PROGRAM
This position is for the Back Home SSVF (Supportive Services for Veteran Families) program which is a Rapid Rehousing program that utilizes a 90-day model to find and stabilize housing for veterans experiencing homelessness or at risk of homelessness. Primarily focusing on housing case management, but also offering wrap-around case management services assisting to connect veterans to Public Benefits, VA benefits, Health referrals, and financial health referrals. JOB SUMMARY
The Case Manager directs and facilitates the delivery of appropriate support services for participant households as indicated by relevant service modalities. Job duties include conducting needs-based and program eligibility assessments, engaging in community outreach, providing case management services, and program-wide service coordination. Case Manager is charged with making demonstrable progress towards obtaining and applying knowledge and experience in field-relevant best practices and standards.
ESSENTIAL DUTIES AND RESPONSIBILITIES
* Provides case management services to Program Clients.
* Responsible for identifying and serving the unique needs of participant households participating in Volunteers of America programs; uses screening and assessment tools to ensure completion of needs-based screenings.
* Responsible for understanding and implementing the assigned program according to program funder and Volunteers of America standards and expectations.
* Conducts community-wide outreach to identify eligible participant households presenting with substantial barriers to housing stability.
* Provides group educational and other activities for current or prospective program clients as assigned.
* Supports team members by serving as a resource for analyzing and solving problems and staying abreast of current issues and theories within the field.
* Attends orientations, trainings, education programs, staff meetings, community meetings, conferences, and workshops as requested and applicable to meet the needs of the position.
* May provide training and mentorship to team members and community stakeholders regarding best practices in relevant service models and practices.
* Promptly and clearly documents all client interactions along with required eligibility and demographic information using designated data bases.
* Performs duties in a professional manner by maintaining the confidentially of all information and by participating effectively within and across teams.
* Participates in professional development activities to promote the development of knowledge and experience in field-relevant best practices and standards and makes demonstrable progress towards working in accordance with these practices and standards.

* Bachelor's degree in human services, social work, or a closely related field or related experience.
* One year of direct experience in the provision of human services.

* Must possess a personal vehicle that may be used for work-related travel (reimbursement for mileage is available).
* Must complete agency and program credentialing within 12 weeks of hire and maintain credentialing standards thereafter.

JOB SUMMARY
The Supervisory Case Manager oversees advocacy, crisis intervention, support services, and coordination with funding sources. Ensures that their assigned program meets or exceeds program objectives, supervises the provision of high-quality services for program participants, and ensures ongoing compliance with funder requirements and agency policy and procedure. Responsible for recruiting, training, supervising, and scheduling staff, volunteers, and interns. Generally, will supervise up to 3 direct reports.
Supervisory Case Manager will also be responsible for conducting outreach in conjunction with the team and all aspects of case management. Acts as a knowledgeable liaison to community agencies and is responsible for promoting awareness of the Volunteers of America resources and services. The position requires ongoing interaction with team members, program participants, and community stakeholders.

You will assist clients within the Psychiatric Urgent Care unit who have co-occurring disorders, substance use disorders, or other mental health urgent care needs, in obtaining functioning and quality of life, by proving clinical services and education.
Your primary focus is to support clients within the outpatient program with obtaining services such as medication assisted treatment, intensive outpatient, therapy services, and relapse prevention groups as well as referrals to community resources needed based on individual client needs.

AGENCY OVERVIEW
CPCS is the state agency in Massachusetts responsible for providing an attorney when the state or federal constitution or a state statute requires the appointment of an attorney for a person who cannot afford to retain one. The agency provides representation in criminal, delinquency, youthful offender, family regulation, guardianship, mental health, sexually dangerous person, and sex offender registry cases, as well as in appeals and post-conviction and post-judgment proceedings related to those matters.

The office will set standards of practice and create training to increase the capacity for removal defense representation within Massachusetts.
POSITION OVERVIEW
The Immigration Impact Unit is seeking a full-time Social Worker as part of the Massachusetts Access to Counsel Initiative (MACI). Under the direction of the Supervising Attorneys, the Social Worker will support MACI attorneys in their direct representation of individuals in removal proceedings to achieve the best possible legal and life outcomes for their clients. The Social Worker helps identify the social and clinical issues that may be at issue in the removal proceedings or any underlying criminal record.
The Social Worker's role includes bio-psychosocial interviewing and assessment, social history investigation, forensic case management, advocacy for clients within court, detention, mental health, and social service systems, fostering family interactions and intervention, treatment and release planning, report writing, testifying, and making direct connections between clients and programs. Social services intervention occurs at all stages of the removal proceedings process.
Please note, these positions are contingent on grant-funding from the Governor's Office for Immigrant and Refugees. Funding for this position is only guaranteed for the duration of the grant period. While it is anticipated that additional funding may be awarded, that is not guaranteed.

Responsibilities
MAJOR DUTIES
1. Patient Care and Documentation
Functions as a full-time integrated team member in the primary care clinic using the Primary Care Behavioral Health (PCBH) model of service delivery. Screens and assesses patients, and performs PCBH model appropriate biopsychosocial evaluations using expert knowledge of general and specific concepts, principles, procedures, and practices relating to clinical, clinical health, and primary care psychology. Provides targeted assessment and evaluation of functional status focused in the context of the presenting problem. Conducts brief assessments and formulates evidence-based interventions appropriate to a primary care setting for a wide range of behavioral health conditions, chronic medical conditions, and health behaviors affecting medical conditions. Provides evaluation, intervention and relapse prevention services in appointments lasting no more than 30 minutes for initial and follow-up visits. Uses a stepped-care approach to maximize delivery of care within the PCMH. Triage and refers patients to Specialty Behavioral Health Care when services required exceed the BHC scope of practice. Assists with implementation and monitoring of treatment plans with Primary Care Managers (PCMs) and other primary care team members. Use objective and measurable outcomes to evaluate treatment effectiveness and make adjustments if appropriate for increased adherence and efficacy. Provide high level expertise in human behavior as a primary care team member to develop and support specific clinical pathways or best practice programs for targeted patient groups (e.g., patients with acute stress symptoms, chronic medical conditions, insomnia). Provides concise documentation of care and recommendations in patient's medical record beginning in the appointment and completed in no more than 72 hours after the patient is seen.
2. Consultation and Team Based Care
Provides timely and succinct feedback to PCMs regarding consultation findings and recommendations as directed by the current DHA PCBH Administrative Instruction and DHA Procedures Manual. Determines if patient is appropriate to receive PCBH model services in the primary care setting. Provides ongoing consultation services for a subset of patients who would benefit from periodic, regular behavioral health monitoring and intervention for sustaining improvement and preventing relapse (i.e., continuity consultation). Participates in all PCMH activities to include huddles, provider meetings, peer review, and trainings. Actively works with PCMH team to identify patients who are appropriate for BHC services and facilitates processes to increase same-day “warm handoff” of patients from PCMs to BHC. Works to increase overall penetration into the enrolled clinic population by targeting common conditions with behavioral components that are amenable to BHC intervention. Performs daily activities to increase BHC clinical productivity (e.g. scrubbing PCM appointment lists, circulating in the clinic and interacting with staff, being vocal in morning huddles etc.)
3. Educational Activities
Develops, teaches, and provides oversight for classes that promote education and skill-building to enhance psychological and physical health, and promote behavioral change related to improved health status. Conducts trainings for the primary care clinic to increase referrals and promote understanding of the role of the BHC.
