Alignment Health is seeking a bilingual Spanish care coordinator located in North Carolina to join the remote Care Anywhere (CAW) team.
As a care coordinator, you will supports the CAW field providers with scheduling, outreach, and managing all care coordination needs for high-risk members enrolled with the CAW program.
Schedule: Mondays - Fridays
- Option 1: 8:00 AM - 5:00 PM Eastern Time (with 1-hour lunch)
- Option 2: 8:30 AM - 5:00 PM Eastern Time (with a 30- minute lunch)
General Duties / Responsibilities:
Manage (4) provider schedules ensuring that schedules are filled.
Prepare charts for upcoming home visit appointments (check member eligibility, gather records needed by the provider prior to the home visit)
Conduct outreach for scheduling, appointment confirmation calls, wellness checks for high risk members, and to providers / pharmacies for member needs.
Daily inbound / outbound call quota of minimum 60 - 80 calls / day.
Obtain medical records from provider offices, hospitals, and skilled nursing facilities (SNF's) and upload medical records to electronic medical records (EMR.)
Submit referral authorizations to independent physician associations (IPA) / medical groups for specialty, durable medical equipment (DME), and home health (HH) services.
Coordinates lab orders and transportation for high-risk members.
Documentation via EMR for inbound / outbound calls.
Support SMS and member outreach campaigns.
Assist nurse practitioner (NP) team with visit preparation needs:
Appointment reminders to members
Assign members to NP in electronic health records (EHR)
Provide needed documentation to NP for visits each day
Direct inbound calls from members / family related to medication refills
Assist with maintaining and updating members' records
Assist with mailing or faxing correspondence to primary care physician (PCP's), specialists, related to, as needed.
Attend CAW meetings / presentations and participates, as appropriate.
Recognize work-related problems and contribute to solutions.
Work with outside vendors to provide appropriate care needs for members

Experience:
Required: Minimum 1 experience entering referrals and prior authorizations in a healthcare setting.
Preferred: Minimum 2 years' healthcare experience.
Education:

Preferred: Completion of a Medical Assistant program from an accredited institution.

The Client Services Representative is responsible for all aspects of follow-up on each client telephone and electronic encounter. SUMMARY OF JOB PURPOSE AND FUNCTION The primary purpose and function of the Shift Lead is to perform the duties of a CSC and/or veterinary assistant/technician, as well as provide continuity and operational support across all shifts, ensuring that the hospital always has leadership onsite to support associates and clients. ESSENTIAL RESPONSIBILITIES AND TASKS The ratio of time spent on leadership duties and paraprofessional responsibilities will vary depending on the hospital situation; the Shift Lead must use good judgment in prioritizing their duties on any given day, ensuring hospital productivity. On average, the ratio is expected to be about 20% leadership duties (primarily when the Practice Manager and/or Chief of Staff are not present) and 80% paraprofessional duties. Perform the duties of a CSC and/or veterinary assistant/technician, which may include the following: Actively recruit new clients by promoting hospital services. Primary Purpose of Position: This position provides nonclinical administrative support to the PartnersACCESS call center. The Call Center Representative primary responsibility is to answer inbound calls and assist callers by connecting them to the appropriate party, sharing information, providing technical assistance, answering questions, handling and/or resolving complaints. Must maintain a high level of professionalism, patience and empathy working with callers who may be frustrated and may have complex medical, behavioral health, intellectual and other developmental conditions; and must still maintain the highest level of customer satisfaction by seeking first call resolution.

Role and Responsibilities:
Ability to learn complex information about two Medicaid health plans and their benefits.
Work in a call center environment and interact with callers who are generally members and providers, to deliver information, answer frequently asked questions, and address complaints.
Route calls to the appropriate resource. Including appropriately identifying and elevating those more complex or crisis calls.
Understand Health/Mental Health (MH)/Substance Use (SU)/Traumatic Brain Injury (TBI)/Intellectual/Developmental Disability (I/DD) treatment needs, benefit information and referral of members calling to determine if they may potentially qualify for services.
Review call notes, enrollments, registrations, or other identified documents for completeness and/or accuracy.
Collect and enter demographic data into the electronic record, completion of appropriate forms, explanation of services, benefits and resources, verifies Medicaid and dispatch.
Provide follow up calls.
This position demands a high level of accuracy and confidentiality. Information must be handled according to NC standards and rules, state and federal laws and LME/MCO and NCQA standards, procedures, policies and protocol.
Trained on the requirements, policies and procedures of the BH I/DD Tailored Plan operating in North Carolina and can respond to all areas within the Member Handbook and Provider Manual, including resolving claims payment inquires in one touch.

Screenings are completed using standard and specialized computer programs.
Inputs accurate information into the system and unlocks electronic service records with appropriate consents, enters all necessary data elements into data systems.

Preferred candidates will already have completed a Certified Peer Support Training Program; however, successful completion of the certification is required within 6 months of hire.
Maintains peer support certificate per state standards. CPI and CPR required on a yearly basis for those working on an inpatient unit.
Previous personal experience with the treatment of psychiatric illness is preferred.
Previous experience working with the SPMI population preferred.
Must have good therapeutic communication skills. Ability and willingness to work as a team member.
Must have good communication skills and the ability to read and write.
Must have computer skills sufficient to document in an electronic medical record. If driving is required by department the defensive driving class is required to be completed annually.

Access Coordinators are the point of contact for all incoming calls for Integrated Family Services during business hours on Monday through Friday. They are responsible for answering the incoming calls for all office locations in a timely and professional manner while assisting callers in reaching the appropriate department, staff, and/or service. They will utilize customer service skills and techniques in determining the need of the caller to ensure that individual receives the best quality of service. Access Coordinators will briefly screen to determine the best appropriate action to occur at the time of the call. They will ensure that if a client is experiencing a crisis that they are linked to the Mobile Crisis Department for further assistance. Must possess the following knowledge/skills: strong knowledge of available community resources; psycho-educational skills; strong individual and group counseling skills; strong assessment skills; strong community integration skills; strong family/caregivers training and consultation skills; strong mentoring skills; strong mediation skills; strong adaptive skill training in all functional domains including vocational, educational, personal care, domestic, social, communication, leisure, problem-solving, etc.; strong behavioral crisis and modeling intervention skills; strong motivational interviewing skills; competency in Wellness Education and symptom management issues, cognitive behavioral therapy interventions and cultural competence.
WORK DUTIES AND TASKS:
Responsible for answering all incoming IFS calls and assisting individuals in reaching the appropriate department, staff, and/or service.

Completes annual adult CPR and First Aid certification courses.

QUALIFICATIONS, EDUCATION AND EXPERIENCE REQUIREMENTS:
A graduate of a college or university with a Master's degree in a human service field and has one year of full time, post graduate degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has one year of full time, post graduate degree accumulated supervised experience in alcoholism and drug abuse counseling; or
A graduate of a college or university with a bachelor's degree in a field other than human services and has four years of full time, post bachelor's degree accumulated mh/dd/sa experience with the population served, or a substance abuse professional who has four years of full time, post-bachelor's degree accumulated supervised experience in alcoholism and drug abuse counseling.
A minimum of one year's experience in providing crisis management services in the following setting: assertive outreach, assertive community treatment, emergency department, or other services providing 24/7 response in emergent or urgent situations AND twenty (20) hours of training in appropriate crisis intervention, strategies within the first 90 days of employment.
No criminal convictions of child abuse or violent crimes.
Must be able to pass background MVR and sexual offender check.
