions, ranges of services, etc.
Skills, experience, and qualifications:
Registered Nurse (RN) license
2+ years of nursing experience is preferred
Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency.
Experienced with electronic medical records required.
Willingness to work a flexible schedule to include evenings, weekends, and on call for both hospice and home care patients.
Exceptional bedside manner and communication skills.
Compassionate and friendly demeanor.
Valid Vermont Driver's License and car insurance.

A DAY IN THE LIFE OF A SPEECH LANGUAGE PATHOLOGIST (SLP)
As an SLP, you will evaluate, plan, and administer medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students and SLP's in CFY year and students. Responsibilities include but are not limited to the following:
Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
Performs a comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
Plans for and provides skilled speech-language therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.
Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork.
As necessary, assesses patients' needs for HCA services, develops and coordinates POC with patient/care giver, and provides on-going supervision.
Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed.
Serve as a Home Care ST as required
Qualifications:
Master's degree in Speech Language Pathology required
Current Vermont State SLP license required
Two years of clinical Speech Language Pathology experience required
Active enrollment in clinical fellowship year or successful completion of certificate of clinical competency required
Valid Driver's License

A DAY IN THE LIFE OF A LICENSED PRACTICAL NURSE (LPN)
As a LPN, under the supervision of an RN or Clinical Manager, this position is responsible for providing individualized goal directed nursing care through the use of the nursing process in accordance with physician's orders. Responsibilities include but are not limited to the following:

Transmits patient incidents reports (infections, injuries, etc) to QI Specialist in a timely manner.
Coordinates a wide variety of patient care with physicians, providers, team members, vendors, etc. in support of the recommended plan of care to ensure safe and effective patient and family care.
Pro-actively advocates for patient care issues to ensure that overall quality and type of care is sensitive to each specific patient/family's needs.
QUALIFICATIONS FOR A LICENSED PRACTICAL NURSE (LPN)
Licensed Practical Nurses (LPN) license
Prior clinical acute care nursing experience requited. Prior Home Care Experience preferred.
Willingness to work a flexible schedule, including rotational weekends, holidays and on call.
Valid Driver's License, Vehicle and Insurance. Ability to successfully pass motor vehicle record check, child abuse, criminal record check and adult abuse check required.

SKILLS, EXPERIENCE, AND QUALIFICATIONS:
Successful completion of an accredited Physical Therapy program required.
Two years of clinical Physical Therapy experience preferred. Prior Home Care experience preferred

Experienced with electronic medical records required.

As an RN, you play a key role in assessing the needs of our patients to create and implement their plan of care. A DAY IN THE LIFE OF A SPEECH LANGUAGE PATHOLOGIST (SLP)
As an SLP, you will evaluate, plan, and administer medically prescribed or educationally relevant speech therapy treatment to clients in their homes or students in their respective educational environment. In addition, this position is responsible for clinical supervision of students and SLP's in CFY year and students. Responsibilities include but are not limited to the following:
Actively monitors and amends patient visit schedule to maximize productivity and patient need. Takes action to account for new patients and changes in existing patient schedule, including but not limited to, calling patient to reschedule visits, reshuffling patient load to meet new demands, communicating recommended changes to supervisor, etc.
Performs a comprehensive (OASIS and SLP) assessments of assigned patients including, but not limited to psychosocial, educational, safety and risk factor parameters, as evidenced by clinical documentation.
Develops a plan of care to promote optimal functional independence and safety as documented by clinical record.
Plans for and provides skilled speech-language therapy treatment including, but not limited to, therapeutic techniques, patient/care giver education, etc. assessing the patient's status and progress on each visit and ensuring quality of care as documented in the clinical record.
As Care Manager and/or team member, coordinates patient care with other clinical team members, including but not limited to, physicians, nurses, care givers, other rehab services and students, ensuring optimal patient care and communication as noted in the clinical record.
Accurately and completely performs functions as related to school-based services, including, but not limited to, evaluations, documentation, development of Individual Educational Plan IEP's, participation in meetings, and provision of direct and consultative services. Fills out and completes insurance paperwork.
As necessary, assesses patients' needs for HCA services, develops and coordinates POC with patient/care giver, and provides on-going supervision.
Coordinates safe patient discharges and/or transfers (or upon knowledge of) and ensures all appropriate paperwork and patient information is processed.
Serve as a Home Care ST as required
Qualifications:
Master's degree in Speech Language Pathology required
Current Vermont State SLP license required
Two years of clinical Speech Language Pathology experience required
Active enrollment in clinical fellowship year or successful completion of certificate of clinical competency required
Valid Driver's License

As an RN, you play a key role in assessing the needs of our patients to create and implement their plan of care. Performs a comprehensive speech and language physical, psychosocial, and safety assessment as evidenced by documentation in the clinical record.
Perform initial and ongoing assessments of patient's communication and swallowing status.
Develop and implement individualized treatment plans based on physician orders and patient needs.
Provide direct speech, language, voice, fluency, and swallowing therapy services in the patient's home.
Instruct and educate patients, caregivers, and family members regarding goals and home exercise programs.
Collaborate with interdisciplinary team members (nursing, PT, OT, social work) to ensure holistic and coordinated care.
Monitor and document progress toward established goals and adjust plans as necessary.
Communicate regularly with physicians and other health care professionals regarding patient status and progress.
QUALIFICATIONS:
Master's Degree in Speech-Language Pathology from an accredited program.
Current licensure as a Speech-Language Pathologist in the state of practice.
Minimum of two years of SLP experience preferred; home health experience a plus.
Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) required.
Valid driver's license, auto insurance, and reliable transportation.

A DAY IN THE LIFE OF A SPEECH LANGUAGE PATHOLOGIST
Under the direction of the Clinical Manager, the Speech-Language Pathologist (SLP) provides skilled speech therapy services to patients in their homes as part of an interdisciplinary care team. The SLP evaluates patients, develops individualized treatment plans, and provides therapeutic interventions to address communication disorders, swallowing difficulties, and cognitive-linguistic impairments resulting from injury, illness, or disability. Responsibilities include but are not limited to:
KEY RESPONSIBILITIES:
Performs a comprehensive speech and language physical, psychosocial, and safety assessment as evidenced by documentation in the clinical record.
Perform initial and ongoing assessments of patient's communication and swallowing status.
Develop and implement individualized treatment plans based on physician orders and patient needs.
Provide direct speech, language, voice, fluency, and swallowing therapy services in the patient's home.
Instruct and educate patients, caregivers, and family members regarding goals and home exercise programs.
Collaborate with interdisciplinary team members (nursing, PT, OT, social work) to ensure holistic and coordinated care.
Monitor and document progress toward established goals and adjust plans as necessary.
Communicate regularly with physicians and other health care professionals regarding patient status and progress.
QUALIFICATIONS:
Master's Degree in Speech-Language Pathology from an accredited program.
Current licensure as a Speech-Language Pathologist in the state of practice.
Minimum of two years of SLP experience preferred; home health experience a plus.
Certificate of Clinical Competence (CCC) from the American Speech-Language-Hearing Association (ASHA) required.
Valid driver's license, auto insurance, and reliable transportation.

A DAY IN THE LIFE OF A REGISTERED NURSE (RN)
As an RN, you play a key role in assessing the needs of our patients to create and implement their plan of care. In accordance with evidence-based practices, state and federal regulations, and our company's policies, you serve as a case manager for assigned patients. Responsibilities include but are not limited to the following:
Actively manage the patient's treatment and needs.
Maintain up-to-date and accurate documentation of nursing care provided to ensure the effective integration of information.
Take action to ensure that infection control standards are maintained and stay educated on a variety of high-tech skills, including IV use and maintenance, wound VAC, and complex wound care.
Carry out visits with patients in a timely manner and ensure that patients and caregivers are educated on health matters
Coordinate with a wide variety of physicians, providers, team members, and vendors in order to fulfill the recommended plan of care.
Assist with any patient discharges or transfers and complete all the correct paperwork and reports.
QUALIFICATIONS FOR A REGISTERED NURSE (RN)
Registered Nurse (RN) license
2+ years of nursing experience is preferred
Have self-direction and initiative, as well as be able to work collaboratively with colleagues within the agency.
Experienced with electronic medical records required.
Willingness to work a flexible schedule, including rotational weekends, holidays and on call for both hospice and home care patients.
Exceptional bedside manner and communication skills.
Compassionate and friendly demeanor.
Valid Vermont Driver's License and car insurance.

Responsibilities include but are not limited to:
Volunteer functions encompass those tasks, duties, and responsibilities that are critical and fundamental to the volunteer position and comprise the means of accomplishing the job's purpose and objectives.
Assists team with patient care in accordance with the plan of care.
Provides needed services to patients/families and as outlined in the Hospice Volunteer Handbook (e.g. companionship, shopping, respite, etc.)
Makes a time commitment of 2-4 hours each week as schedule permits.
Provides notification when a substitute is needed.
Uses listening skills effectively to provide emotional support to the patient/family.
Reports and/or records any changes in the patient's condition or environment to the Hospice Volunteer Services Coordinator, case manager, or other appropriate team member.
Provides accurate, timely, and objective documentation of volunteer activity.
Submits annual mandatory paper to maintain active status as a volunteer.
Attends scheduled rehearsals and assigned bed-side or community presentations (if applicable)
Is able to demonstrate competency requirements and attends educational in-services and workshops as schedule permits.
Accountable for and expected to integrate VNAHSR's HEALTH Matters values into the execution of the essential functions of their volunteer work.
