Responsibilities
Administers and documents medications/treatments as directed on the medication administration record (MAR) and treatment administration record (TAR) per physician orders.
Orders all necessary medication and treatments from appropriate pharmacy or notifies responsible party when supply refill is needed.
Responsible for receiving and verifying all medicine received is accurate according to pharmacy label and MAR. Reports any discrepancies to supervisor.
Responsible for proper storage of all medications and dating of appropriate vials or bottles at time of initial opening.
Participates in the development and updating of each resident's service plan.
Provide personal care and general nursing care to residents as needed or assigned.
Perform vital signs, B/Ps, weights and other trained procedures as directed by Supervisor.

Qualifications
Must be 18 years of age or older
Must read, write, speak, and understand English.
High school diploma or GED, preferred.
Required Registered Medical Aide (RMA) Certification through the Virginia Board of Health Professions.
Previous experience with geriatric nursing preferred.

This may include bathing, grooming, personal and dental hygiene, bowel and bladder functions, dressing, hair care, escorting to and from meals and activities and tidying residents room
• Support team members as they encourage personalized and meaningful resident participation in life enrichment programming and ADLs
• Assist with meal service in dining room. Coach team members on hospitality standards during meal service
• Accurately document care and services provided to residents. Review care manager documentation throughout shift. Identify and bridge gaps in documentation by coaching team members
• Ensure compliance with Sunrise's Timekeeping and Meal/Rest Period policies. Monitor team member break schedule and duration; ensure care managers clock in/out using correct job codes. Coach team members as required
• Display and encourage flexibility in work schedule by working holidays, evenings, weekends and additional shifts when necessary
• Prepare for shift by obtaining resident group assignments before care managers arrive. Print and hand out daily assignment sheets to appropriate team members
• Participate in crossover meetings by sharing/receiving pertinent information with/from other Lead Care Managers
• Carry a resident group assignment on overnight shifts and pick up group assignments in the event of an unfilled call off on day and evening shifts
Other Responsibilities
• Cross trained and credentialed (including certifications where required by state) to safely administer medications if needed during times of high volume or unexpected absence
• Lead by example when clocking in/out and taking proper meal/rest breaks
• Participate in group interview process including job preview video and community tour
• Support onboarding of new team members including shadowing and skills demonstration
• Maintain a safe and secure environment for all staff, residents and guests by following established safety standards; actively support safety practices
• Report incidents and complete appropriate paperwork immediately
• Communicate any observed or suspected resident change of condition to the department care coordinator; assist care coordinators, when required, in development and/or updates to ISPs and communicating with residents and families
• Prevent and handle conflict by working with the team towards a solution. Communicate issues to department care coordinator when necessary
• Contribute to care manager annual performance review by providing feedback to department care coordinator
• Project a positive, professional and friendly image through action, words and dress
Qualifications:
• High School diploma or GED strongly preferred and may be required per state regulations
• Ability to read, write, communicate effectively with Care Managers, Community Managers, residents and family members
• In states where appropriate, must maintain applicable certifications
• Must be at least 18 years of age
• Medication Management Certificate required; ongoing training and re-education per Sunrise and state requirements
• Previous experience working with seniors and desire to serve and care for seniors

What You'll Do:
The Hospice Aide provides compassionate personal care and homemaker services for patients when appropriate and maintains a safe and healthy environment as directed by the IDT plan of care.
More Specifically:
Assist patients with appropriate techniques to maintain personal hygiene and grooming, including shower, tub or bed baths and mouth, skin, nail and hair care.

Position Overview - Hospice Aide:

• Serve as a member of the patient's clinical care team

• Perform personal care needs specific to the patient including if needed: baths/shampoos, oral hygiene, dressing, feeding assistance including food preparation, assist with ambulation, transfer/positioning, assisting with bedpans/urinals, and changing bed linen and light laundry needs
• Observe the patient and the patient's condition and requests, report these observations, and document observations and care performed
• Provide positive communication within the home with ethical and respectful care, including confidentiality
• Take and record oral, rectal, and auxiliary temperatures, pulse, respiration, and blood pressure when ordered with appropriate completed/demonstrated skills competency

Assists with the administration of prescribed medications within the established scope of practice.

Completes all other duties as assigned.
West Virginia AMAP only: Assists with administration of medications within the established scope of practice. AMAP direct supervisor regarding all medication practices is the RN. All new medications or medication changes must be reviewed with the RN before administration.

Watercrest Richmond is looking for an Assisted Living RMA/Med Tech to join their Resident Care team! As a key member of the team, you will ensure the day-to-day medication management program of our residents is provided in an accurate, sensitive, professional and effective way in accordance with current regulations, The Assisted Living Med Tech may also perform day-to-day activities of care giving for the residents and participates in the Dining and Community program to ensure the best quality of care is provided for all residents.
ESSENTIAL JOB FUNCTIONS:
Distribute prescribed medication to residents and maintains related medical records under supervision of the Resident Wellness Director and maintains compliance with the RIGHTS of medication
Presents medication to resident as required by the MOR and observes ingestion or other application, or assists in the administration of medication, using specified procedures
Takes vital signs or observes resident to detect response to specified types of medications and prepares report or notifies designated personnel of unexpected reactions
Documents reasons prescribed drugs are not administered, held, changed, disposed of, discontinued or returned on the MOR
Review and participate in 24 Hour Reports, Daily Logs, Shift Cross-over Reports and verbal reports as needed
Monitors medications to ensure adequate accountability measures are taken when medications are ordered, received into the community, administered and at change of shift as required by policy and procedures
Respects the resident's rights regarding medication
Notifies the Wellness Nurse (Licensed Practical Nurse) or Resident Wellness Director (Director of Nursing) of unauthorized medication held by the resident
Seeks advice from the Wellness Nurse or Resident Wellness Director prior to administering any medication that does not seem correct
Understands and can explain the medication dosage/pharmacology when necessary
Refrains from giving any PRN medications as required by law (if applicable by state)
Complete incident and accident reports and documentation timely
Report changes in resident condition to RWD or Nurse designee immediately
Refrains from providing any treatment that does not fall under their specific licensing
Assist residents with their personal care and activities of daily living as assigned and as requested by residents
Perform daily housekeeping tasks as assigned using established procedures and standards
Respond to resident emergency calls
Assure that established infection control practices are maintained
Report all hazardous conditions and equipment to the supervisor
Assure the equipment is cleaned and properly stored at the end of each shift
Report all accidents and incidents as soon as possible to your supervisor, no matter how minor
Maintain the confidentiality of residents' personal care information
Honor the residents' personal and property rights
Must be in company uniform and resident ready at all times
KNOWLEDGE, SKILLS AND ABILITIES:
Able to communicate effectively with all levels of management, team members, residents, family members, vendors, referral sources and outside contacts
Able to make independent decisions
Must be able to communicate in a warm, friendly and caring manner
Must possess a passion to work with and around senior citizens
Knowledge of customer service principles and practices
EDUCATION REQUIREMENTS:
High School Diploma or GED
RMA License as required per state regulations
CPR certification / First Aid certification
EXPERIENCE REQUIREMENTS:
Experience as a Resident Caregiver or Med Tech preferred
PHYSICAL REQUIREMENTS:

Occasionally lift / carry up to 50 pounds
