Washburn Center's case managers work collaboratively with families and across multi disciplines internally, to develop a care plan and ensure access to needed services, including mental, social, educational, health, vocational and recreational assistance. Our case managers advocate on behalf of the child and family's needs and coordinate care across multiple providers. These services are provided for children age 3 through 17. Case Management is a unique field which will provide you with an expansive understanding of family mental health. As such this role is well suited for Social Work experienced individuals or Mental Health qualified Professionals. Additionally, this role is a highly collaborative function, and you will experience exposure to all programs at Washburn Center and work in collaboration with the various experienced professionals at Washburn to ensure the best care for the children and families in your care. The case management professional will be actively present in the community and the home life of the client. We encourage independent working for all our case managers to allow for the best client care and services. ROLE RESPONSIBILITIES:1. You will be managing a caseload of 17-18 children or youth and their families with all client service expectations that accompanies this caseload.2. At Washburn Center for Children we are committed to provide culturally based, individualized targeted Children's Mental Health Case Management to children/students and families in their home, schools and community based on each family's needs. 3. You will coordinate mental health and other health related services. The purpose of this involvement in the child and family's life is to improve or maintain child/student's emotional and behavioral functioning at home and in the community, to help the family access needed resources for the child including obtaining or maintaining third-party insurance or Medical Assistance coverage, and to reduce the risk of out-of-home placement, risk of maltreatment, or the need for protective services. 4. You will be expected to complete timely documentation and planning including Individual Family Community Support Plans, Progress Notes and case management authorizations. DAY TO DAY ACTIVITIES:Treatment - Case Managers will provide services to educate and support families to become empowered to better self-advocate and navigate the various systems with more confidence and skill, as well as the following services:Assessment - for your clients you will complete a functional assessment of the medical, social, educational, and other services and resources the child will need to address their mental health needs.Service Plan Development - You will be developing an Individual Family Community Support Plan (IFCSP) based on the functional assessment and diagnosis. Clinical Treatment - Clinicians will provide a combination of the following school-based services within the school, clients' homes, and across the community, as agreed upon with the school staff and the Washburn Center supervisor, including:* Conduct diagnostic assessments* Develop and implement a treatment plan with the family based on a diagnosis* Provide individual and family therapy* Participate in IEP meetings* Care coordination, including collaboration with psychiatrists and other professionals working with the child* Create a discharge plan
***All school-based clinicians will be expected to maintain monthly services consistent with budgeted quota expectations, an average of 15 billable sessions per week.
Ancillary Services - Clinicians will also provide ancillary services as agreed upon with the school staff and the Washburn Center supervisor, including:* Conduct child group therapy, skills training, and/or prevention groups* Provide care coordination, including collaboration with parents, school staff, and other agencies* Participate in classroom observations* Provide teacher consultation and training* Attend collaboration and planning meetings with school staff, program directors, and grant coordinators to review the status of the program and manage referrals in order to facilitate program development and the coordination of the program at the school
Documentation - Clinicians will be expected to maintain required documentation as specified in our Clinical Policy Manual, including:* Diagnostics assessments* Treatment plan for each client* Daily progress notes* Completion of screening instruments (SDQ) and any other required outcome instruments* Discharge summaries* Timely record keeping of all clinical activities and grant related services as defined* Timely response of communications from the program director, the supervisor, or program support to ensure clear communication throughout the program

Supervision and Professional Development - Clinicians are expected to:* Attend weekly supervision and team meetings, present case information and formulation, and identify questions for team meetings and supervision* Receive regular clinical supervision as required under Rule 29* Provide clinical and administrative supervision for interns as approved by the program supervisor based on licensure and experience and assist with providing training, feedback, and learning experience for program interns* Maintain required CEUs As Washburn Center for Children serves the community it is important that our therapists are able to demonstrate cultural humility and responsiveness in their therapeutic process and in their interpersonal relationships. Other primary accountabilities include, recruitment and hiring, providing supervision and training to unlicensed staff, non-clinical staff, staff working towards licensure within their role, clinical trainees, and occasionally volunteers. Job Qualifications
Associate's degree required (Bachelor's preferred), with 3-5 years of relevant nursing experience, and at least 1 year experience in training, leading, and supervision of direct service staff. MN Nursing Licensure. Experience in geriatric needs and issues, home health, customized living or long term care setting preferred. Must be self-directed and detail oriented with the ability to differentiate between independent nursing decisions and those requiring delegation from physician. Strong organizational skills, problem-solving ability, active listening and excellent communication skills. Manage time well and perform a variety of tasks. Wilder is recruiting a Mental Health Practitioner working towards Mental Health Licensure (supervision towards licensure will be provided) to provide therapeutic interventions in the classroom, to develop and teach Social Emotional Learning lessons, partner and model interventions with the school staff, provide mentalization, validation and labeling of emotions with students and work to understand the meaning of behaviors. Services are provided in the classroom with weekly opportunities for consultation with interdisciplinary team and discussions throughout each week. Provide group mental health treatment in school setting during the academic calendar and in community for summer social groups. In the summer and during breaks, group and individual services may be provided in the community and at home. Collaborate with individual/family therapist and school staff to help integrate treatment across providers. Family Supportive Housing is dedicated to creating, designing, and implementing community-based services through a proactive, holistic, and person-centered approach. We currently have several full-time openings in our Rapid Re-Housing and Transitional Domestic Violence Programs, which serve individuals and families who are either experiencing homelessness or are at imminent risk of becoming homeless.
Housing Case Managers provide culturally responsive and trauma-informed case management services. Their responsibilities include conducting intake and assessments, developing and monitoring individualized service plans, documenting client progress, and facilitating both virtual and in-person visits in homes, schools, community settings, and courts. Case Managers collaborate with partner agencies and community resources to reduce barriers and support housing stability in alignment with FHPAP and HUD requirements. They maintain complete, accurate, and up-to-date case records and interpret reports from external providers.
Depending on the specific role, Case Managers may also identify market-rate housing options, administer housing subsidies, conduct housing inspections, facilitate or coordinate client education groups, and complete daily documentation, along with other assigned duties. This is primarily an in-office position, with the potential to transition to a hybrid schedule following successful training and demonstrated competence. Job Qualifications
Candidates should possess a Bachelor's degree in the Human Services field, along with at least one year of relevant work experience. However, those with at least 4 years of closely related experience may substitute this for the degree requirement.
Preference will be given to candidates who know about homelessness, mental health, chemical dependency, domestic violence, community resources, and the process of navigating housing systems. A two-year degree in Social Work, Psychology, Sociology, Human Services, or a related field, combined with at least two years of relevant experience, is also acceptable.
Applicants must hold a valid Minnesota driver's license, provide proof of insurance, and have access to a reliable vehicle that meets the Wilder Foundation's driver policy requirements.
The ideal candidate will be able to work both independently and collaboratively, understand supportive housing models, and possess the skills to build strong, lasting relationships and deliver high-quality customer service. Under direction of the Rehabilitative Services Clinical Manager, the Peer Support Services Clinical Supervisor will provide supervision, as well as program and project leadership to the Peer Support line of service and its staff. This role is responsible for providing clinical leadership and day-to-day oversight of client services provided by Peer Support Specialist staff, which include treatment plan goal setting, teaching of skills and personal and/or family advocacy, and community resource sharing. The position may include occasional direct care or interaction with clients to support the Peer Support Services team, and to help support the service area's needs when necessary. Other primary accountabilities include, recruitment and hiring, providing supervision and training to unlicensed staff, non-clinical staff, staff working towards licensure within their role, clinical trainees, and occasionally volunteers.
