SUMMARY Serves as a liaison between the professional staff, community, hospital, healthcare facilities, physician, physician's staff, elder care attorneys and elder planning financial advisors. Acts as a resource for information to ensure quality care for the patient. ESSENTIAL DUTIES AND RESPONSIBILITIES 1. Complies with agency policies and procedures. 2.
