PACT is an evidenced based multidisciplinary outreach mental program that serves adults with serious and persistent mental illnesses in the community. PACT staff work as a team to collaborate with consumers to develop treatment interventions that assist them in their recovery process.
Responsibilities:
Assist consumers in being fully integrated in their communities by supporting them in the use of community resources, encouraging them in establishing memberships within their communities, and reducing stigma within the community through education. Provide and maintain therapeutic boundaries and a professional relationship with consumers. Actively take part in supervision, staff meetings, in-services and trainings as required or determined by supervisor. Act as Liaison between the courts and correctional facilities. Deliver and monitor consumer medications in accordance with Agency, DMH and DPH regulations, policies and procedures. Ensure the maintenance and accuracy of all documentation and administration. Follow all motor vehicle rules and regulations when driving consumers. Participate in Daily Organizational Meeting (DOM) in accordance with Eliot policies and PACT standards. Document daily progress notes for all contacts with each consumer in accordance with Eliot Policy and PACT fidelity standards. Perform shift management as assigned by the Team Leader in accordance with Eliot policies and PACT standards. Act as a primary case manager to 7-15 consumers in accordance with Eliot's policies and PACT standards.

A DAY IN THE LIFE OF A VOCATIONAL REHABILITATION CASE MANAGER
As a Vocational Rehabilitation Case Manager in human services, you are responsible for developing trustworthy relationships with our clients to monitor their engagement and progress in preparing for and obtaining employment. You communicate with external providers to ensure our customers are making meaningful progress toward employment as well as conduct outreach with customers to encourage engagement and participation in provider activities.
Using your exceptional attention to detail, you review administered assessments, develop detailed individual responsibility plans (DIRPs), and help our customers overcome any barriers preventing them from achieving employment goals. Additionally, you coordinate multiple activities as outlined in our customer's DIRPs as well as help them identify appropriate activities, resources, and support systems. To help customers achieve their goals, you assist them with capturing and recording employment information as well as working toward and obtaining promotions or salary increases.
You also diligently verify and report customer milestone achievements and provide counseling, intervention, or program action planning as needed for continuous improvement. Using a learn-by-doing approach, you guide customers through service planning and delivery as well as assist with job readiness training, searches, retention, and additional educational services. You document all activities, participation, compliance, and employment progress using our electronic case management (ECM) systems. Your social work responsibilities also include compiling data for reports, including customer timesheets, placement information, and retention records.
