About the job:
The primary purpose of the VOA Case Manager role is to provide specialist, end-to-end case management of residents, with the aim of supporting the resident to take responsibility for completing activities aimed at addressing their rehabilitation and reintegration needs and ultimately to reduce the likelihood and seriousness of re-offending. The Case Manager will operate within case management frameworks which are aligned to reducing re-offending operating principles and frameworks for rehabilitation and reintegration.
Essentials:
The role includes all facets of case management from initial assessment, identification of resident needs and case planning incorporating specialist rehabilitation and reintegration interventions and services, as well as facilitating links with the resident's support groups and external providers.
Develop an enhanced reintegration plan.
Proactively remain informed on resident progress and events.
Encourage resident motivation through the use of a range of techniques and tools including evidence-based practice, motivational interviewing.
Monitor resident progress against the plan and regularly review resident plans ensuring all rehabilitation and reintegration interventions and services are provided to the resident according to the standards set for standard and enhanced case management.
Develop and maintain effective networks to assist with reintegration of residents.
Ensure the reintegration needs of residents are met prior to discharge to the community, including preparation of individual reentry plans in consultation with service providers.
Liaise with BOP, DOC Probation Officers on a regular basis to facilitate "seamless" transitions to the community.
Provide timely support and advice to Program Service Providers.
Assist in completion of risk and needs assessment of residents including identification of needs and priorities and recommending residents for either standard or enhanced case management as recommended by the assessment instrument.

The primary duty of a VOA High School Administrative Professional / Program Specialist will be to oversee and manage non-academic, operational functions of Volunteers of America High School while maintaining federal, county, and city compliance. Building Operations
Manage existing building contracts for repeating services by scheduling work, reviewing work and maintaining ongoing communication with vendors.
Engage new vendors when required and secure and schedule additional services as needed.

Complete reimbursements and purchasing documents for VOA High School.
Order and distribute office supplies, printing paper, teacher requested materials and manage budget associated with materials.

About the Job:
The MDS Coordinator is responsible for planning and organizing the MDS/RAI process in coordination with the RAI/PPS Coordinator. This position assures compliance with all State and Federal MDS transmission requirements. The MDS Nurse is responsible for collaborating with Clinical and Administrative staff regarding issues relative to the Resident Assessment Instrument (RAI) process.

Works with Interdisciplinary team to determine resident significant change in status and OMRA assessments.
Completes Nursing Sections of MDS per facility policy and ensures completion of assigned sections by other disciplines.
Follows and updates facility MDS schedule and tracking forms per policy and procedure.
Follow facility policy and procedure on Resident Care Planning.
Ensure that care plans accurately reflect the cares and clinical monitoring provided.
Audits completion and review of completed MDS assessments randomly on each floor monthly.

The Cook provides quality food service in accordance with all State and Federal regulations and meets the nutritional and individual needs of the residents.
Essentials:
Completes defined food preparation, serving of meals and cleaning duties according to proper procedures, using various dietary equipment and supplies, as assigned by the Director of Dietary and Management Team.
Plans and initiates cooking schedule for food preparation to meet meal schedule.
Prepares, seasons, cooks and serves meat, main dish, soup and vegetable for assigned meal;
ensures appropriate portioned servings according to portion control standards.
Prepares nutritional snacks for residents as assigned.
Tastes and smells prepared food to determine quality and palatability.

Key Responsibilities:
• Team Leadership: Select, train, and support Resident Support Specialists, ensuring 24/7 coverage and offering ongoing training to meet the needs of both staff and residents.
• Resident-Centered Care & Compliance: Ensure all residents are properly supervised and supported, adhering to Bureau of Prisons and Volunteers of America MN policies. Oversee medication management, resident activities, and ensure proper documentation of all interactions.
• Health & Safety Oversight: Maintain a clean, safe, and healthy environment for all residents by managing food services, sanitation, and safety protocols. Conduct regular inspections to ensure the facility meets all health and safety standards.
• Supportive Environment: Perform room inspections, ensure personal belongings are handled with care, and partner with Case Managers to assist with resident transitions. Conduct random checks to ensure the environment remains safe and respectful.
• Crisis & Emergency Response: Lead emergency preparedness efforts, including fire drills and other safety procedures. Share on-call duties to provide round-the-clock support for residents and staff.
• Operational Excellence: Maintain accurate and up-to-date logs, support resident check-ins and check-outs, and manage facility supplies to ensure a well-functioning environment for both residents and staff.

Retired Seniors Volunteer Program (RSVP), sponsored
by Volunteers of America-Minnesota, is looking for an energetic team member to
coordinate our Wellness program. Programs include our Fit & Strong! and
Bone Builders initiatives. Fit & Strong! is a low-cost, evidence-based,
multiple component exercise program. It combines flexibility, strength training
and aerobic walking with health education for sustained behavior change among
older adults with lower extremity osteoarthritis (OA). Bone Builders is a free,
osteoporosis prevention exercise class that uses weight training and balance
exercises. This is a
PAID
position, 30 hours a week and has flexible hours
dependent on program needs.
As a program leader, job responsibilities include:
• Recruiting, training, and managing volunteer
class leaders
• Working with host sites for classes
• Maintaining paperwork, including ensuring that
documents are following HIPAA compliance guidelines
