Washburn Center's case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. Our case managers advocate on behalf of the child and family's needs and coordinate care across multiple providers. These services are provided for children age 3 through 17. Additionally, this role is a highly collaborative function, and you will experience exposure to all programs at Washburn Center and work in collaboration with the various experienced professionals at Washburn to ensure the best care for the children and families in your care. The case management professional will be actively present in the community and the home life of the client. We encourage independent working for all our case managers to allow for the best client care and services. ROLE RESPONSIBILITIES:1. You will be managing a caseload of 17-18 children or youth and their families with all client service expectations that accompanies this caseload.2. At Washburn Center for Children we are committed to provide culturally based, individualized targeted Children's Mental Health Case Management to children/students and families in their home, schools and community based on each family's needs. 3. You will coordinate mental health and other health related services. The purpose of this involvement in the child and family's life is to improve or maintain child/student's emotional and behavioral functioning at home and in the community, to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage, and to reduce the risk of out-of-home placement, risk of maltreatment, or the need for protective services. 4. You will be expected to complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes and case management authorizations. DAY TO DAY ACTIVITIES:Treatment - Case Managers will provide services to educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill, as well as the following services:Assessment - for your clients you will complete a functional assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.Service Plan Development - You will be developing an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis. Our compassionate, experienced therapists support children and their families through challenging times by offering individual therapy to help with a wide range of concerns, including school adjustment problems, depression, anxiety, difficulty dealing with loss or family changes, abuse, traumatic events, attention problems, parent-child conflicts and acting-out behaviors. ***All school-based clinicians will be expected to maintain monthly services consistent with budgeted quota expectations, an average of 15 billable sessions per week.
Ancillary Services - Clinicians will also provide ancillary services as agreed upon with the school staff and the Washburn Center supervisor, including:* Conduct child group therapy, skills training, and/or prevention groups* Provide care coordination, including collaboration with parents, school staff, and other agencies* Participate in classroom observations* Provide teacher consultation and training* Attend collaboration and planning meetings with school staff, program directors, and grant coordinators to review the status of the program and manage referrals in order to facilitate program development and the coordination of the program at the school
Documentation - Clinicians will be expected to maintain required documentation as specified in our Clinical Policy Manual, including:* Diagnostics assessments* Treatment plan for each client* Daily progress notes* Completion of screening instruments (SDQ) and any other required outcome instruments* Discharge summaries* Timely record keeping of all clinical activities and grant related services as defined* Timely response of communications from the program director, the supervisor, or program support to ensure clear communication throughout the program
All clinicians are to ensure Minnesota and HIPAA data privacy requirements are followed.
Supervision and Professional Development - Clinicians are expected to:* Attend weekly supervision and team meetings, present case information and formulation, and identify questions for team meetings and supervision* Receive regular clinical supervision as required under Rule 29* Provide clinical and administrative supervision for interns as approved by the program supervisor based on licensure and experience and assist with providing training, feedback, and learning experience for program interns* Maintain required CEUs As Washburn Center for Children serves the community it is important that our therapists are able to demonstrate cultural humility and responsiveness in their therapeutic process and in their interpersonal relationships. The Senior Program Developer, Wilder Research works closely with the Program Development Team, Advancement Office, and Wilder Research leadership/project leads to provide strategic leadership to the Wilder Research division. Specifically, this position will develop and implement a full-scale development effort to support select Wilder Research projects and initiatives, including the Minnesota Compass project and the Minnesota Homeless study.
This role is fundamental to achieving our mission to champion stability and wellness through direct services, advocacy, and research by sustaining and increasing research revenue for key projects. In particular, the Senior Program Developer manages, sustains, and grows a funding portfolio of no less than $1,250,000 on an annual basis to support an identified portfolio of Wilder Research projects. The Senior Program Director reports to the Director of Program Development and is part of the Advancement Team, which also includes individual giving, communications, and volunteer engagement.

Job Qualifications

Position summary The Acute Response Therapist II provides intensive support to children and adolescents who are temporarily staying at Children's Minnesota Hospital (St. Paul), often in the emergency department or other units, while awaiting safe placement or additional services. The therapist will carry a caseload of 5-10 clients and deliver comprehensive, culturally responsive, wraparound care for both youth and their families.
Services may be short-term (10 sessions or fewer) or extend over several months of community-based support, depending on client needs. Support may take place in the hospital, at home, in schools, within the community, or through telehealth. Services include assessment, crisis intervention, therapy, parent education and skills training, care coordination, case management, bridging to longer-term providers, and connection to community resources.
The goal of this position is to help children and families transition safely from the hospital back into their homes and communities, reduce the likelihood of future crises or repeat hospital visits, and strengthen overall stability. The therapist will work collaboratively with families and providers to create safety plans, coordinate ongoing care, and ensure each child has the support they need to thrive in the long term. Under direction of the Rehabilitative Services Clinical Manager, the Peer Support Services Clinical Supervisor will provide supervision, as well as program and project leadership to the Peer Support line of service and its staff. This role is responsible for providing clinical leadership and day-to-day oversight of client services provided by Peer Support Specialist staff, which include treatment plan goal setting, teaching of skills and personal and/or family advocacy, and community resource sharing. The Senior Program Developer, Wilder Research works closely with the Program Development Team, Advancement Office, and Wilder Research leadership/project leads to provide strategic leadership to the Wilder Research division. Specifically, this position will develop and implement a full-scale development effort to support select Wilder Research projects and initiatives, including the Minnesota Compass project and the Minnesota Homeless study.
This role is fundamental to achieving our mission to champion stability and wellness through direct services, advocacy, and research by sustaining and increasing research revenue for key projects. In particular, the Senior Program Developer manages, sustains, and grows a funding portfolio of no less than $1,250,000 on an annual basis to support an identified portfolio of Wilder Research projects. This is achieved through identifying, building, and sustaining relationships with government agencies, foundations, and corporations to bring in revenue through grants and other means.
The Senior Program Director reports to the Director of Program Development and is part of the Advancement Team, which also includes individual giving, communications, and volunteer engagement.

Job Qualifications
Must reside in the Twin Cities metropolitan region and be permanently authorized to work in the United States. Participate as a member of a Therapeutic Teaching Model (TTM), a co-lead multi-disciplinary team in a Level IV education setting in the West Metro. Wilder is recruiting a Mental Health Practitioner working towards Mental Health Licensure (supervision towards licensure will be provided) to provide therapeutic interventions in the classroom, to develop and teach Social Emotional Learning lessons, partner and model interventions with the school staff, provide mentalization, validation and labeling of emotions with students and work to understand the meaning of behaviors. Services are provided in the classroom with weekly opportunities for consultation with interdisciplinary team and discussions throughout each week. Provide group mental health treatment in school setting during the academic calendar and in community for summer social groups. In the summer and during breaks, group and individual services may be provided in the community and at home. Collaborate with individual/family therapist and school staff to help integrate treatment across providers. Preferred experience working with early childhood or elementary age children and their families. Must be developmentally oriented, attachment and trauma informed. Telehealth services have been and may continue to be provided.

Job Qualifications
Mental Health Professional (LICSW, LMFT or LPCC) license preferred, but Master's Level Practitioners working towards licensure are encouraged to apply
A strong understanding of attachment theory, child development and trauma treatment is most useful. Day treatment or milieu experience with young children is also helpful.
A passion for serving individuals and families in our community, and in providing trauma informed, culturally responsive mental health treatment. A high regard and respect for education professionals and schools.

About the Role:
Join a passionate early childhood education team at the Wilder Child Development Center in making a positive impact on families through a whole family focus. At Wilder we are proud to be a Head Start co-education site, NAEYC-accredited, and hold a 4-star Parent Aware rating, ensuring our commitment to excellence. As a valued team member, you will actively engage in caring for children ages 16 months to 5 years using best practices, create a safe and positive environment, and interact to promote development. You will also assist in planning activities that support children's learning and growth. Support the Lead Teacher by observing children's behavior to identify developmental needs, potential abuse or neglect, and cognitive or social delays. Assist in documenting assessment outcomes. Maintain compliance with DHS Rule 3, NAEYC, Head Start, certification, and safety standards and procedures.
Qualifications:
Early Childhood coursework, CDA or Associates degree preferred, HS diploma or GED required. Previous early childhood experience preferred. A commitment to working with children and families through a whole family approach. Willing to learn and be trained. CPR or First Aide certified preferred. Must be at least 18 years of age and able to meet MN DHS Rule 3 teacher qualifications.
